The Colorado Department of Health hereby notifies Eagle County Board of County Commissioners
that, effective 1/l/93 , the maximum amount of reimbursement for the WIC Program contract FAA
WIC 930648 covering the period October 1, 1992 through September 30, 1993 is hereby increased
by six thousand. eight hundred seven Dollars ($ 6,807 ) to a new total of thirty two thousand, four
hundred twenty seven Dollars ($ 32,427 ). Section 1 of Paragraph 23 of the contract is changed to
read:
Allowable administrative costs will be reimbursed upon the basis of actual documented "out-of-pocket"
expenditures or a maximum of $ 6,405.00 per quarter for the period 10/1/92 - 12/31/92 ; and a
maximum of $ 8,674.00 per quarter for the period 1/1/93 - 9/30/93 ; whichever is less.
Please sign and return all copies of this notification. A copy will be returned to you when it has been
fully executed.
This Letter of Agreement to the contract is intended to be effective January 1, 1993, but in no event
shall this Letter of Agreement be deemed valid until it shall have been approved by the State Controller
or such assistant as he may designate.
