I am in contact with Luiza Petrovska of LKP
Engineering, Inc. She has informed that we must first get a topographical
survey and percolation test done before establishing an engineered plan for
the site. Therefore I am sending the application anyway to get things stared,
and will be in contacted with the county as soon as I have an engineered
plan.

I am writing to you as a follow-up to our phone conversation of March 12, 2008, regarding the
Individual Sewage Disposal System which was installed on the property located at 0054 McCoy
Road, McCoy, CO. As stated during our conversation, you indicated that the system was installed
prior to you purchasing the property, and that to the best of your knowledge the system is
functioning properly.
However, as I advised during our conversation the county still has an open permit file for the
system, and there is no indication that an inspection was ever been done to determine its
efficiency. At this point what we would like to have from you is written documentation that states
the system is functioning properly and if possible a drawing indicating where the system and
leach fields are located. This information would be placed in the file, and we could then
permanently close it.

Illegal installation/repair of a septic system.

11/09/00 13:10 P/R reports that Mr. Vigil has been attempting to repair his failed septic system. He has
been in contact with a contractor, Bob Ford, who explained to Mr. Vigil where a new system would
need to be located along with the cost. Mr. Vigil did not obtain and ISDS permit and installed the new
septic system during the night within the past two months. Mr. Vigil is attempting to sell the property
and needed the septic repaired prior to the sale. Mr. Vigil does not live on the property, however, rents
to Tom & Tawnie Hiller.
13:30 ISDS records do not show a permit being issued for this property. Janet Kohl, Env. Health
Div. has had communication with Mr. Vigil during October, 2000 regarding a septic repair. Two
facsimile transmittals from Ms. Kohl to Mr. Vigil are attached to and made part of this case.
11/13/00 14:15 could not tell if there was any recent excavation. Yard is small -narrow. House set way
back - no room in front - all driveway. Dumpster on site, building material inside - Jo Ellen Nash -
Realtor - for sale at driveway entry - house & rear yard fenced. No way to access property w/o door
knock & permission. /rz/
11/17/00 Notice of violation sent - due 12/18/00.
12/21/00 Janet Kohl, Env. Health advises that the property has now sold to new owners. She is working
with them and an engineer to resolve this problem.
No further enforcement action required at this time.
