Enclosed is your ISDS Permit No. 2028-00.. It is valid for 120 days. The enclosed copy of the
permit must be posted at the installation site: Any changes in plans or specifications
invalidates the permit unless otherwise approved. Please note any special requirements
that may have been added to the design by this department.
Systems designed by a Registered Professional Engineer must be certified by the Engineer
indicating that the system was installed as specified. Eagle County does not perform final
inspections on engineer designed systems. Your TCO will not be issued until our office
receives this certification.
Permit specifications are minimum requirements only, and should be brought to the property
owner's attention.
This permit does not indicate conformance with other Eagle County requirements. Please

As requested, we observed the installation of the onsite wastewater system (OWS) for the subject site
on September 15, 2000. The system was designed under our Job No. 12965W, dated August 31, 2000.
The system includes the installation of a 1250-gallon; two -compartment, precast concrete septic tank
with an effluent filter on the outlet, followed by a 1250-gallon two -compartment precast concrete septic
tank with a pump in the second compartment. Four chamber -trenches were installed using serial
distribution, as indicated on Figure 1. The first two trenches have one row of seven Infiltratofrm
chambers and the last two trenches have one row of 12 infiltratofrm chambers, for a total infiltrative area
of 589 square feet. An as -built drawing is included as Figure 1.
The construction foreman and homeowner, Rick Mandell, may activate pumping system at his discretion.
The components of the OWS were installed in conformance with our plans and specifications.

The lot is located in a moil residential area in Eagle County, Colorado. There is an e')dging residence with
two wood barns and a drain field of unknown dimensions. The existing residence and dnin field will be
abandoned. The proposed drain field area is essentially flat. There is an existing asphalt drive in the
proposed field area, which will be removed prior to installation_
PROPOSED CONSTRUCTION
A four-bedroorn residence is proposed at the location presented on Figure 1. 1n accor ante with the
Individual Sewage Disposal System (ISDS) regulations, the sewage loading for a four boor m residence is
at 600 Gallons Per Day (GPD) average. The sewage loading with a peaking factor of 1.5 is f i0o GpD. This
loading includes washing machines and garbage grinders. The residence v+iIl be served water by an otasite
well, 'which is located 100+ feet from the -proposed dry field.
SUBSURFACE CONDITIONS
We performed a subsurface investigation by digging three percolation boles, 34 to 41 inches deep, and one
profile pit at the locations indicafed on Figure 1- Subsurface conditions at profile-1 consisted of 6 inches of
asphalt and road base, underlain by sandy clay with scattered gravel to 2 feet, underlain by said with gravel
and cobbles to the maximum depth explored of 8 feet. Into ground water or bedrock wass, encountered.
Percolation test results indicate percolation rates of 24 to 60 minutes per inch {IvII'r}_ The average
percolation rate is 41 MPI_
RECOMMENDATIONS
The OWS should include the installation of two 1000-gallon, two -compartment septic I anks with an
effluent filter added to the outlet ofthe first tank and a pump in the second compartment oft second tank

Three chamber -trenches must be installed, each with one row of 13 chambers, as pred on Figure 1,
Serial distribution is proposed for the drain field, An evaluation of the LTAR. with a medit m sand requires
750 SF of drain field, with no reduction taken. We are designing the drain field b on an average
percolation rate of 41 MPt which requires 1153 SF of area. A 5(°,/o reduction was taken to bring the
required area to 577 SF_
Movement over the drain field trench bottom during construction should be kept al a minimum to
prevent compaction of the receiving soils_ The bottom of each trench should be scarif ied with a pick,
shovel, or backhoe bucket teeth prior to the placement of chambers.
OPERATION AND MAINTENANCE
The owner must realize an OWS is different from public sewer service, and must assume a responsibility
for maintenance of the OWS. The system is relatively maintenance free, but the owner muE t have the septic
tanks pumped. In the absence of a monitoring program, the septic tank should be pumped and the effluent
filter cleaned every two years. There are daily considerations, such as not putting plastic or other nonbiode-
gradable material into the OWS. Water use must be monitored so toilets are not allowed t run when seals
malfunction. To illustrate the point, a running toilet can consume in excess of 1.000 GPA. loading of 1000
CPD could flood and irreparably harm the OWS,
The loading from the backwash of a water softener may be harp dW to the OWS_ A separa:t drywell should
be constructed for the backwash waste, if a softener is installed. Chemically treated water crri a swimming
pool or spas should not be iutroduced to the OWS,
LB TTATIONS
An OWS design requires installation by a contractor who is experienced in its installation. r investigation,
layout, design and recommendations are based on data submitted. If subsurface conditions considerably
different from those described in this report are encountered, we should be notified to evalL ate the effect on
the proposed OWS. If the health department makes modifications to this design, we shoult be contacted to
evaluate the impact on the performance ofthe OWS.
All construction is to be in accordance with the ISDS regulations. Pipe type and size, buil al requirements,
septic tank construction, and other specifications, which are not depicted in our report, aze to conform to
the requirements of the ISDS regulations. The installer of the system is to have downstrati A knowledge of
the Eagle County ISDS regulations and requirements.

Dear Mr. Mandell,
As requested, we performed a subsurface investigation at the subject site. The purpose of our investigation
was to determine subsurface conditions and perform percolation rates for an Onsite Wastewater System
(OWS).
S>rrE CONDITIONS
The site is located in a rural area along Emma Road in Eagle County, Colorado. The lot has two existing
wood barns and an existing residence. A new residence is planned to the northeast of the percolation holes.
The location of the profile pit and percolation holes is presented on Figure 1. The site is relatively flat with
native grasses and sparse trees.
SUBSURFACE INVESTIGATION
A subsurface investigation was conducted by digging one profile pit and three percolation holes 21 to 29

In regards to a conversation with Laura at Eagle County Health Department, we would like to make the
following clarifications to the OWS design for the subject site.
1. We recommend a M-gallon dose per pump cycle.
2. The first septic tank must be a 1000-galion single-comparinwrit precast concrete septic tank
3. We believe the percolation rate of 41 minutes per inch (MPQ for the receiving soils at the
subject site is `higher' than normally found. We have based our design on the percolation rate
and not the LTAR. Based on our observations we feel that our design is suitable far the soil
conditions found on site.

Dear Mr. Mandell,
As requested, we performed a subsurface investigation at the subject site. The purpose of ourinvestigation
was to determine subsurface conditions and perform percolation rates for an Onsite Wastewater System
(OWS).
SITE CONDITIONS
The site is located in a rural area along Emma Road in Eagle County, Colorado. The lot has two existing
wood barns and an existing residence. A new residence is planned to the northeast of the percolation holes.
The location of the profile pit and percolation holes is presented on Figure 1. The site is relatively flat with
native grasses and sparse trees.
SUBSURFACE INVESTIGATION
A subsurface investigation was conducted by digging one profile pit and three percolation holes 21 to 29
inches deep at the locations indicated on Figure 1. Subsurface conditions in the profile pit consisted of 6
inches of gravelly fill, underlain by sandy clay with gravel to 2 feet, underlain by sand with gravel and
cobbles to the maximum depth explored of 8 feet. No ground water was encountered. Percolation tests
indicate percolation rates of 24 to 60 minutes per inch (MPI). The average percolation rate is 41 MPI.
Percolation Test Results are attached as Table 1.
