SITE VISIT: 10/16/01- During a visit to the residence, Officers Hale and Rigoli were unable to contact
the home owner and left a business card.
ACTION: 10/17/01- During a telephone conversation with Officer Hale and Ray Merry, the
owner(Ms.Hazard) was informed of the seriousness of a failing septic system and told of
her responsibility to correct the problem. To aid in determining the status of.the septic

Brief Description or Additional Information: The contractor has informed me
that, in order to transport a 1.500 gal. concrete septic tank into Kimberly's
backyard, they would have to construct a substantial earthen ramp for the truck.
Therefore, the contractor requests permission to install a polyethylene tank.
I have included specs for the proposed 1500-gal. tank. Based on these specs,
HCE is willing to accept the tank as a viable alternative. Please review and call
me with your response.

Subject: Percolation Testing, Proposed Septic Disposal System, 0253 Elk Range
Drive, Eagle County, Colorado.
Dear Ms. Hazard:
As requested, a representative of Hepworth-Pawlak Geotechnical, Inc. conducted
percolation testing at the subject site to evaluate'the feasibility of an.infiltration septic
disposal system, We understand that the existing system is considered too close to the
drainage easement and an addition is being proposed for the residence. The results of
our work are presented in this report. The work was done in accordance with our
agreement for professional services to you, dated September 1, 2000. .
A profile pit and three shallow backhoe pits had been excavated at the locations shown
on Fig. 1. Percolation test holes were hated dug in the bottom of the backhoe pits. The
test holes .were soaked one day prior to testing. The soils exposed in the percolation
holes are similar to those exposed in the profile ypit and consist of about 11/2 to 2 feet of
topsoil overlying -stiff t � I 3' depth o eet.
Groundwater was not observed mithe prome pit and the soils were slightly moist.
percolation tests were performed on September 7, 2000 by a representative of
Hepworth-Fawlak Geotechnical., Inc. The percolation test results are presented on
Table 1. The percolation test results indicate an infiltration rate between 9 and 10
n1imnes per inch with an average of 10 minutes per inch. - Based on the subsurface
conditions observed and the percolation test results, the tested area should be suitable
for a conventional inf&Tataon septic dispOsal system,

Dear Ray:
On September 18, 2002, High Country Engineering personnel observed the construction of
the ISDS for the Hazzard Residence located at 0253 Elk Range Drive in Eagle County,
Colorado. One 1500-gallon pvc septic tank with baffle, one 1000-gallon conrete dosing tank
and 45 standard Infiltrator units had been installed in a bed configuration. Minimal
backfilling had taken place. The accessory dwelling unit had not been connected to the
sewer main. The contractor was advised to install an effluent filter, to bring all tank lids to
finish grade, and install two air -vent inspection wells on the easterly end of the absorption
bed due to excessive depth. Otherwise, the installation of the system was in conformance
with the intent of the design. If you have any questions, or need additional information,
