The system consists of a Valley Precast 1,250 gallon, two compartment septic tank (item #1250T-2CP) equipped with an effluent screen in the outlet tee and750 square feet of soil treatment area (prior to taking the 30%adjustment for using chambers)via the installation of 44 Infiltrator Quick 4 Plus Standard chambers arranged in two shallow trenches with22 chambers per trench.Each trench will receive equal amounts of effluent from a four hole Tuf -Tite distribution box with speed levelers. NOTE:Inspection portals must be placed at both the front and distal ends of each trench and the maximum trench depth must remain less than 2' to meet vertical separation requirements to bedrock. The well must be installed at least100' from the soil treatment area and shown on the record drawing as meeting this horizontal setback. Contact Eagle County Environmental Health and the design engineer at least48 hours of requesting inspections.All inspections necessary to certify proper installation must be performed by the design engineer.System certification along with photos and a record drawing are required to be submitted to, and approved by, Eagle County Environmental Health prior to the use of the system and occupancy of the cabin. Box 1672 Eagle, Colorado 81631 bruce.r.hagedorn@gmail.com Project No. 18-7-212 Permit No. OWTS-4-18-19004 Subject: As-Built Evaluation of Onsite Wastewater Treatment System, Proposed Cabin, Forest Road No. 701, Near Piney Lake, Township 4 South, Range 81 West, Eagle County, Colorado Dear Bruce: A representative of H-P/Kumar observed the installation of the Onsite Wastewater Treatment System (OWTS) for the proposed seasonal cabin at the subject site as required by the Eagle County Environmental Health Department. Our observations and recommendations are presented in this report. The services are supplemental to our agreement for professional services to WindsWest, Inc. dated March 21, 2018. H-P/Kumar previously designed the OWTS for the proposed cabin as submitted in a report dated July 25, 2018, Project No. 18-7-212. The system was designed for four bedrooms. The OWTS was installed under Eagle County Permit Number OWTS-4-18-19004 by WY Construction. We performed our As-Built evaluation August 24, 2018. The OWTS components observed included: sewer line, cleanouts, septic tank, effluent line, distribution box and soil treatment area (STA) and the locations of these components. The installation and locations of the system components were verified as shown on Figure 1. Pictures of the OWTS components made during our site visit are also included with this report. The distance locations to the tank shown on Figure 1 were made by taping. The cleanout between the cabin and the septic tank was moved into the crawlspace to prevent damage from snow off the roof. A C900 pipe was used to sleeve the water line from the cabin for 25 feet due to encroachment on the setback to the water line. The distance to the water line was 5 feet. The STA was reconfigured to three trenches of 14, 15 and 15 chambers from the designed configuration of two trenches of 22 chambers each. The new configuration maintained the required 44 chambers. The changes to the original design area acceptable.
