Install the OWTS exactly as depicted in the LKP Engineering, Inc design dated November21 ,2017,
stamped and signed by Luiza Petrovska, P.E.The system is designed to serve the 4 bedroom main single
family residence.
The system consists of a 1,250 gallon two compartment Valley Precast septic tank followed by 2,000 feet
of soil treatment area (prior to 30%reduction for using chambers)via 117Quick4 Plus Standard Infiltrator
chambers arranged in five shallow, serially connected, trenches as shown on the engineer design, with
inspection ports installed on both ends of each trench.Gently rake the trench bottoms and sidewalls
prior to placement of the chambers.
Contact the design engineer well in advance to conduct all inspections necessary to certify the system
and to prepare the record drawing prior to backfilling.System certification along with the record drawing
and photos are required to be submitted to, and approved by, Eagle County Environmental Health prior to
the use of the system or occupancy of the residence.

Be aware that changes in the use of your property or alterations of your building may require

For the soil treatment area at the secondary residence, we recommend that the trenches
are excavated 4 feet, through the topsoil and blocky clay and into the sandy gravel with cobbles
and boulder. We also recommend that one (1) foot of concrete sand be placed in the bottom of each
trench. The infiltrator chambers will then be constructed on top of the sand filter.

NOTE:Prior to installation and in order to validate this permit, the design engineer must observe a
profile test pits down gradient of the proposed location of the soil treatment area to confirm soil type and
provide documentation to Eagle County Environmental Health.Changes to the original design will
require an amended design followed by and amended OWTS permit.

For the soil treatment area at the secondary residence, we recommend that the trenches
are excavated 4 feet, through the topsoil and blocky clay and into the sandy gravel with cobbles
and boulder. We also recommend that one (1) foot of concrete sand be placed in the bottom of each
trench. The infiltrator chambers will then be constructed on top of the sand filter.
