Thankfully the work was completed under the direction of a professional engineer with experience in Onsite Wastewater Treatment Systems, so final certification will not be as difficult had it not been. However, plans for the work that was completed, including calculations, site plan, and a description of the problem and the resolution, must be prepared by the design engineer and submitted along with the additional $400 for a major repair. Please reference your address and permit OWTS-016666-2019 when resubmitting the application and fee. Please submit the additional fees along with the stamped work of a the design engineer to remove reg flag hold on your property, that would hold any further work on the property. Additionally, installing an OWTS outside of the permitted work constitutes a Class 1 petty offense and the installer may lose their license if this is not corrected. A separate warning notice will be sent to the installer, John Baskins for installing an OWTS with out a permit. Let me know if you have any questions.
