Grzesiak, PE. The system is designed to serve the proposed 3 bedroom residence and a possible two bedroom ADU for a total of 5 bedrooms. The OWTS consists of a 1,500 gallon two-compartment Valley Precast septic tank with an effluent filter in the outlet tee, followed by 840 square feet of soil treatment area via three shallow trenches each with 15 ARC 36 chambers, for a total of 56 chambers. Install inspection ports on both ends of each trench. Contact Eagle County Environmental Health and the design engineer well in advance of requesting inspections prior to backfilling any component of the OWTS. The design engineer is responsible for conducting all inspections necessary to certify the installation and assure the functionality of the system. System certification, along with photos and a record drawing is required to be submitted to and approved by Eagle County Environmental Health prior to the use of the system or occupancy of the dwelling. Additional information about the maintenance of your septic system needs can be accessed through our website links, provided on the Environmental Health Department’s septic system resource page. Be aware that changes in the use of your property or alterations of your building may require commensurate changes to, or relocation of, your septic system. Landscape features, trees with taproots, irrigation systems, and parking areas above the soil treatment area can cause a premature system failure. It is equally important that you notice and immediately repair dripping faucets and hissing toilets as this will certainly cause the system to fail. If you have any questions regarding this information, please contact us at (970) 328-8755 and reference the OWTS septic permit number. Where soil, geological or other conditions warrant, greater distancesmay be required by the local board of health or by the Water Quality Control Commission pursuant to section 25-8-206, C.R.S. and applicable regulations. For repair or upgrading of existing OWTSwhere the size of lot precludes adherence to these distances, a repaired OWTS shall not be closer to setback features than the existing OWTS, as reviewed and approved by the local public healthagency. Components that are not watertight should not extend into areas of the root system of nearby trees.1. Includes infiltration galleries permitted as wells by the Division of Water Resources.2. Crossings or encroachments may be permitted at the points as noted above provided that the water or wastewater conveyance pipe is encased for the minimum setback distance on eachside of the crossing. A length of pipe shall be used with a minimum Schedule 40 rating of sufficient diameter to easily slide over and completely encase the conveyance. Rigid end caps of at leastSchedule 40 rating must be glued or secured in a watertight fashion to the ends of the encasement pipe. A hole of sufficient size to accommodate the pipe shall be drilled in the lowest section of therigid cap so that the conveyance pipe rests on the bottom of the encasement pipe. The area in which the pipe passes through the end caps shall be sealed with an approved underground sealantcompatible with the piping used.3. Add eight feet additional distance for each 100 gallons per day of design flows between 1,000 and 2,000 gallons per day, unless it can be demonstrated by a professional engineer orgeologist by a hydrologic analysis or the use of a barrier, consisting of a minimum 30 mil PVC liner or equivalent, that contamination will be minimized. If effluent meets Treatment Level 3N and thelocal public health agency has a maintenance oversight program in accordance with section 14.D. of this regulation, the distance addition is not required. Flows equal to or greater than 2,000 gallonsper day must be hydrologically analyzed for flow, velocity, hydraulic head, and other pertinent characteristics as means of estimating distances required to minimize contamination as part of theDivision site application process. 1517 Blake avenue, suite 101 Glenwood Springs, CO 81601 970-945-8676  phone 970-945-2555  fax www.hceng.com An Employee-Owned Company MEMORANDUM To: Claire Lewandowski, Eagle County Environmental Health Department From: Michael Grzesiak, PE Date: November 10, 2020 Project: Bogan Residence – 15200 Cottonwood Pass Road Subject: OWTS Construction Approval Letter, Permit No. OWTS-019678-2020 The proposed OWTS design at the Bogan Residence, 15200 Cottonwood Pass Road, Eagle County, has been installed per the approved and permitted plans. The installation contractor was Holmes Excavation; 970-963-2007. Two site visits for system installation inspection were conducted on November 2, 2020, and November 10, 2020. Soil test pits were dug and tactile tests were performed August 31, 2020. The chambers were trenched down to the tested soil elevation and installed on native material that was consistent with the tactile evaluation. Native soils were consistent through the chamber trenches as described within the geotechnical report. The field is located in the general area of design and has proper separation between trenches. The field is equal to the required 840 sf of chamber infiltration area per the design. Inspection ports were installed on both ends of the chamber runs. The ports that were installed were brought to above grade as required. The septic tank meets sizing requirements per the design. The tank lids have been raised to the proposed grade elevation. The distribution box has been installed and lid has been raised to the proposed grade elevation. Please see attached photos of the installation. Please let me know if you have questions pertaining to this acceptance letter submittal. Thank you, Michael G. Grzesiak, P.E. High Country Engineering, Inc.
