FOR INDIVIDUAL SEWAGE DISPOSAL SYSTEMS
A permit fee of $75. shall be charged for alteration, enlargement, or any repair
involving alteration of an existing sewage disposal system. This fee is authorized
by Eagle County Individual Sewage Disposal System Regulations adopted and effective
March 27, 1980.
For minor repairs of less than $100 for maintenance of the individual sewage
disposal system, no fee shall be required.
A percolation test fee of $50 shall be charged for all new leach fields on repair
permits. Percolation testing may be waived at the ,discretion of the Environmental
Health Officer on certain repair cases where prompt action must be taken to prevent
a wealth hazard.

Permitted Construction / Details:
Install the OWTS exactly as depicted in the LKP Engineering, Inc design stamped, signed, and dated by Luiza
Petrovska, PE. The system is designed to completely replace the existing OWTS, with no permit record for the
system serving the residence, and will served the residence of three-bedrooms.
Abandonment of the existing tank must be completed in conformance with ECPHA OWTS Regulation 43.9.C.
The OWTS consist of a Valley Precast 1,000 gallon three compartment septic tank (1,500 gallon total capacity) with
and effluent filter and an Orenco high head effluent pump PF500511 in the third compartment with floats set to
pressure dose approximately 90 gallons per dose over 57 feet of effluent line to a mounded soil treatment area
(STA). The mounded STA consists of two 49 ft laterals, set atop one foot of gravel distribution cell, 50' long by 11.25'
wide, placed over two feet of secondary sand media basal area, 76' long by 37.25' wide, exactly as shown in the
engineer design.
Note: Gradation of the sand media use must be completed within one month prior to the installation and confirmed
with the design engineer. If the sand does not conform to secondary or preferred sand media requirements, it
cannot be used for the sand treatment media.
Contact Eagle County Environmental Health and the design engineer well in advance of requesting inspections prior
to backfilling any component of the OWTS. The design engineer is responsible for conducting all inspections
necessary to certify the installation and assure the functionality of the system. System certification, along with
photos and a record drawing is required to be submitted to and approved by Eagle County Environmental Health
prior to the use of the system or occupancy of the dwelling.
THIS PERIMT EXPIRES BY TIME LIMITATION AND BECOMES NULL AND VOID IF THE WORK AUTHORIZED BY THE PERMIT IS NOT COMMENCED WITHIN 120 DAYS OF

Dear Perri and Barbara:
At the request of the installer, Travor Dunsdon, Altitude Septic LLC, on October 19 and
November 4, 2020, we visited your property at 3221 Brush Creek Road, Lot 1, Helm Subdivision
Exemption, Eagle County, Colorado. The purpose of our site visits was to inspect the installation of
the new septic system that replaces the failed septic system, for the existing 3-bedroom residence.
The system was installed in overall compliance with the septic system design, Drawing No.
20-3401OWTS.dwg, dated August 5, 2020. At the time of the inspection we observed the following:
They installed 1000-gallons, three-compartments, Concrete Septic Tank by Valley Precast
with an effluent filter (the handle was extended to within 6 inches of the riser cover) in the second
compartment and with an Orenco PF500511 high-head pump assembly in the third compartment. The
inlet of the tank had the Tee installed. The tank access ports had corrugated, plastic risers with
secured, green, plastic covers. The covers were insulated and will be secured with nuts, bolts and
screws. All compartments had 24 inches risers.
The building sewer line was connected to the existing sewer line on the east side of the
existing residence, as shown on the attached as-built sketch. A cleanout was constructed on the new
building sewer line about 2.7 feet away from the house. The building sewer line was 4-inch diameter
SCH40 PVC pipe.
The effluent line from the pump chamber to the end manifold of the mound system, was about
64 feet, 1.25-inch diameter, SCH40 PVC pipe.
The soil treatment area consisted of a mound system, consisting of a sand fill with a gravel
distribution cell. The sand was from United Gravel Pit, was less than 30-days old, and it passed the
classification for a secondary sand. The pressure dosing system consisted of two, 49-feet long, 1.25-
inch diameter sch40 pvc, distribution laterals, perforated at their inverts with 5/32-inch holes at 3-foot
intervals, arranged on the gravel bed with an end manifold. The end manifold was also, 1.25-inch,
sch40 pvc. The ends of the laterals were equipped with flushing assemblies. A squirt test was
performed during the inspection. Two inspection ports were installed, one on each side of the
distribution cell, about 2 feet away from the laterals.

The septic tank installation was done by Valley Precast. The startup of the pump system was
done by Travor Dunsdon, Altitude Septic. The control panel for the pump was on the south side of
the Pump Tank.
The old, plastic septic tank was abandoned by breaking and burying in place.
There is no other warranty either expressed or implied.

RE: Soil and Site Evaluation for an OWTS
Replacement

On May 26, 2020 and July 28, 2020, we visited your property at 3221 Brush Creek Road,
Lot 1, Helm Subdivision Exemption, Eagle County, Colorado. The purpose of our site visits was
to observe the site and help select an area for the proposed onsite wastewater treatment system.
There are several existing structures on the property, including the main residence. The
property has public water supply. The following evaluation was prepared for the replacement of
the existing septic system for the existing 4-bedroom residence. From the information provided
by the ECEH Department, there is a record of an existing septic system consisting of an outhouse
for a cabin, permitted 9/28/1998, under an ISDS Permit # 1825-98. There is no record of the
septic tank and leachfield in use by the existing residence. The owner shared that the existing
septic tank is plastic.
The proposed new OWTS was designed for a 3-bedroom residence. During the first site
visit, we met with you and your installer, Travor Dunsdon with Altitude Septic. Travor dug one
profile holes within the selected new soil treatment area. We logged and evaluated the soil
exposed in the profile hole.
The property is a 3.3-acre parcel of land, according to the Eagle County Assessor’s
Records and according to the survey prepared by Ted Archibeque with Archibeque Land
Consulting, Ltd. Directions are from the intersection of Sylvan lake Road and Brush Creek Road
south on Brush Creek Road for about 1.4 miles. The property is on the south side of the road.
! Preliminary Investigation

G Existing Structures: There is an existing residence and accessory
buildings on the site.

is an existing septic system for the residence, just north and northeast of
the residence. The age of the existing septic system is unknown and it is
failing.

G Soil Data: The profile holes were observed on May 26, 2020. The soil

vacant parcels.

Past use might have been ranching, typical to the surrounding areas.

Dear Sir,
This is a follow-up letter per our phone conversation two weeks ago on an outhouse
to service a log cabin we own at 3221 Brush Creek Rd.
As we discussed, a portable outhouse, is the best solution to having an outhouse
for the cabin. This will be framed around to look like an old outhouse but
integrity of the manufactured outhouse will not be compromised. It will just be
shell around the outside.
I have spoken with two local portable septic services who wil service the tank as
needed. I f you need something in writing, I would be glad to furnish this. The
retailer as well as the service companies have fully informed me of the maintenance
required.
The location of the outhouse will be where the small shed by the electric meter is
as indicated on the enclosed improvement location certificate.
Also enclosed is information provided by the salesperson. Products used in odor
control are cirled on the brochure.
I hope this satisfies all your concerns and regulations. Thanks for your help!

Note: There is no FEMA mapping or flood plain study available for Brush Creek
in this area.. The northerly boundary of the 100-year flood plain, as shown on
page 1 hereof, is interpreted from a flood plain study for BRUSH CREEK STABLES,
