Box 6189 Eagle, CO 81631-6189 carlshepard@hotmail.com 314-374-2903 RE: Inspection of Septic System Installation 3-bedroom residence Lot 19, Kaibab Filing 2 527 Kaibab Road Eagle County, Colorado Project No. 21-3504– OWTS-021736-2021 Dear Carl: At the request of Jeff Beaumont, Beaumont Excavating, Inc., the installer, on August 26, September 8and 13, 2021, we visited your property on Lot 19, Kaibab Filing 2, 527 Kaibab Road County, Colorado. The purpose of our site visits was to inspect the installation of the new (replacement) septic system. They installed the system in overall compliance with the septic system design, Drawing No. 21-3504OWTS-Tank.dwg, dated May 12, 2021, and as shown on the as-built sketch. At the time of the inspections, we observed the following: They installed, 1500-gallons, two compartments, concrete, septic tank from Valley Precast. The inlet and outlet of the tank had the T’s installed. In the outlet “T” there was an effluent filter that will need to be maintained every six months. Both tank access openings had 24-inch risers with secured covers. They installed cleanout at the outlet of the septic tank, as shown on the as-built sketch. New building sewer line was constructed from about 3 feet away from the building to the inlet of the new septic tank. Three trenches with Twenty-five (25), Q-4 Plus, Standard Infiltrator Chambers in each were installed. The trenches were connected with serial distribution. There was a minimum of six feet separation between the trenches. Inspection ports were installed on the first and last infiltrator of each trench in the outside openings of the end caps. The installer broke the old septic tank and buried it in place. There is no other warranty either expressed or implied. If you have any questions, please do not hesitate to call. The purpose of our site visit was to observe the site of the existing septic system and soil treatment area, and select an area for the proposed new septic tank and new Soil Treatment Area. At the same site visit, you excavated two profile holes. The original system, according to the county data files, was from 1974 and had been partially repaired over the years. The old septic tank and the old leachfield had both failed. The old and the new OWTS were both designed for a 3-bedroom residence. The OWTS replacement will consist of a 1500-gallons, two-compartments concrete septic tank by valley Precast with an effluent filter, and a Soil Treatment Area consisting of 75 Quick 4 Plus, Standard Infiltrator Chambers in trenches connected with a serial Distribution. The old leachfield is on the east side of the property, while the selected new site for the STA replacement is on the north side of the property. A topographical survey was not available. Contour data was purchased from the Eagle County GIS Department for the graphical presentation of the OWTS layout. Directions to the site are from US Hwy 6 in the Town of Eagle to 3rd Street for about 0.6 miles, merge to Polar Star Drive and continue for 0.3 miles to Kaibab Road. From Polar Star Road to the driveway for 527 Kaibab Road is about 0.5 miles, and the property is on the south side of the road. ! Preliminary Investigation G Address: The property address is 527 Kaibab Road, Eagle, CO, 81631. G Legal Description: Lot 19, Kaibab Filing 2, Town of Eagle, Eagle County, Colorado. G Existing Structures: There is an existing residence with a garage. G Location of existing and proposed wells on property: There is no well on the property. It is services by the Town of Eagle. G ECEHD (Eagle County Environmental Health Department) records: Old records were received by email and reviewed. The old system was built in 1974, Permit number 0032, and altered in 2001, ISDS Permit #2116R-01. The soil exposed in the profile holes was as follows: Profile Hole #1 0 to 14" Topsoil 14" to 20" Light brown, calcareous, gravelly, sandy, silt and clay 20" to 8 feet Light brown, silty, clay LOAM No ground water encountered Profile Hole No. 2 0 to 12" Topsoil 12 to 30" Blocky clay, calcareous with roots and sand 2.5 to 8 feet Light brown, silty, clay LOAM No ground water encountered Soil samples were taken from both Profile Holes at 48-inches, and were tested for tactile test and classified as silty-clay LOAM, Type 3, with less than 35% Rock (>2mm) LTAR = 0.35 gpd/sf, Gravity System G Location of applicable setbacks in Table 7-1 ! Reconnaissance G Landscape position: The proposed STA will be located in the Back/Side Slope Position north from the existing residence (down gradient). G Topography: The topography of the proposed leachfield site is moderate. The trenches with rows of infiltrator will run along the contours. G Vegetation: The vegetation within the proposed leachfield site consisted of grass, sage brush and few junipers. The site surrounding the leachfield was similar. G Natural and cultural features: The property is located about 0.9mi southeast from Eagle River and about 0.7 miles northeast from Brush Creek. It is also about 1.5 miles south from US Hwy 6. G Current and historic land use: The current use is residential. Prior use was ranching. !
