Permitted Construction / Details:
Install the new system exactly as depicted in the LKP Engineering design dated, stamped and signed March 10th,
2021 by Luiza Petrovska P.E.. The new system will be sized to accomodate the maximum daily flows for a 3 bedroom
residence.
This OWTS consists of one sewer line from the residence. A Valley Precast 1,000 gallon, three-compartment septic
tank with a 1,500 gallon total volume shall be used with an effluent filter in the outlet tee. An Orenco PF500511 High
Head effluent discharge pump shall be used. The floats will be set to dose 90 gallons per dose. The STA consists of
two rows of 24 Infiltrator Quick 4 Plus Standard chambers for a total of 48. Gradation analysis must confirm that
sand is secondary sand media prior to installation.
Contact Eagle County Environmental Health and the design engineer well in advance of requesting inspections prior
to backfilling any component of the OWTS. The design engineer is responsible for conducting all inspections
necessary to certify the installation and assure the functionality of the system. System certification, along with
photos and a record drawing is required to be submitted to and approved by Eagle County Environmental Health
prior to the use of the system or certifying occupancy.
THIS PERIMT EXPIRES BY TIME LIMITATION AND BECOMES NULL AND VOID IF THE WORK AUTHORIZED BY THE PERMIT IS NOT COMMENCED WITHIN 120 DAYS OF
ISSUANCE, OR BEFORE THE EXPIRATION OF AN ASSOCIATED BUILDING PERMIT

Dear Larry:
At the request of the installer, Richie Bossow, Bossow Excavation, LLC, on May 7 and July 6, 2021,
we visited your property at 2451 Alkali Creek Road, Lot 3, Horse Mountain Ranch, Eagle County, Colorado.
The purpose of our site visits was to inspect the installation of the septic system.
The system was installed in overall compliance with the septic system design, Drawing No. 20-
3451OWTS.dwg, dated February 24, 2021. At the time of the inspection, we observed the following:
The building sewer line was connected to the west side of the residence under construction (attached
garage) and the cleanout was constructed at 12-inches from the foundation wall, as shown on the attached as-
built sketch. The building sewer line was about 30 feet of 4-inch diameter SDR35 PVC pipe. They installed
1000-gallons, three compartments, precast, concrete, septic tank by Valley Precast with an effluent filter in the
second compartment and an Orenco PF500511 high-head pump assembly in the third compartment. The inlet
of the tank had the Tee installed. The tank access ports had corrugated, plastic risers with secured, green,
plastic covers. The covers were secured with nuts, bolts and screws. All risers were 36-inch high.
The effluent line from the pump chamber to end manifold of the soil treatment area, was about 180
feet long, 1.5-inch diameter, sch40 pvc pipe. The end manifold was about 3 feet long, 1.5-inch sch40 pvc. The
soil treatment area consisted of infiltrator chambers arranged in a bed configuration with two rows of 24 Quick
Four Plus Standard Infiltrator Chambers in each. Three feet of the onsite silty, clayey soil was overexcavated
from the bottom of the trenches and replaced with three feet of secondary sand media (the graph is attached to
this report). Pressure dosing system consisting of 95-ft, 1.5-inch sch40 laterals perforated at the top with 1/8-
inch holes at 4-ft intervals, suspended from the inside side of each row of infiltrator chambers. The ends of the
laterals were equipped with flushing assemblies. A squirt test was performed during the inspection and it was
satisfactory. All inspection ports were installed (in the end caps of the first and last chamber of each row of
infiltrators) in the opening opposite the pressure distribution lines.
The septic tank installation and startup of the pump system were done by Valley Precast. The control
panel for the pump was on the back of the house.
There is no other warranty either expressed or implied.
