Permitted Construction / Details:
Install the new soil treatment area exactly as depicted in the CBO Inc design dated January 12th, 2022, stamped and
signed on January 31st, 2022 by Romeo A Baylosis, PE. This system is designed to replace the existing STA
permitted under #1148-92IS. This system is designed to accommodate the maximum daily flows for 3 bedrooms.
The OWTS consists of an existing sewer line from the residence. An existing 1,250 gallon, two-compartment
concrete septic tank will remain in use. Risers will be added to bring the lids to grade. A Polylok 3-hole distribution
box equipped with flow equalizers will be accessible at grade. The STA consists of three trenches of 15 Infiltrator
Quick4 Plus Standard chambers for a total of 45 and 525 sq ft of infiltrative area.
Contact Eagle County Environmental Health and the design engineer well in advance of requesting inspections prior
to backfilling any component of the OWTS. The design engineer is responsible for conducting all inspections
necessary to certify the installation and assure the functionality of the system. System certification, along with
photos and a record drawing is required to be submitted to and approved by Eagle County Environmental Health
prior to the use of the system or occupancy of the dwelling.
THIS PERIMT EXPIRES BY TIME LIMITATION AND BECOMES NULL AND VOID IF THE WORK AUTHORIZED BY THE PERMIT IS NOT COMMENCED WITHIN 120 DAYS
OF ISSUANCE, OR BEFORE THE EXPIRATION OF AN ASSOCIATED BUILDING PERMIT

Eagle County Board of Health declares that its general policy is to require the use of public sewer
systems where and whenever feasible and to limit the installation of OWTS only to areas that are
not feasible for connection to public sewer. Eagle County will not issue an OWTS permit where the
structure to be served is within the applicable Water and Sanitation District that provides public
sewer, unless the public service provider is not willing to serve the structure, and authorizes the use
of an OWTS.

Emergency Use Permit May Be Required
Concurrently with the issuance of a repair permit, the ECPHA may issue an emergency use permit
authorizing continued use of a malfunctioning system on an emergency basis for a period not to exceed
the period stated in the repair permit. Such an emergency use permit may be extended, for good cause
shown, in the event repairs may not be completed in the period stated in the repair permit through no fault
of the owner or occupant and only if the owner or occupant will continue to make repairs to the system.

CBO Inc. observed the installation of the onsite wastewater treatment system (OWTS) on November 1,
2022 for the subject property. Jack Huggard installed the system.
The OWTS design is based on 3-bedrooms. An average daily wastewater flow of 450 GPD was used.
The existing septic tank remained in use with new risers and lids installed. The existing soil treatment
area (STA) was abandoned in place. A double-sweep clean-out was installed between the house and
the septic tank. A full-size effluent filter was installed on the outlet side of the septic tank. This filter
must be cleaned annually, or as needed.
Effluent gravity flows to a distribution box, and then to three gravelless chamber trenches. The
distribution box was accessible from grade and flow equalizers were installed on each outlet pipe in the
distribution box to assure equal flow to each trench.
Each trench consisted of 12 ‘ADS’ ARC 36 chambers for a total of 36 chambers and 540 square feet of
infiltrative area. There was at least 4-feet of undisturbed soil between each trench. Inspection ports
were installed at the beginning and end of each trench. Ports may be cut to grade and placed in sprinkler
boxes for access.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS
in the future. Any additional OWTS construction must be according to the county regulations.
LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent on the expertise of the installer, soil
type, and weather conditions.

Permitted Construction / Details:
Install the new soil treatment area exactly as depicted in the CBO Inc design dated January 12th, 2022, stamped and
signed on January 31st, 2022 by Romeo A Baylosis, PE. This system is designed to replace the existing STA
permitted under #1148-92IS. This system is designed to accommodate the maximum daily flows for 3 bedrooms.
The OWTS consists of an existing sewer line from the residence. An existing 1,250 gallon, two-compartment
concrete septic tank will remain in use. Risers will be added to bring the lids to grade. A Polylok 3-hole distribution
box equipped with flow equalizers will be accessible at grade. The STA consists of three trenches of 15 Infiltrator
Quick4 Plus Standard chambers for a total of 45 and 525 sq ft of infiltrative area.
Contact Eagle County Environmental Health and the design engineer well in advance of requesting inspections prior
to backfilling any component of the OWTS. The design engineer is responsible for conducting all inspections
necessary to certify the installation and assure the functionality of the system. System certification, along with
photos and a record drawing is required to be submitted to and approved by Eagle County Environmental Health
prior to the use of the system or occupancy of the dwelling.

Subsurface Investigation and Onsite Wastewater Treatment System Design

CBO Inc. performed a subsurface investigation and completed an onsite wastewater treatment system
(OWTS) design for the subject residence. The 1.44-acre property is located outside of Basalt, in an area
where OWTSs are necessary.

SITE CONDITIONS A 3-bedroom, single-family residence presently exists and is utilizing an existing OWTS. The existing
OWTS consists of a 1250-gallon, 2-compartment concrete septic tank followed by a soil treatment area
(STA) consisting of two trenches, each with 6 ‘Standard’ Infiltrator® chambers. The OWTS is
documented by Eagle County Individual Sewage Disposal System Permit 1148-92.
The residence is served potable water from a community water system. The water line is not noted on
the survey; however, the nearest water meter is located on McLaughlin Lane to the west of the
residence. The new STA must be at least 25-feet from the water line.
The STA is experiencing saturated conditions and should be replaced.
The existing STA will be abandoned in place and the new STA constructed downhill from the existing.
The area is vegetated with native grasses and slopes to the southwest at an approximate 20% slope to
the southwest.
There should be no traffic or staging of material over the future STA site to avoid compaction of soils
prior to construction of the STA.

SUBSURFACE The subsurface was investigated on August 17, 2021 by digging two soil profile test pit excavations (Test
Pits). A visual and tactile soil analysis was completed by Carla Ostberg after the excavation.1 Findings were
consistent with previous Eagle County percolation testing which resulted in an average percolation rate of 8
minutes per inch (MPI).
The materials encountered in Test Pit #1 consisted of reddish brown, moist loam to a maximum depth
explored of 9.0-feet. No bedrock or groundwater was encountered.
The materials encountered in Test Pit #2 were consistent of reddish brown, moist loam to a maximum
depth explored of 9.0-feet. No bedrock or groundwater was encountered.
A sample of the soil was taken from Test Pit #1 at 3-feet below grade. Soil structure grade was
moderate. The soil formed a ball and a ribbon less than 1-inch in length before breaking. Soil structure
shape was blocky and consistence was friable. Soil texture was more gritty than smooth.
STA sizing is based on Soil Type 2, Sandy Loam with moderate structure grade. A long term
acceptance rate (LTAR) of 0.6 gallons per square foot will be used to design the STA.

The new OWTS design is based on 3-bedrooms. An average daily wastewater flow of 450 GPD will be
used.
For the purposes of this OWTS design, Benchmark Elevation has been established as 100’ as the sewer
line exiting the existing septic tank. CBO Inc. should be notified of any discrepancies or problems with
grade elevations of proposed components during installation of the OWTS.

The existing septic tank will remain in use. The existing STA will be abandoned in place. A double-
sweep clean-out should be installed between the house and the septic tank if one does not already exist.
A full size effluent filter must be installed on the outlet side of the septic tank. The sewer line exiting the
septic tank must be properly re-aligned. Additional risers are required to bring the manhole lids
accessible from grade. This will require approximately 2-feet of risers on the inlet side and 1-foot of riser
on the outlet side of the septic tank.
Effluent will gravity flow to a distribution box, and then to three gravelless chamber trenches. The
distribution box must be accessible from grade and must have flow equalizers, or similar product,
installed on each outlet pipe in the distribution box to assure equal flow to each trench.
Each trench will consist of 15 ‘Quick 4’ Standard Plus Infiltrator® chambers for a total of 45 chambers
and 540 square feet of infiltrative area. There must be at least 4-feet of undisturbed soil between each
trench. Inspection ports must be installed at the beginning and end of each trench. Ports may be cut to
grade and placed in sprinkler boxes for access.

CBO Inc. must approve alternative components prior to installation of the OWTS. Requests must
be submitted, in writing, to our office for approval prior to installation. Component technical data

according to Eagle County On-Site Wastewater Treatment System Regulations, the OWTS Permit provided
by Eagle County Environmental Health Department, and this design.
INSTALLATION CONTRACTOR
CBO Inc. expects that the installer be experienced and qualified to perform the scope of work outlined in this
design. The installer must review this design thoroughly and coordinate with our office in advance of
installation. Any additional conditions in this design or county permit must be completed and documented
prior to final approval of the OWTS installation. Communication between the installer and this office is
expected throughout the installation.
INSTALLATION OBSERVATIONS CBO Inc. must view the OWTS during construction. The OWTS observation should be performed before
backfill, after placement of OWTS components. Septic tanks, distribution devices, pumps, dosing
siphons, and other plumbing, as applicable, must also be observed. CBO Inc. should be notified 48 hours
in advance to observe the installation.
In an effort to improve the accuracy of the record drawing, we request that the installer provide a
sketch of the installation, including path of the sewer lines, water line installation (if applicable),
septic tank location, STA location, and measurements from building corners or another fixed
objects on the property. This sketch is most easily provided on Sheet W2.0 of the OWTS Design
Packet. Photographs of the installation and final cover are also requested to supplement our installation
documentation.
REVEGETATION REQUIREMENTS
An adequate layer of good quality topsoil capable of supporting revegetation shall be placed over the entire
disturbed area of the OWTS installation. A mixture of native grass seed that has good soil stabilizing
characteristics (but without taproots), provides a maximum transpiration rate, and competes well with
successional species. No trees or shrubs, or any vegetation requiring regular irrigation shall be placed over
the area. Until vegetation is reestablished, erosion and sediment control measures shall be implemented
and maintained on site. The owner of the OWTS shall be responsible for maintaining proper vegetation
cover.
OPERATION INFORMATION AND MAINTENANCE
The property owner shall be responsible for the operation and maintenance of each OWTS servicing the
property. The property owner is responsible for maintaining service contracts for manufactured units,
alternating STAs, and any other components needing maintenance.
Geo-fabrics or plastics should not be used over the STA. No heavy equipment, machinery, or materials
should be placed on the backfilled STA. Machines with tracks (not wheels) should be used during
construction of the STA for better weight distribution. Livestock should not graze on the STA. Plumbing
fixtures should be checked to ensure that no additional water is being discharged to OWTS. For example, a
running toilet or leaky faucet can discharge hundreds of gallons of water a day and harm a STA.
If an effluent filter or screen has been installed in the OWTS, we recommend this filter or screen be cleaned
annually, or as needed. If the OWTS consists of a pressurized pump system, we recommend the laterals be
flushed annually, or as needed.

The homeowner should pump the septic tank every two years, or as needed gauged by measurement of
solids in the tank. Garbage disposal use should be minimized, and non-biodegradable materials should not
be placed into the OWTS. Grease should not be placed in household drains. Loading from a water softener
should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS.
Mechanical room drains should not discharge into the OWTS. The OWTS is engineered for domestic waste
only.
ADDITIONAL CONSTRUCTION NOTES If design includes a pump, weep holes must be installed to allow pump lines to drain to minimize risk of
freezing. The pump shall have an audible and visual alarm notification in the event of excessively high
water conditions and shall be connected to a control breaker separate from the high-water alarm breaker
and from any other control system circuits. The pump system shall have a switch so the pump can be
manually operated.
Excavation equipment must not drive in the excavation of the STA due to the potential to compact soil.
Extensions should be placed on all septic tank components to allow access to them from existing grade.
Backfill over the STA must be uniform and granular with no material greater than minus 3-inch.
LIMITS: The design is based on information submitted. If soil conditions encountered are different from conditions
described in report, CBO Inc. should be notified. All OWTS construction must be according to the county
regulations. Requirements not specified in this report must follow applicable county regulations. The
contractor should have documented and demonstrated knowledge of the requirements and regulations of
the county in which they are working. Licensing of Systems Contractors may be required by county
regulation.

Hi Claire, this is Jack. I have photos for inspection from the job at 15200 on cottonwood pass. You Hawk ln, Ruedi
Shores you can reach me at 970-930-5769. For our inspection pipes they will be put in valve boxes which is why they
look short in the photos. The rows are 60 feet long with 15 4ft infiltrators. These photos are of the two exterior fields with
17ft between the center of infiltrator to the other center of infiltrator in the other trench. I will get you photos of the third
row tomorrow as well as the distribution box and the cleanouts from the septic tank as well as the filter installed in the
tank if you need. Sorry about the miss read on the permit. Thank you for getting on site quickly.

Thank you Jack. I got there yesterday. What I could see of the STA from the end caps looked good. Please take pictures
of the middle trench when installed, the lines to the trenches, the gasket around the outlet pipe in the tank, of the tank
capped, and replacement of the tank risers (the lids, especially on the second compartment were in poor condition), as
well as an effluent filter in the outlet tee. I copied Carla in case you or she has questions for me.
