Permitted Construction / Details:
Install the new system exactly as depicted in the LKP Inc design dated, stamped and signed on July 26th, 2023 by
Luiza Petrovska, PE. This system is designed to accommodate the maximum daily flows for a 5 bedroom residence.
The OWTS consists of one sewer line from the residence. A Valley Precast 1,500 gallon, concrete, two-compartment
septic tank with an effluent filter in the outlet tee shall be used. Effluent will flow to the STA serially to 5 rows of
Infiltrator Quick4 Plus Standard chambers. The two trenches on the ends will have 18 chambers while the middle
three trenches have 22 chambers for a total of 100.
Contact Eagle County Environmental Health and the design engineer well in advance of requesting inspections prior
to backfilling any component of the OWTS. The design engineer is responsible for conducting all inspections
necessary to certify the installation and assure the functionality of the system. System certification, along with
photos and a record drawing is required to be submitted to and approved by Eagle County Environmental Health
prior to the use of the system or occupancy of the dwelling.
THIS PERIMT EXPIRES BY TIME LIMITATION AND BECOMES NULL AND VOID IF THE WORK AUTHORIZED BY THE PERMIT IS NOT COMMENCED WITHIN 120 DAYS
OF ISSUANCE, OR BEFORE THE EXPIRATION OF AN ASSOCIATED BUILDING PERMIT

The State Law requires that the OWTS permit be issued before the building permit is issued. If your
application is incomplete, it will delay the issuance of your OWTS permit, and the issuance of the
building permit.
Eagle County Board of Health declares that its general policy is to require the use of public sewer
systems where and whenever feasible and to limit the installation of OWTS only to areas that are
not feasible for connection to public sewer. Eagle County will not issue an OWTS permit where the
structure to be served is within the applicable Water and Sanitation District that provides public
sewer, unless the public service provider is not willing to serve the structure, and authorizes the use
of an OWTS.

Emergency Use Permit May Be Required
Concurrently with the issuance of a repair permit, the ECPHA may issue an emergency use permit
authorizing continued use of a malfunctioning system on an emergency basis for a period not to exceed
the period stated in the repair permit. Such an emergency use permit may be extended, for good cause
shown, in the event repairs may not be completed in the period stated in the repair permit through no fault
of the owner or occupant and only if the owner or occupant will continue to make repairs to the system.

08/02/23 - Depth of infiltrative surface at NW corner of first two trenches needs to be adjusted at time
of final grade. Max depth of infiltrative surface is 4'. Setback distance to creek from NE corner of last
two trenches needs to be confirmed prior to closing last two trenches. Required setback distance is
50' from stream. System currently has 7 extra infiltrators. Infiltrators can be removed to create more
setback distance if needed. Dave Bronn (and LKP?) to confirm for final. One hundred and seven
(107), Q-4 Plus, Standard Infiltrator Chambers were installed in five trenches, with twenty-two
infiltrators in each of the first four trenches, and 19 chambers in the last (fifth) trench.

Dear Greg:
At the request of Dave Bronn, Bronn Excavating, the installer, on August 2, 2023, we visited the
construction site for the Adkins, 5-Bedroom Residence, on Lot 6, Timber Springs Subdivision, 300
Timber Springs Drive, Eagle County, Colorado. The purpose of our site visit was to inspect the installation
of the septic system. Kirk Weems, from the ECEH Department was there too.
The system was installed in overall compliance with the septic system design, Drawing No. 22-
3613OWTS.dwg, revised, July 26, 2023. At the time of the inspection we observed the following:
They installed a 1500-gallons, two compartments, precast, concrete, septic tank by Valley Precast on
the east side of the residence under construction. The building sewer line was about 57 feet long, 4-inch
diameter SDR35 pvc pipe. Double cleanout was constructed on the sewer line, at about 12.5 feet south from
the northeast building corner and about 3 feet east from the building, as shown on the as-built sketch.. The inlet
and outlet of the tank had the T’s installed. The tank access ports had 36-inch corrugated, plastic risers with
green, plastic covers. The covers were secured with nuts, bolts and screws. In the outlet tee, an effluent filter
was installed.
One hundred chambers were required. One hundred and seven (107), Q-4 Plus, Standard Infiltrator
Chambers were installed in five trenches, with twenty-two infiltrators in each of the first four trenches, and 19
chambers in the last (fifth) trench. There was a minimum of 4 feet separation between the trenches. The rows
of infiltrator chambers, were connected with serial distribution. All inspection ports were installed (in the end
caps of the first and last chamber of each row of infiltrators). The effluent line from the septic tank to the
leachfield was about 17 feet long, 4-inch diameter SDR35 pvc pipe. The septic tank covers, the inspection
ports on the STA, and the closest stream, were located by the surveyor, Matt Slagle, PLS.
There is no other warranty either expressed or implied.

Dear Greg,
On May 18, 24, and 28. 2022, we visited the site for the proposed Adkins Residence on Lot 6,
Timber Springs Subdivision, 300 Timber Springs Drive, Eagle County, Colorado. The purpose of our
site visit was to observe and evaluate an area for the proposed onsite wastewater treatment system
(OWTS) for the proposed 5-bedroom residence.
The OWTS will consist of a 1500-gallons, two-compartments concrete septic tank by Valley
Precast with an effluent filter, and a Soil Treatment Area consisting of 100 Quick 4 Plus, Standard
Infiltrator Chambers in trenches connected with a serial distribution.
The site plan by your architect Greg Macik, NCARB, TAB Associates, Inc., was used as a basis
for the septic system design. Directions to the site from Edwards are as follows: from US Highway- 6
turn right onto Hillcrest Drive, continue north through the I-70 underpass as the road turns into Beard
Creek Trail for about 375 feet to Timber Springs Drive on the left. Go through the gate and continue to
300 Timber Springs Road. The building site is on the left side of the road.

G Topography: The topography of the leachfield site is moderate with an average
slope of about 25 percent. Drainage is to the east-southeast.
