Be aware that changes in the use of your property or alterations of your building may require
commensurate changes to, or relocation of, your septic system. Landscape features, trees with
tap roots, irrigation systems and parking areas above the drain field can cause premature
system failure. It is equally important that you notice and immediately repair dripping faucets
and hissing toilets as this will certainly cause the system to fail. By following a few simple
guidelines, like avoiding grease or other additives down the drain and making sure your septic
tank is on a pumping schedule, your system has been designed to last for many years.

Dear Mr. Small:
Hepworth-Pawlak Geotechnical, Inc. has observed the installation of the Individual
Sewage Disposal System (ISDS) for the four bedroom residence on the project site. We
previously designed the ISDS at the subject site and presented our recommendations in a
report dated May 21, 2007, Job No. 407 0097. The ISDS components observed include:
septic tank, advanced treatment unit, effluent pump, sewer pipe, cleanouts, Infiltrator
trenches, serial distribution connection pipes, inspection ports, and the locations of all
components. Based on our observations made during our site visits on August 14 and 22,
2007, it is our opinion that the ISDS was constructed in general accordance with the ISDS
design plan, dated May 21, 2007. To facilitate construction, several of the components
were installed in locations different from the design by the installer, Don Odell; however,
these changes meet the intent of our design. The ISDS As -Built plan view is shown on
the attached Figure 1, detailing the new component locations.
Based on our phone conversation with Don Odell, we understand that the sewer pipe was
installed about 10 feet below the ground surface and passes beneath the foundation of the
residence. The pipe was embedded in gravel and sloped at least 2% towards toward the
septic tank. The septic tank and advanced treatment unit were installed level. All
minimum horizontal distances from ISDS components required by Eagle County

Hepworth-Pawlak Geotechnical, Inc. had observed the installation of the Individual
Sewage Disposal System (ISDS) for the four bedroom residence on the project site and
prepared an As -Built letter dated November 28, 2007, Job No. 407 0097. We previously
designed the ISDS at the subject site and presented our recommendations in a report dated
May 21, 2007, Job No. 407 0097. As requested by the Eagle County Environmental
Health Department, additional information is provided below.
• An Advantex AX20 Advanced Treatment Unit (ATU) was installed in the 3B
Mode configuration. A diagram showing the configuration is provided as an
attachment.
• During installation of the septic tank, the previous absorption field (now
abandoned) aggregate was encountered at approximately 3 feet below the existing
ground surface. The aggregate was used as backfill for the new tank. The
abandoned absorption field was not encountered during the installation of the new
absorption trenches.

Dear Mr. Small:
As requested, Hepworth-Pawlak Geotechnical, Inc. has designed an Individual Sewage Disposal
System (ISDS) based on the Long Term Acceptance Rate (LTAR) of the subsoils for the proposed
four bedroom residence at Lot 3, Red Canyon Acres, located at 050 Canyon Court in Eagle, Colorado.
Our scope of services was performed in accordance with our agreement to you dated April 27, 2007.
The data obtained and our ISDS design recommendations based on the proposed construction and
subsurface conditions encountered are presented in this report.
Percolation Testing: Percolation tests were conducted on April 4, 2007 by our Glenwood Springs
office to evaluate the percolation rate of the natural soils. The percolation test results are presented in
the attached Percolation Test Report, Job No. 106 0973, dated April 11, 2007. The subsoils exposed
in the Profile Pit, below about 2 feet of granular fill, consisted of relatively dense, silty sandy gravel
and cobbles with occasional boulders to the depth explored of approximately 8.5 feet. The average
soil percolation rate was about 10 minutes per inch which correlates to a Long Term Acceptance Rate
of 1.2 gallons per square foot per day, which was used to size the absorption area.
ISDS Analysis: Based on the subsurface conditions encountered and the LTAR of the natural soils,
the tested area is suitable for an ISDS utilizing Infiltrator chambers and the shallow soils for the
treatment and dispersal of effluent. A site plan showing the proposed ISDS, proposed residence,
property lines and the minimum horizontal setbacks of site features relative to the ISDS components is
shown on Figure 1. The subdivision is served by a community water system. All materials and
installation should meet the requirements of the current Eagle County Land Use Regulations for
Individual Sewage Disposal Systems.

Septic and Dosing Tanks: A concrete 1,500 gallon, two chamber, Front Range Precast septic tank, or
equivalent, will be used for primary treatment of sewage from the residence. To accommodate the
Advantex system, the first two chambers of the septic tank should have a total settling capacity of at
least 1,500 gallons. Sewage will gravity feed from the residence to the septic tank where it will be
pretreated by an Advantex advanced treatment unit. Following pretreatment, effluent will gravity feed
to the absorption trenches.
If the septic tank access riser lids will be installed at grade, with no backfill cover, then the lids should
be insulated with foam insulation board to provide frost protection. The sewer pipe inlet tee in the
first chamber of the septic tank should be installed beneath the access lid of the first chamber of the
septic tank so that it can be reached for service should it become clogged.
Sewer and Effluent Piping: The sewer line from the residence to the septic tank should not be less
than the diameter of the building drain and not less than 4 inches in diameter and will service all levels
of the proposed residence. We understand that the residence will be constructed over a crawlspace.
The sewer pipe should leave the foundation as shallow as practical to limit the depth at which the
absorption trenches must be installed to allow the effluent to gravity feed. Soil backfill beneath the
sewer pipe must be adequately bedded and compacted to prevent settlement of the pipe. The sewer
pipe should be installed in landscape areas with at least 2 feet of soil backfill covering the pipe, or the
pipe should be insulated on the top and sides with 2 inch thick rigid foam insulation board for frost
protection. Beneath driven surfaces, we suggest Schedule 40 PVC sewer pipe should be used and the
pipe should be insulated if it will be installed with less than 4 feet of backfill soil cover.
We recommend using Schedule 40 pipe between the proposed residence and the septic tank, though
this is not required by local regulations. The portion of the sewer pipe extending from 5 feet outside
the building foundation should have a slope of at least 2%. Cleanout pipe(s) should be installed in the
sewer line adjacent from the building foundation, adjacent to the inlet of the septic tank, where the
sewer pipe bends at 90 degrees or more and at least one cleanout should be installed at least every 100
linear feet of non -pressurized sewer or effluent pipe throughout its length. Any required 90 degree
bends in the sewer pipe should be accomplished through the use of a series of 45 degree smooth
elbows or a 90 degree long sweep to help prevent clogging of the pipe.

The 2 inch diameter schedule 40 PVC transport pipe should be sloped at 2% minimum to drain to the
absorption trenches. It is critical that the effluent transport pipe be properly bedded in compacted road
base or pea gravel to protect the pipe from large rocks in the natural soil and to prevent settlement,
which could cause standing effluent to freeze in the pipe.
Absorption Area: The soil absorption area is designed to be 375 square feet in size and will consist
of three trenches connected in a serial distribution system. A total of (thirty-eight) Quick 4 standard
Infiltrator chambers will be utilized. Each chamber was given 10 square feet of absorption area per
Eagle County regulations. The first trench will have an approximate dimension of 72 feet long by 3
feet wide and consist of (sixteen) standard Infiltrator chambers, for an absorption capacity of 160
square feet. The second trench will have an approximate dimension of 55 feet in length and 3 feet
wide and consist of (twelve) standard Infiltrator chambers, for an effective absorption area of 120
square feet. The third trench will have an approximate dimension of 46 feet long by 3 feet wide and
will consist of (ten) standard Infiltrator chambers, for an absorption capacity of 100 square feet. The
trench excavations must maintain a minimum 15 foot setback from the boulder wall location shown on
Figure 1 to ensure the wall integrity and to inhibit effluent surfacing. Absorption area calculations are
shown on Figure 2. The absorption area trenches should be excavated to the approximate depths
shown on Figure 4. When utilizing a serial distribution system, it is important to note that the trenches
must be excavated to allow the effluent to be gravity fed. Care must be taken during installation to
ensure that the soils exposed in the bottoms and sidewalls of the trench excavations are not compacted.
Once the trench excavations are complete they should be left open, if necessary, to allow the soils to
air dry prior to placement of the chambers. Once the soils have air dried, if necessary, the bottoms and
sidewalls of the excavations should be scarified prior to placement of the chambers.
The on -site soils can be used as backfill over the absorption area and should be graded to deflect
surface water and precipitation away from the absorption area. We recommend the soil backfill be re -
vegetated as soon as possible with a mountain meadow grass mix or suitable equivalent. It is
important not to plant large vegetation or vegetation with invasive roots in and around the absorption
area and manifold piping as the roots may damage piping.
Inspection ports should be installed at each end of each absorption trench. The ports should be
constructed of 4 inch diameter PVC pipe installed into the knockouts provided on the tops of the

Infiltrator chambers. The ports should extend a minimum of 8 inches above the finished grade and
should have removable caps. Eagle County requires that the vent holes of the inspection ports be
covered with # 16 fly-tite screen. The absorption area manifold and distribution piping layout and
serial trench connections are shown on Figure 3.
All materials used and installation methods should meet the requirements of the current Eagle County
Land Use Regulations for Individual Sewage Disposal Systems.

AdvanTex Treatment System: An AdvanTex AX20 treatment system installed in the iB 2 i 0
configuration will be used to ensure that high quality effluent is applied to the absorption area. The J4
effluent will be pumped from the septic tank to the AdvanTex system for advanced treatment, then U albe
gravity fed to the absorption trenches. Research performed by the National Sanitation Foundation has
shown that the AX20 is capable of reducing the Total Suspended Solids in a septic tank effluent by
approximately 83% and is capable of reducing Biochemical Oxygen Demand of septic tank effluent by
approximately 96%. The AdvanTex treatment system is sold as a package unit that includes
component installation and 2 to 3 years of operation and maintenance. It is required that a
maintenance contract with a certified AdvanTex maintenance company exist at all times because the
system does require regular maintenance to function properly.
The system will be monitored by Vericom, which is a web -based telemetry monitoring system. The
use of Vericom does not require a dedicated phone line but will automatically send an alert to the
maintenance provider if an alarm condition develops. The system should be observed for the first
month of operation to ensure proper control settings and operation. Once "start-up" is completed a
semi-annual observation of the system is required. During the semi-annual observations, the septic
tank sludge and scum accumulation should be measured and this data used to develop a tank pumping
schedule. The effluent quality should be observed to ensure that clear odorless effluent is dosed to the
absorption area. In addition, the air vent/inspection ports in the absorption area should be measured
and recorded for future use. The AdvanTex system is available from SCG Enterprises in Conifer and

any excavations on the site, we should be notified to re-evaluate the need for a subsurface drain to
protect the absorption area.
ISDS Maintenance: The on -site ISDS will require maintenance. The level of maintenance will vary
with the complexity of the system and water use habits of the users. We recommend that fats, oils,
bath oils, greases, paint, solids, water softener backwash solution, water from hot tubs and other
constituents that can clog and foul collection and disposal equipment are not disposed of in the
residence drains. The absorption area and any areas directly up gradient of it should not be watered
excessively by lawn irrigation systems or other means as the soils may become hydraulically
overloaded. Septic tanks and other treatment tanks should be pumped whenever the sludge and scum
occupy one third of the liquid capacity of the first chamber of the septic tank. We recommend that an
operation and maintenance manual be developed for the ISDS. The ISDS should be operated and
maintained as stipulated in the manual. If you desire our services, we can prepare the operation and
maintenance manual for the on -site ISDS.
Limitations: This design has been conducted in accordance with generally accepted engineering
principles and practices in this area at this time. We make no warranty either express or implied. The
conclusions and recommendations submitted in this report are based upon the data obtained from the
Profile Pit and percolation test holes excavated at the approximate locations indicated on Figure 1, the
proposed type of construction, and our experience in the area. Our findings include interpolation and
extrapolation of the subsurface conditions identified at the Profile Pit and percolation test holes and
variations in the subsurface conditions may not become evident until excavation is performed. If
conditions encountered during construction appear different from those described in this report, we
should be notified at once so re-evaluation of the recommendations may be made.
Based on site reconnaissance, it is our opinion that the designed location of the ISDS does not violate
any setback requirements of the current Eagle County Land Use Regulations for Individual Sewage
Disposal Systems. The staked locations of the ISDS components are approximate and not for
construction. We recommend that the position of the ISDS components, proposed residence and
setback requirements be verified in the field by a Professional Land Surveyor licensed in the State of
Colorado.

This report has been prepared for the exclusive use by our client for the ISDS design submittal
process. We are not responsible for technical interpretations by others of our information. As the
project evolves, we should provide continued consultation and field services during ISDS construction
to review and monitor the implementation of our recommendations, and to verify that the
recommendations have been appropriately interpreted. We recommend on -site observation of the
absorption area excavation, chamber placement, pipe and tank installation, and soil backfill placement
by a representative of the geotechnical engineer. On -site observations of the ISDS installation or other
out of scope services will be provided for an additional cost.
Eagle County requires that an As -Built evaluation of engineered ISDS designs be performed by the
ISDS engineer. We should be contacted to perform the ISDS As -Built evaluation the day before you
are ready for the evaluation. Prior to evaluation, all ISDS components should be installed including:
sewer pipe, effluent pipe, septic tank, effluent filter, inlet and outlet tees, Infiltrator chambers and
distribution piping. No components of the ISDS should be backfilled prior to our performing the
ISDS As -built evaluation.

As requested by the Eagle County Environmental Health Department, Hepworth-Pawlak
Geotechnical has prepared this letter as an addendum to our Individual Sewage Disposal System
(ISDS) design dated May 21, 2007 (Job No. 407 0097). Our additional recommendations based
on the site conditions are presented below.
During review of the ISDS design, Eagle County made us aware of an existing ISDS at the
subject site. We did not observe any components of the existing ISDS during our site
reconnaissance or subsurface exploration; therefore, the exact location and depth of the existing
absorption area is not known at this time. If the existing field is encountered during excavation,
the minimum and maximum cut depths specified in the May 21, 2007 design should still be
adhered to. If material from the existing absorption area is still present after excavation, an
additional 6 inches of material should be removed and backfilled with concrete sand. The
Infiltrator chambers will be placed on the sand or native soil.
Due to the advanced treatment of effluent utilizing the Advantex system, we do not believe that
removal of the existing absorption field is necessary.

As requested, .Hepworth - Pawlak Geotechnical, Inc. performed percolation testing at the
subject site. The testing was done as supplement to our previous agreement for
professional services with Sierra Construction, dated November 13, 2006. We previously
conducted a subsoil study for design of foundations at the site and presented our findings
in a report dated :December 21, 2006, Job No. 106 0973.
A profile pit and three percolation test holes were excavated on April 4, 2007 at the
locations shown on Figure 1. The subsoils exposed in the Profile Pit, below about 2 feet
of granular fill, consisted of relatively dense, silty sandy gravel and cobbles with
occasional boulders to the bottom pit depth of 8%z feet. A gradation analysis performed
on a sample of the natural coarse granular soils performed for our previous subsoils study
indicated a percent passing the No. 200 size sieve of 17%. Based on this information and
our field observations, the soils classify as sandy loam to loamy sand (USDA
classification) which corresponds to a long term acceptance rate of about i gallon per
square foot per day. No free water was observed in the pits and the soils were generally
moist. The subsoils are similar to those encountered .in our previous borings at the site.
Three percolation test holes were hand dug in the bottom of shallow backhoe pits and
soaked with water on April 4, 2007. I -lie percolation testing was conducted the following
day on April 5, 2007 by a representative of Hepworth - Pawlak Geotechnical, Inc. The
percolation test results, summarized in Table 1, indicate percolation rates from about 7 to
