This letter is to inform you that the above -referenced ISDS Permit has been inspected and finalized.
Your septic system has been sized to accommodate 3 bedrooms. Enclosed is a copy to retain for
your records along with a brochure regarding the care of your septic system. Additional information
about the maintenance of your septic system as well as information about private wells can be
accessed through web site links provided on.the Environmental Health Department's home page at

Be aware that changes in the use of your property or alterations of your building may require
commensurate changes to, or relocation of, your septic system. Landscape features, trees with
tap roots, irrigation systems and parking areas above the drain field can cause premature
system failure. It is equally important that you notice and immediately repair dripping faucets
and hissing toilets as this will certainly cause the system to fail. By following a few simple
guidelines, like avoiding grease or other additives down the drain and making sure your septic
tank is on a pumping schedule, your system has been designed to last for many years.

Dear Mr. Adams,
Please find enclosed ISDS permit, number IS-2496A-05 to conduct the construction exactly as indicated
on the permit. It remains valid for 120 days or for the duration of a current building permit. The enclosed
copy of the permit must be posted at the installation site. Any changes in plans or specifications must be
pre -approved by this department. If you are receiving this permit and have not been contracted to
perform the installation, please contact us immediately.
Also enclosed is a final inspection completeness check list to assist you in making sure all of the detailed
points of an inspection have been properly addressed before calling for your final inspection. It is,
important to pay close attention to the special conditions which may have been included to address site
specific issues. All components of the system must be exposed for our inspection. Projects that require
multiple inspections will be assessed a re -inspection fee of $55.00 payable before or at the time of
inspection. It is extremely important to remember to rake all infiltrative surfaces and avoid installing
systems in wet or frozen ground.
In addition to avoiding installations during unfavorable site conditions, please be advised of our annual
time frame for ISDS installations, unless otherwise specifically approved by this department. Permits
issued on or before November 15 must be installed prior to December 1. Spring field activities typically
resume around mid March.
Please keep this office informed regarding the progress of your installation so we can provide responsive
customer service when scheduling your final inspection. The Temporary Occupancy Permit (TCO)
associated with the building permit cannot be issued without the septic system being finally inspected.
Systems designed by a registered professional engineer must be inspected and certified by the design
engineer as well as Eagle County Environmental Health.

Dear Mr. and Mrs. Brasington,
Peggy, thanks for our conversation on June 29th related to the addition made to your main
residence under building permit # 17473. Although you may have been led to believe
that an alteration to your septic system was not needed, a further review of our records
has determined that the drainfield does in fact need to be enlarged. Even though the
1,500 gallon septic tank is adequate to handle the increase in waste water, your current
drainfield does not provide the necessary area to accept the amount of waste water
generated by a three bedroom home. The septic system permit IS-2496A-05 for the
alteration of the drainfield states that 30 Q-4 Infiltrators should be added in serial a a,
distribution to the existing 29 standard infiltrators to increase the area to a total of-60"-
square feet. This permit was to be issued to Chris Adams once he became licensed to
install ISDS systems; however, since Chris did not become licensed in 2006 so the permit
could not be sent to him. Enclosed is a copy of the permit that was prepared for the
alteration as well as the letter that was to be issued to Chris Adams.
In order to prevent problems with your septic system, and to not affect the status of
occupancy, your drainfield must be altered as soon as possible. Please contact this office
to indicate who you would like to complete the ISDS work. If Chris is the person who
you would like to install the system, we'll notify him to become licensed.

As per your request, I'm writing to provide you with information regarding the Individual
Sewage Disposal System (ISDS) serving the barn/guest house stricture on your property
located at 3758 Salt Creek Road, Eagle County, Colorado. Our records indicate that
work done on the septic system serving this structure was conducted under two
constriction permits (ISDS Permits 1402-94 & 2496A-05). I believe we have an
accurate record of exactly what is installed and as such, can explain what the system is
sized to accommodate based on current septic system sizing criteria.
The original septic system, ISDS Perniit 1402-94, was installed in 1996 and consisted of
a 1,500 gallon septic tank with a drain field area consisting of 29 Standard Infiltrator
units, serially connected in a trench configuration.
In 2006, you proposed to remodel the structure and add bedroom capacity. It was
determined that more drain field area was needed to adequately serve the structure based
on current sizing criteria. Since the installation of your original system in 1996, the
method of calculating the drain field area; credit given for using Infiltrator chambers as
well as the dimensions of the Infiltrator chambers has changed, while septic tank sizing
criteria has remained the same. Therefore, an ISDS permit was issued to amend the
original septic system to add drain field area. Our records indicate that this work actually
occurred in 2007 and your installer added 8 more chambers to the minimum amount
permitted. The chambers used today are smaller than those used in 1996. Your system
as installed today consists of a 1,500 gallon septic tank with 1,659 square feet of
absorption area credit, capable of serving a three bedroom structure.

7/11/07 I spoke to Peggy Brasington about the letter sent to her regarding the necessity
of expanding her leachfield to accommodate the third bedroom addition She now
understands that the soils require more drainage area and she will be contacting us with a
licensed installer.
08/02/07 New installer is Paul Phyfer # 78-07
4/11/08 Paul Phyfer came in today for copies of the permits due to Peggy Brasington
saying that the installer before Paul did the work. On the note above is the date that Paul
came to say that he would be the installer. I wrote the change on the outside of the paper
file but just crossed out the previous installer and put Pauls name & information in its
place anticipating that I would get to the ADAPTS file when I had time. On 4/11/08 I
made Paul a copy of the incorrect permit that we had crossed out the previous installers
name & information on in order for him to start the work, and also made him a copy of
the final permit where all the information had been updated and changed in the system.

The Environmental Health Division would like to notify you to
make a formal request to extend your Individual Sewage Disposal
System(ISDS) Permit #/4/-� into the 1996 construction year. The
Environmental Health Division discontinued percolation tests on
November 15, 1995 and.final inspections on December 2, 1995 due
to climatic conditions.
ISDS permits are active for 120 days after the date of issue if
no Building Permit has been issued for the same property. If a
Building Permit has been issued for the same property the ISDS
will expire at the same time as the building permit.
If you still plan to apply for a Building Permit this year or in
the early months of 1996 you will need to have a percolation test
conducted before your Building Permit will be released. You will
have to contact a Registered Professional Engineering(RPE) firm
to conduct your percolation test.
If you have had your permit issued and have not had your final
inspection completed, but plan on constructing the system in the
next few weeks, please give our office a call and we will
evaluate your permit on a case by case situation.
Please give the Environmental Health office a call at 328-8755,
if you have any questions regarding your permit extension
process.

I asked Peggy Brasington for a letter stating how they were going to handle the waste coming from their barn. I also
requested that she provide us with a site plan showing their spring or well location, the leach field, septic tank, and
barn drainage pipe locations, and distances. The barn drainage system was not completed at the time of inspection.
I also told her that after we got her letter, and approved of the barn drainage system, we would finalize the ISDS. I
asked Ron Grout not to cover up the system until this approval was given, because I thought that there might be a
possibility that they would want to run the barn drainage into the septic tank. They put in a 1500 gallon tank, and
1044 square feet of leach field, for a one bedroom house. They submitted a letter enclosed in the file on your desk.
Please review and let Ron Grout (328-6209) know if he can cover up the system or if things need to change.

PERCOLATION TEST RESULTS
Six (6) percolation tests were performed in the clays at locations shown as P-1
through P-6 on Figure 1. We recommend using a percolation rate of 60 minutes/inch for
the caretakers apartment and stables site and 100 minutes/inch at the residence site for
percolation field design. Results are shown on Figures 5 through 10.
SURFACE DRAINAGE
The performance of foundations and concrete flatwork is influenced by moisture
conditions in the subsoils. Wetting of foundation soils can be reduced by grading the
ground surface to cause rapid run-off of water away from the buildings. Wetting or drying
of the open foundation excavations should be avoided. The ground surface surrounding
the buildings should be sloped to drain away from the buildings in all directions. We
recommend a slope of at least 12 inches in the first 10 feet. Roof downspouts and drains
should discharge well beyond the limits of all backfill. Splash blocks or downspout
extensions should be provided at all discharge locations. We recommend allowing
sufficient clearance between the caretakers apartment and stables and the excavated
Islopes at the back of the pad for the snow removal equipment.
LIMITATIONS
' At the time of our investigations plans which showed the locations of the planned
buildings were not available. Our test holes were spaced to obtain a reasonably accurate
picture of the subsurface. Variations in these subsurface conditions not shown by ourtest
' holes will occur. We need to observe the completed excavations to confirm the soils at
the actual building locations are as anticipated from our test holes.

Our report was based on conditions disclosed by our test holes, results of
i_
laboratory testing, engineering analysis and our experience. Criteria presented reflects
L' the proposed buildings as we understand them. We should be advised if the final design
differs from our assumptions to permit us to re-evaluate our conclusions and
IJ recommendations.
This investigation was conducted in a manner consistent with the level of care and
skill ordinarily exercised by members of the profession currently practicing under similar
