Please note that the tank configurations (septic, dose, and denitrification unit) shall be placed outside of the building envelope. The building sewer must stub out from the building to maintain at least %" per foot of slope. 90 degree elbows should not be used in the building sewer configuration, instead 90 degree sweeps or 45- degree fittings will be acceptable. Jamie Pappas June 14, 2004 Vail Resorts Development Company PO Box 959 Avon, CO 81620 Dear Jamie: Per your request, I am providing you with the methodology utilized in leak testing the dose and septic tanks located on Lot 52. LOT 52 1000 Gallon Tank: On October 13, 2003 at approximately gam NexGen filled the 1000 gallon tank with water. A measurement was taken from the lip of the manhole ring and cover down to the surface of the water. The distance was 54.5". This water level was at the invert of the outlet pipe on the tank. NexGen returned on October 15, 2003 at approximately l lam and again measured the depth from the lip of the manhole ring and cover to the surface of the water. A measurement of 54.5" was recorded showing no measurable water loss had occurred. 1500 Gallon Tank On October 13, 2003 at approximately 9:30am NexGen filled the 1500 gallon tank with water. A measurement was taken from the lip of the manhole ring and cover down to the surface of the water. The distance was 67.75". This water level was at the invert of the outlet pipe on the tank. NexGen returned on October 15, 2004 at approximately 11:30am and again measured the depth from the lip of the manhole ring and cover to the surface of the water. A measurement of 67.75" was recorded showing no measurable water loss had occurred. Squirt Test On 2pm October 19, 2003, pumps from Lot 52 operational and adequately delivering effluent. Squirt test was performed and results were approximately 6'-7' of vertical squirt. All flushing assemblies were functional. Michael and Jeff from Fluidyne and I spent last week Thursday, Friday and Saturday inspecting all the systems located in Red Sky Ranch. We did come up with a punch list of a few items needing some attention, (buried inspection covers, floats set to high or to low, etc.) overall, everything is working properly. Permit number 2314-03, lot 88 owned by Jim Thompson. Jim has decided not to build at this time, if the permit has not already expired please extend it for the remainder of the year. If Jim does not finalize the plans by the end of December we can let permit expire, and issue a new permit when he is ready to build. If the permit has now expired please let me know so I can cross it off my active list also. Permit number 2270-03, lot 75, Three "R" Adventures This system was water tightness tested in November of 2004, was squirt tested in March of 2005. As built swing ties have been sent to Tom by Deb on April 4, 2005. After the Fluidyne site visit last week, Tom should be able to supply certification for this system. Permit number 2330-04, lot 59, Head Property Corp. This system was water tightness tested in November of 2004, was squirt tested in March of 2005. This system feeds a community leach field, testing confirmed effluent arrival at the community dose tank by visually verifying the increased flow. As built swing ties have been sent to Tom by Deb on April 4, 2005. After the Fluidyne site visit last week, Tom should be able to supply certification for this system. Permit number 2285-03, lot 71, Kaplan This system was water tightness tested in November and Squirt tested in March of 2005. In April 2005. In Deb's files I found a copy of a faxed letter to Eagle County from Tom Kallenbach accepting the installation as complete. The letter did state he was mailing the as -built drawings. This system should be waiting for your approval. Let me know if you need an additional copy of the letter, or if the drawings are still outstanding. Permit number 2231-02 lot 52, Lot 52 was installed 2 years ago, water tightness tested and squirt tested by NexGen construction. The house is nearing completion and waiting for TCO. I have not been able to find any records on this system. Michael and Jeff from Fluidyne and I inspected this system last week. Tom should be able to sign off the completion based on Michael's testing and notes. I will need to forward as -built swing tie measurements to Tom today for completion of the drawings. Tom should be able to supply the certification letter as soon as he receives the swing tie measurements from me. Permit number 2272-03 lot 20, Gregg This system is completed and installed correctly. The location drawings need to be completed. The system has had the water tightness test completed. Michael and Jeff inspected this one this past it week- There is still no power hooked up to the control panel. The squirt test and pump float alibration will be performed and the system finalized within the next few weeks. This home should be ready for a TCO sometime mid -summer. I am confident we can have completed information to you before TCO is requested. Permit number 2329-04 lot 7, Bard This system is completed and installed correctly. The location drawings need to be completed. The system has had the water tightness test completed. Michael and Jeff inspected this one this past week. There is still no power hooked up to the control panel. The pump test and float calibration will be performed as soon as power is supplied by the builder. This pumps into a common leach field located on the Fazio hole 18 fairway. This home should be ready for a TCO sometime late -summer, am confident we can have completed information to you before TCO is requested. Lot 26, not yet permitted. If you have not yet received the application, Tom should be sending it along with all requested information. I will be listed as the installer and Community Concepts as the property owner. I received your notice on the red flag of the work. This is being done by Community Concepts, not Vail Resorts. My understanding is they had already stopped work prior to the red flag. Community Concepts had anticipated a building permit sooner than this and had installed micro piles and other work in preparation for the first inspection. PO Box 850 500 Broadway Eagle Colorado, 81631 RE: Red Sky Ranch Lot 52 Septic inspection * as 3 ( — o ;2— Dear Ray, FIECEdVIED JUL 11 2005 This letter is to inform you that the ISDS testing requirements for Red Sky Ranch lot 52 have been completed. Derek Woodland, COO, NexGen Construction performed the water tightness and squirt tests as directed by Fluidyne. I have included a copy of the letter from Ed Woodland CEO of NexGen explaining the test results. Water Tightness test, Septic and dose tanks Performed by NexGen on October 13, 2003. Squirt test Performed by NexGen on October 15, 2003. As Built Drawings These tanks were installed per plan; swing tie measurements have been added to the attached diagram. Other Inspections Michael Koller with Fluidyne, and myself inspected this system on June 24, 2005. We found a blown control panel fuse and a float problem. It appears this panel had been hit by lightening. The problems have since been repaired and the system is now ready for operation. After the fuses and float were fixed, the inspection covers on the Eliminate tank and the leach field laterals were opened by Bryan Crossly and myself, visually verifying proper flows. We then manually activated the system floats to verify proper control functions. Using this information and the attached certification letter from the design engineer, Thomas Kallenbach, PE. I am asking for final approval of this system. Please contact me directly with any questions.
