NOT INSTALL IN WET WEATHER. SCARIFY ALL TRENCH SURFACES PRIOR TO PLACING THE INFILTRATOR UNITS. BE
SURE TO MAINTAIN ALL APPLICABLE SETBACK REQUIREMENTS. ANY VARIATIONS TO THE 12/17/02 PLANS REQUIRE
APPROVAL FROM FLUIDYNE ENGINEERING AND EAGLE COUNTY ENVIRONMENTAL HEALTH PRIOR TO THE WORK
BEING PERFORMED. CERTIFICATION OF THE INSTALLED SYSTEM IS REQUIRED FROM FLUIDYNE ENGINEERING AND

2. THIS PERMIT IS VALID ONLY FOR CONNECTION TO STRUCTURES WHICH HAVE FULLY COMPLIED WITH COUNTY ZONING AND BUILDING
REQUIREMENTS. CONNECTION TO, OR USE WITH ANY DWELLING OR STRUCTURE NOT APPROVED BY THE ZONING AND BUILDING
DEPARTMENTS SHALL AUTOMATICALLY BE A VIOLATION OF A REQUIREMENT OF THE PERMIT, AND WILL RESULT IN BOTH LEGAL ACTION
AND REVOCATION OF THE PERMIT.
3. CHAPTER IV, SECTION 4.03.29 REQUIRES ANY PERSON WHO CONSTRUCTS, ALTERS OR INSTALLS AN INDIVIDUAL SEWAGE DISPOSAL SYSTEM
TO BE LICENSED.

Re: Lot 65 Application for Individual Sewage Disposal System Permit
Ray Merry:
Included in this submittal are the application and supporting plan sheets to obtain
an ISDS Permit for Lot 65 of the Red Sky Ranch Subdivision. Please note that
the tank configurations (septic, dose, and denitrification unit) shall be placed
outside of the building envelope. The building sewer must stub out from the
building to maintain at least'/4" per foot of slope. 90 degree elbows should not
be used in the building sewer configuration, instead 90 degree sweeps or 45-
degree fittings will be acceptable.
The submittal includes the following items:
• Completed Application for ISDS Permit
• Submittal Fee of 350$
• Site Plan
• Wastewater System Detail Sheets

Property Location: 232 Stag Horn Trail, Wolcott, CO., Malernee residence.
Enclosed is your ISDS Permit #2236-02. You will be receiving the construction plans from the
engineer with the permit image on the fron page. The permit is valid.for 120 days or for the
duration of your current building permit for this property. The enclosed copy of the permit must
be posted at the installation site. Any changes in plans or specifications invalidates the
permit unless otherwise approved. Please note any special requirements that may have
been added to the design by this Department.
Systems designed by a Registered Professional Engineer must be certified by the engineer
indicating that the system was installed as specified. Eagle County Environmental Health
must also view the installation prior to back filling. Please call well in advance for your
final inspection. Your TCO will not be issued until our office receives this certification
from the engineer and views the installation.
Due to the onset of winter, we anticipate your installation to occur after
March 15, 2003 when our ISDS program is expected to resume. Please
contact this office in the spring of 2003 to update us regarding your
construction schedule so we can prepare to provide you with the best service
possible.
Please note that our ISDS Permit specifications are typically minimum requirements only and
should be brought to the property owner's attention in case they may wish to install a system
capable of handling larger wastewater flows.
This permit does not indicate conformance with any other Eagle County requirements.
Please notify this office if you have not been contracted to perform this installation.

Dear Adie,
This letter is to iaform you that I have completed the ISDS testing requirements at Red
- - Sky Ranch -Lot 65. The 'water tightness and pressure Squirt tests were performed as
directed by Fluidyne and are discussed below,
Septic/Dose Tank Water Tightness Test
This test was actually performed by ramie Pappas in July 2004 and my inspection on
10/25/04 indicates that the tanks are still completely full_ Therefore, there has been no
apparent leakage over the last three -months.
Squirt Test
Ou the morning of 10/25/04 I verified that there was equal pressure distribution to each
of the four drain field laterals for this system. Each lateral squirted a stream to a height
of between 4.5 and 5.0-feet above ground level through a 3/16-inch hole.
As -Built Drawing
This system was built per Fluidyne's specifications but the locations of the tanks were
modified slightly to accommodate field conditions. Attached is the swing -tie information
for you to prepare an as -built record drawing.
Using this information, I am requesting that you prepare a stamped letter addressed to
Ray Merry at Eagle County verlfyAng completion of this ISDS so that the builder can
schedule a final certificate of occupancy inspection.

Install the OWTS exactly as depicted in the Eliminite Inc design issued September23,2017, stamped and
signed by Thomas J.Kallenbach, P.E.This system was designed to serve the two bedroom bunkhouse
addition at this address.
The OWTS consists of two 1,500 gallon Infiltrator IM-1530 tanks in series with the primary tank followed
by the secondary advanced treatment tank.The advanced treatment tank will be assembled with
complete installation and connection of drainfield pump and recirculation pump by Eliminite.Pressurized
effluent is pumped to 1,000 feet of soil treatment area (STA) (prior to 20%reduction for pressure dosed
application to a trench STA)via 68 chambers arranged in four shallow trenches with17 chambers in each.
Install the flushing assembly as shown on the engineer's design, along with inspection ports on both
ends of each trench.Scarify bottom of trenches prior to placing sand fill material and chambers.
Notes:Holland Creek Metro District is to verify the soil profile and conditions during excavation.Obtain
all necessary electrical permits for the control panel.
Contact Eagle County and the design engineer well in advance of any required inspections.Engineer

Thanks for dropping off payment for the OWTS Permit. We logged payment on June 27, 2017 and you can refer to
OWTS Permit 6-17-17086 in future correspondence.
Tom had asked that we provide you a list of things that need to be addressed in order to meet our current regulations.
Please recall that the information you had on file and submitted previously appeared to be a work in progress in 2013
between Holland Creek's Don Odell and Tom. We adopted new regulations to conform to WQCC Reg. 43 in 2014. This
bunkhouse project was placed on hold by the owner in 2013 and now its moving forward.
We've reviewed the preliminary information you sent to us as a starting point. The following is a list of items that are
absent or need to be updated in order to have a design packet we can permit.
Prior to our new regulations, soil information at Red Sky Ranch was performed on a regional basis for OWTS design
purposes. 8' soil profile information on at least two pits adjacent to the soil treatment area are necessary if previously
collected soil information was not obtained at or near this site.
The new regulations require the soil treatment area to be based upon USDA soil classifications in 43.10 (Table 10-1) to
determine a Long Term Acceptance Rate (LTAR). The draft design calculations indicate a "soil application rate" of 0.75
gallons/sq.ft./day so the redesign must indicate either 0.6 or 0.8 LTAR, should the USDA classification match. The draft
calculations for estimated flow must be redone to indicate a 2 bedroom bunkhouse flow of 300 gallons per day and the
minimum area (prior to taking any adjustments (if desired) for pressure dosing (20%) and for using chambers (30%).
The cut sheets for the Eliminite system should be included along with pump information and soil treatment area layout,
showing the details of components, etc. Keep in mind that the Eliminite components must be installed by an individual
sanctioned by the manufacturer while the soil treatment area can be installed by Stenson Excavating.
Details such as the control panel, float tree assembly, orifice size and intervals, end trench cleanout detail for squirt
testing, components accessible at grade, installation details with notes, etc. are typically supplied with the final, stamped
drawing.

Thanks for the clarification. We understand the proposed bunkhouse will be served by it's own OWTS.
The 2013 plans appear to have been in the developmental stage between Holland Creek and the design engineer, Tom
Kallenbach, as one of the sheets isn't stamped or labeled and includes comments from Don Odell which would
normally have been incorporated into a final set of plans.
We are still waiting for the check to arrive so we can initiate the OWTS permitting and review process. In the
meantime, one thing you can do is fill out an updated OWTS application with your information and a current licensed
contractor's information. If you are using Stenson Excavating, Bill's current license number is 7-17.
Once we get the check and create the file, we will initiate the review and provide a list of the basic information
necessary to conform to the 2014 Eagle County Public Health Agency OWTS Regulations and email it to you with a

We are unsure what is being proposed relative to the wastewater needs. Is the proposal to connect the bunkhouse
plumbing to the existing OWTS that serves the residence; or is the bunkhouse going to be served by a stand-alone
OWTS?
We didn't see any new engineered sewer line details showing connection or location details nor did we notice a
separate set of engineered plans for a stand-alone OWTS.
Please explain why the application indicates a new system is being proposed. Connecting to the existing OWTS
would trigger an alteration permit and an evaluation of the existing system's capability of handling the additional
wastewater flows/strength.

taker house on 232 Stag Horn in Red Sky. The building design is also attached. The letter and design are from
2013. The only changes made to the design of the care taker house is the addition of a garage. The square footage
is the same. I will have that district manager submit a check for $800 ASAP.

WQCP LLC conducted the tests and verifications that were requested in order for the design engineer to
sign off on the system.
Watertight test: The water tightness test was not necessary with this install. The tanks were
preassembled at the Eliminite shop and delivered to Colorado.
Drain field test: The drain field test was conducted on 3/26/2018. The flushing valves were opened,
then the laterals were flushed before the start of the test. Since the drain field was already backfilled a
modified squirt test was conducted using the capped cleanout ends. The caps were replaced with caps
that had 3/16” holes drilled in them. The drain field pump was started and the results were as follows:
Lateral #1 –6 ’
Lateral #2 – 5’
Lateral #3 – 7’
Lateral #4 – 8’
Installation: The Eliminate system was constructed per the drawings received from Tom Kallenbach PE.
The primary and Eliminite tanks were installed per the drawing. A 2” force main leaves the pump
chamber and discharges into four 1 1/2” laterals that run to the west. The laterals are 70’ long. 3/16”
orifices were drilled every 60”.
Pump check: Float system was tested and working properly. Eliminite pump 4.2 amps. Drain field pump
9.1 amps.
