"PLEASE READ DIRECTIONS THOROUGHLY BEFORE ASSEMBLING TANK
These suggested steps should be followed to insure proper assembly and
installation of the septic tank. These tanks must be assembled on a
warm day or in a building with a temperature of at least 50°F.
1. Place the bottom half of the tank on a level
open area. Be sure that a man can walk
around all sides.
2. Apply the sealer to the bottom half of the
tank by removing the paper from one side
and pressing it into place. Once it is in place
remove the paper from the top side of the

sealer. Place one strip of sealer in the baffle
and
groove of the bottom half. This is to keep the

baffle from moving during assembly and in-

3. Place the baffle into the groove in the bottom
half of the tank. Press it well down into the
sealer.
4. Place the top half of the tank onto the bottom
half being sure to align it for the best fit.
5. Use a drill with a 1/4" drill bit and drill through
the existing holes in the top flange through
the sealer and through the bottom flange of
the tank. Place the 1/4" bolts through these
holes and tighten uniformly. The tank is now
ready for installation.
6. Place the tank in position in the hole with the
inlet and outlet ends as needed. The baffle
end of the tank or the smaller compartment
of the tank is the outlet end.
7. Cement a piece of sewer pipe 9" long into
the bottom of a 4" sewer Tee for the inlet
side. Insert a piece of 4" sewer pipe through
the inlet end of the tank until it is under the
manway. Cement the tee onto the inlet line
with the 9" extension facing down. Pull the
inlet line back until the tee is against the side
of the tank. Continue attaching the rest of
the drain line to the exposed end of the pipe.
8. For the outlet fittings attach a 21" piece of
pipe to the bottom of a sewer tee and con-
nect in the same manner as the inlet line.
9. Install the covers of the tank and extend a
piece of pipe from the collar in the tank
covers to just below ground level and cap it.
This extension is to be used to pump the
sediment from the. tank without digging
down to the top of the tank.
10. Backfill and water pack around the tank. It
is usually best to fill the tank at the same
rate as it is being backfilled and packed.
This exerts even pressure on the walls of
the tank. If possible hand fill around the
tank being careful not to drop large rocks
or chunks of dirt onto it.
11. Be sure to seal between the outlet line and
the wall of the tank with sealer provided
with the tank.
CAUTION: Do not install where vehicles can travel over
the location of the tank. Not recommended
for installation over four feet below grade.

RE: Issuance of Individual Sewage Disposal System Permit #2148-01, Tax Parcel #2111-
091-01-003. Property Location: 1851 Cooley Mesa Rd, Gypsum, CO., Faulkenberg
residence.
Enclosed is your ISDS Permit #2148-01. It is valid for 120 days. The enclosed copy of the permit
must be posted at the installation site. Any changes in plans or specifications invalidates the permit
unless otherwise approved. If you have not been contracted to install this system please contact
Eagle County Environmental Health.
Also enclosed is the ISDS Final Inspection Completeness Form. The items on this form need to be
completed before you call for your final inspection. Also, please note any special conditions which
may have been placed on the permit. Do not back fill any part of the installation until it has been
inspected. If all items are not completed, a reinspection fee of $47.00 must be paid before a
reinspection is made.
Due to the onset of inclement weather, all installations must be completed prior to
December 1, 2001 in order for Eagle County Environmental Health to perform a final
inspection. In the event that inclement weather interrupts your installation, please
cover the installed components with plastic sheeting so that they are not covered with
snow and are visible for the inspector. All field work will resume, weather permitting,
on March 15, 2002.
Please call our office well in advance to allow for scheduling of final inspection. Your building
permit TCO will not be issued until final approval has been given for the ISDS Permit.
Permit specifications are minimum requirements only, and should be brought to the property owner's
attention.
This permit is for your proposed main dwelling on your lot zoned AL. It is our understanding
there is an existing dwelling on your lot. The AL zone district restricts the square footage of
accessory dwelling units to 850 square feet. Therefore, please be advised that the issuance of
this permit does not indicate conformance with other Eagle County requirements, especially as
it relates to the zoning of the property. Zoning regulations may not recognize both dwellings as
legal land uses.

Subject: Observation of Excavation and Percolation Testing, Proposed Residence,
1851 Cooley Mesa Road, Gypsum, Colorado
Dear Mr. Faulkenberg:
As requested, a representative of Hepworth-Pawlak Geotechnical, Inc. observed the
excavation and performed percolation testing at the subject site on September 26, 2000
to evaluate the soils exposed for foundation support and the feasibility of a conventional
infiltration septic system. The findings of our work and recommendations for the
foundation design are presented in this report. The work was done in accordance with
our agreement for geotechnical engineering services to you, dated September 21, 2000.
The proposed residence will be one story log construction over a walkout basement.
Ground floor will be slab -on -grade. The proposed septic system will be located
southeast and downhill of the residence.
At the time of our visit to the site, the foundation excavation had been cut in one level
up to 8 feet below the adjacent ground surface. The frost wall on the south side of the
house had not been excavated. The soils exposed in the bottom of the excavation
consisted of mostly sandy silt and clay. Eagle Valley Evaporite bedrock with gypsum
was observed in the southwest corner and on the west side of the excavation. Results of
swell -consolidation testing performed on a sample taken from the site, shown on Fig. 2,
indicate the soils are moderately compressible under conditions of loading and wetting
and have a low collapse potential (settlement under constant load) when wetted. No
free water was encountered in the excavation and the soils were slightly moist.

The site is underlain by Pennsylvania Age Eagle Valley Evaporite bedrock. The
evaporite contains gypsum deposits. Dissolution of gypsum under certain conditions
can cause sinkholes to develop and can produce areas of localized subsidence. Based on
our present knowledge of the site, it cannot be said for certain that sinkholes will not
develop. In our opinion, the risk of ground subsidence at this site is low but the owner
should be aware of the potential for sinkhole development.
The recommendations submitted in this letter are based on our observation of the soils
exposed within the foundation excavation and do not include subsurface exploration to
evaluate the subsurface conditions within the loaded depth of foundation influence.
This study is based on the assumption that soils beneath the footings have equal or
better support than those exposed. The risk of foundation movement may be greater
than indicated in this report because of possible variations in the subsurface conditions.
In order to reveal the nature and extent of variations in the subsurface conditions below
the excavation, drilling would be required. It is possible the data obtained

subsurface exploration could change the recommendations contained in this letter.
