Two -sheet set of stamped and signed plans, dated July 28, 2004 for the septic system repair on the Brownson property. Two stamped originals attached. 2. Eagle County permit application, started by Gamba & Associates, Inc., completed by Gary Brownson, for the name and license number of the contractor he proposes to use. 3. Permit Application Fee of $350.00 from Brownson. 4. Percolation test results for the site. You have, I believe, visited the site, and observed the profile hole. I have noted on the plans that all materials from the existing field that have to be removed shall be disposed of in accordance with State of Colorado and Eagle County regulations. I am proposing to re -use the existing piping from the septic tank to the field area, and install an effluent filter just before the trenches. If you have any questions, please call. I believe that everything else related to this repair is in your hands. I have used the EPA Long Term Acceptance Rates (LTAR) for the calculation of the field size. The field size has been increased by 25 Quick4 units. The owner has recently informed me that since the previous renters have moved out, the system has not shown any signs of failure. It seems likely that the failure was related to the number of persons in the building, and the apparent use of the building's facilities for more than the septic system was designed to accommodate. The permit is valid for 120 days or for the duration of your current building permit for this property. The enclosed copy of the permit must be posted at the installation site. Any changes in plans or specifications invalidate the permit unless otherwise approved. Please note any special requirements that may have been added to the design by this Department. Systems that are designed by a Registered Professional Engineer must be certified by the engineer indicating that the system was installed as specified. Eagle County Environmental Health must also view the installation prior to back filling. Please call well in advance for your final inspection. Your TCO will not be issued until our office receives this certification from the engineer and views the installation. Please note that our ISDS Permit specifications are typically minimum requirements only and should be brought to the property owner's attention in case they may wish to install a system capable of handling larger wastewater flows. This permit was for a replacement system to be installed at your home at 002439 State Highway 82, outside of Basalt. I have never been asked to inspect the system, and I have no idea if it was even installed. The last communication in my electronic files is a memo that I sent to Randy Hughes at Hughes Excavating, on May 13", 2005. The memo indicates that the complete plans package, and a copy of the Eagle County permit, had been sent to Randy on March 3`d, of 2005.
