design packet prepared by Carla Ostberg, stamped, signed and dated September 18, 2019, by Richard H. Petz, PE.
The replacement tank is a 1,500 gallon two compartment Infiltrator poly septic tank with effluent filter on the outlet
tee. Risers must be added as necessary to bring both access manholes to or above grade. The old tank is to be
abandoned in place as specified in43.9(C.).
Install a double-sweep clean out as specified on the design drawings between the tank and the house. Replace cast
iron sewer pipe as necessary with ASTM Standard D 3034 or greater strength, schedule 40 is preferred. Be sure to
stake soil treatment area to prevent soil compaction from heavy equipment or tank staging.
Contact design engineer to perform all inspections necessary to certify proper installation. Engineer certification
must be submitted to and approved by Eagle County Environmental Health along with a record drawing and photos
prior to the use of the new tank.

Onsite Wastewater Treatment System (OWTS) Installation Observations

ALL SERVICE septic, LLC observed the installation of the onsite wastewater treatment system (OWTS)
on April 3, 2020 for the subject property. Hagist Excvation installed the system.
The existing septic tank was pumped and the sides and top collapsed and the void filled with onsite soils.
A new 1500-gallon, two-compartment Infiltrator® septic tank was installed with Orenco® Biotube effluent
filter on the outlet tee. This filter must be cleaned annually, or as needed.
B&R jetted the sewer line exiting the septic tank and STA after the new septic tank installation.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are
made to the OWTS in the future. Any additional OWTS construction must be according to the county
regulations.
LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent of the expertise of the installer, soil
type, and weather conditions.

Onsite Wastewater Treatment System Design – Septic Tank Replacement

ALL SERVICE septic, LLC has completed a septic tank replacement design for the subject residence.
The legal description of the 2.0-acre property is as follows:

CBO Inc. performed an Onsite Wastewater Treatment System (OWTS) inspection at of the subject
OWTS on May 3, 2009. The inspection report is enclosed.
SITE CONDITIONS
The subject OWTS consists of a two-compartment concrete septic tank (either 1000-gallons or 1250-
gallons) followed by a soil treatment area (STA). Documents note the STA was to provide a minimum
infiltrative area of 800 square feet and the file “as-built.pdf” notes a 24’ x 42’ STA. If the system was
installed in 1978, we assume the STA consists of pipe and gravel.
Although the system is experiencing no issues at this time, there is a desire to upgrade the septic tank
and potentially the sewer line from the house to the septic tank.
DESIGN SPECIFICATIONS
The existing septic tank will be pumped and the sides and top collapsed and the void filled with onsite
soils or the tank may be removed.
For the purposes of this OWTS design, Benchmark Elevation has been established as 100’ (existing
sewer line exiting the residence). ALL SERVICE septic, LLC should be notified of any discrepancies or
problems with grade elevations of proposed components during installation of the OWTS.

The existing sewer line may be cast iron, which is prone to clogging and deterioration. The sewer line
must be explored and if found to be in poor condition, it must be replaced. A double-sweep clean out
must be added to the sewer line between the residence and the new septic tank. The sewer line must
maintain a minimum 2% grade from the house to the septic tank.
A new 1500-gallon, two-compartment Infiltrator® septic tank must be installed with Orenco® Biotube
effluent filter on the outlet tee.
The sewer line to the STA must be replaced if found to be cast iron. The existing STA will remain in
use.
The component manufacturers are typical of applications used by contractors and engineers in this area.
Alternatives may be considered or recommended by contacting our office. Construction must be
according to Eagle County On-Site Wastewater Treatment System Regulations, the OWTS Permit provided
by Eagle County Environmental Health Department, and this design.
INSTALLATION CONTRACTOR
ALL SERVICE septic, LLC expects that the installer be experienced and qualified to perform the scope of
work outlined in this design. The installer must review this design thoroughly and coordinate with our office
in advance of installation. Any additional conditions in this design or county permit must be completed and
documented prior to final approval of the OWTS installation. Communication between the installer and this
office is expected throughout the installation.
INSTALLATION OBSERVATIONS
ALL SERVICE septic, LLC must view the OWTS during construction. The OWTS observation should be
performed before backfill, after placement of OWTS components. Septic tanks, distribution devices,
pumps, dosing siphons, and other plumbing, as applicable, must also be observed. ALL SERVICE septic,
LLC should be notified 48 hours in advance to observe the installation.
REVEGETATION REQUIREMENTS
An adequate layer of good quality topsoil capable of supporting revegetation shall be placed over the entire
disturbed area of the OWTS installation. A mixture of native grass seed that has good soil stabilizing
characteristics (but without taproots), provides a maximum transpiration rate, and competes well with
successional species. No trees or shrubs, or any vegetation requiring regular irritation shall be placed over
the STA. Until vegetation is reestablished, erosion and sediment control measures shall be implemented
and maintained on site. The owner of the OWTS shall be responsible for maintaining proper vegetation
cover.
OPERATION INFORMATION AND MAINTENANCE
The property owner shall be responsible for the operation and maintenance of each OWTS servicing the
property. The property owner is responsible for maintaining service contracts for manufactured units,
alternating STAs, and any other components needing maintenance.
Geo-fabrics or plastics should not be used over the STA. No heavy equipment, machinery, or materials
should be placed on backfilled STA. Livestock should not graze on the STA. Plumbing fixtures should be
checked to ensure that no additional water is being discharged to OWTS. For example, a running toilet or
leaky faucet can discharge hundreds of gallons of water a day and harm a STA.

If an effluent filter or screen has been installed in the OWTS, we recommend this filter or screen be cleaned
annually, or as needed. If the OWTS consists of a pressurized pump system, we recommend the laterals be
flushed annually, or as needed.
The homeowner should pump the septic tank every two years, or as needed gauged by measurement of
solids in the tank. Garbage disposal use should be minimized, and non-biodegradable materials should not
be placed into the OWTS. Grease should not be placed in household drains. Loading from a water softener
should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS.
Mechanical room drains should not discharge into the OWTS. The OWTS is engineered for domestic waste
only.
ADDITIONAL CONSTRUCTION NOTES
If design includes a pump, weep holes must be installed to allow pump lines to drain to minimize risk of
freezing. The pump shall have an audible and visual alarm notification in the event of excessively high-
water conditions and shall be connected to a control breaker separate from the high water alarm breaker
and from any other control system circuits. The pump system shall have a switch so the pump can be
manually operated.
Excavation equipment must not drive in excavation of the STA due to the potential to compact soil.
Extensions should be placed on all septic tank components to allow access to them from existing grade.
Backfill over the STA must be uniform and granular with no material greater than minus 3-inch.
LIMITS:
The design is based on information submitted. If soil conditions encountered are different from conditions
described in report, ALL SERVICE septic, LLC should be notified. All OWTS construction must be
according to the county regulations. Requirements not specified in this report must follow applicable
county regulations. The installer should have documented and demonstrated knowledge of the
requirements and regulations of the county in which they are working.

As requested, CBO Inc. performed an Onsite W astewater Treatment System (OWTS) inspection at 4553
Frying Pan Road, Basalt, Colorado on May 3, 2019. The legal description of the 2.0-acre property is as
follows:

We were provided the attached records on the property from Eagle County Environmental Health
Department. Permit No. 239 was issued for a 4-bedroom duplex. The current residence has 4-
bedrooms and the proposed remodel will have no more than 4-bedrooms.
Other documents indicate issues with the site plan that was provided by the then-property owner. There
is conflicting information on the county documents as to whether the septic tank was 1000 gallons or
1250 gallons. The drawing provided by the county, with the electronic file name “as-built.pdf” notes a
1250-gallon septic tank. The septic permit never received final approval. B&R Septic pumped the septic
tank in June 2016. The invoice was for 1000-gallons; however, this may not be accurate. The difference
in 250 gallons is sometimes difficult to determine in the pump truck.
We were also provided documents for a system installed in 2001, but we believe this is a neighboring
property.
At the time of our inspection, both septic tank lids were accessible from grade. Given the age of the
structure, it is possible that the sewer line from the house to the septic tank could be cast iron. Cast iron
is prone to clogging and deterioration. We recommend this sewer line be explored to assess the
condition of the sewer line.
The subject OW TS consists of a two-compartment concrete septic tank (either 1000-gallons or 1250-
gallons) followed by a soil treatment area (STA). Documents note the STA was to provide a minimum
infiltrative area of 800 square feet and the file “as-built.pdf” notes a 24’ x 42’ STA. If the system was
installed in 1978, we assume the STA consists of pipe and gravel.
At the time of our inspection, both manhole lids were accessible from grade. Three feet of risers were
added to each side of the septic tank. Both inlet and outlet tees were present. The outlet tee is tucked
under the lid of the septic tank, making our typical recommendation of adding an effluent filter not
feasible. The outlet tee is cocked up slightly, indicating possible settling of the pipe outside of the septic
tank. Levels in the septic tank were normal at the time of our inspection.
We walked the area of the STA and no inspection ports were observed. We found no evidence of
surface saturation or failure at the time of our inspection.
This evaluation is not a guarantee of future system performance.

based upon what was observed and the Licensed Inspector’s expertise in onsite wastewater technology.
The Inspector has not been retained to warrant or guarantee the proper functioning of the system for any

as well as the inability of the Inspector to supervise or monitor the use or maintenance of this OWTS, this

arising from the inspection of the OWTS or this report. The evaluation does not ascertain the impact the

to be completed before you call for your final inspection. Also, please note any special
conditions which may have been placed on the permit. Do not back fill any part of the
installation until it has been inspected. If all items are not completed, a reinspection fee of
$47.00 must be paid before a reinspection is made.
Please call our office well in advance to allow for scheduling of final inspection. Your building
permit TCO will not be issued until final approval has been given for the ISDS Permit.
Permit specifications are minimum requirements only, and should be brought to the property
owner's attention.
This permit does not indicate conformance with other Eagle County requirements.

Replacement of the septic tank originally permitted under #239IS exactly as depicted in the All Service Septic, LLC
design packet prepared by Carla Ostberg, stamped, signed and dated September 18, 2019, by Richard H. Petz, PE.
The replacement tank is a 1,500 gallon two compartment Infiltrator poly septic tank with effluent filter on the outlet
tee. Risers must be added as necessary to bring both access manholes to or above grade. The old tank is to be
abandoned in place as specified in43.9(C.).
Install a double-sweep clean out as specified on the design drawings between the tank and the house. Replace cast
iron sewer pipe as necessary with ASTM Standard D 3034 or greater strength, schedule 40 is preferred. Be sure to
stake soil treatment area to prevent soil compaction from heavy equipment or tank staging.
Contact design engineer to perform all inspections necessary to certify proper installation. Engineer certification
must be submitted to and approved by Eagle County Environmental Health along with a record drawing and photos
prior to the use of the new tank.

ALL SERVICE septic, LLC observed the installation of the onsite wastewater treatment system (OWTS)
on April 3, 2020 for the subject property. Hagist Excvation installed the system.
The existing septic tank was pumped and the sides and top collapsed and the void filled with onsite soils.
A new 1500-gallon, two-compartment Infiltrator® septic tank was installed with Orenco® Biotube effluent
filter on the outlet tee. This filter must be cleaned annually, or as needed.
B&R jetted the sewer line exiting the septic tank and STA after the new septic tank installation.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are
made to the OWTS in the future. Any additional OWTS construction must be according to the county
regulations.
LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent of the expertise of the installer, soil
type, and weather conditions.

CBO Inc. performed an Onsite Wastewater Treatment System (OWTS) inspection at of the subject
OWTS on May 3, 2009. The inspection report is enclosed.
SITE CONDITIONS
The subject OWTS consists of a two-compartment concrete septic tank (either 1000-gallons or 1250-
gallons) followed by a soil treatment area (STA). Documents note the STA was to provide a minimum
infiltrative area of 800 square feet and the file “as-built.pdf” notes a 24’ x 42’ STA. If the system was
installed in 1978, we assume the STA consists of pipe and gravel.
Although the system is experiencing no issues at this time, there is a desire to upgrade the septic tank
and potentially the sewer line from the house to the septic tank.
DESIGN SPECIFICATIONS
The existing septic tank will be pumped and the sides and top collapsed and the void filled with onsite
soils or the tank may be removed.
For the purposes of this OWTS design, Benchmark Elevation has been established as 100’ (existing
sewer line exiting the residence). ALL SERVICE septic, LLC should be notified of any discrepancies or
problems with grade elevations of proposed components during installation of the OWTS.

The existing sewer line may be cast iron, which is prone to clogging and deterioration. The sewer line
must be explored and if found to be in poor condition, it must be replaced. A double-sweep clean out
must be added to the sewer line between the residence and the new septic tank. The sewer line must
maintain a minimum 2% grade from the house to the septic tank.
A new 1500-gallon, two-compartment Infiltrator® septic tank must be installed with Orenco® Biotube
effluent filter on the outlet tee.
The sewer line to the STA must be replaced if found to be cast iron. The existing STA will remain in
use.
The component manufacturers are typical of applications used by contractors and engineers in this area.
Alternatives may be considered or recommended by contacting our office. Construction must be
according to Eagle County On-Site Wastewater Treatment System Regulations, the OWTS Permit provided
by Eagle County Environmental Health Department, and this design.
INSTALLATION CONTRACTOR
ALL SERVICE septic, LLC expects that the installer be experienced and qualified to perform the scope of
work outlined in this design. The installer must review this design thoroughly and coordinate with our office
in advance of installation. Any additional conditions in this design or county permit must be completed and
documented prior to final approval of the OWTS installation. Communication between the installer and this
office is expected throughout the installation.
INSTALLATION OBSERVATIONS
ALL SERVICE septic, LLC must view the OWTS during construction. The OWTS observation should be
performed before backfill, after placement of OWTS components. Septic tanks, distribution devices,
pumps, dosing siphons, and other plumbing, as applicable, must also be observed. ALL SERVICE septic,
LLC should be notified 48 hours in advance to observe the installation.
REVEGETATION REQUIREMENTS
An adequate layer of good quality topsoil capable of supporting revegetation shall be placed over the entire
disturbed area of the OWTS installation. A mixture of native grass seed that has good soil stabilizing
characteristics (but without taproots), provides a maximum transpiration rate, and competes well with
successional species. No trees or shrubs, or any vegetation requiring regular irritation shall be placed over
the STA. Until vegetation is reestablished, erosion and sediment control measures shall be implemented
and maintained on site. The owner of the OWTS shall be responsible for maintaining proper vegetation
cover.
OPERATION INFORMATION AND MAINTENANCE
The property owner shall be responsible for the operation and maintenance of each OWTS servicing the
property. The property owner is responsible for maintaining service contracts for manufactured units,
alternating STAs, and any other components needing maintenance.
Geo-fabrics or plastics should not be used over the STA. No heavy equipment, machinery, or materials
should be placed on backfilled STA. Livestock should not graze on the STA. Plumbing fixtures should be
checked to ensure that no additional water is being discharged to OWTS. For example, a running toilet or
leaky faucet can discharge hundreds of gallons of water a day and harm a STA.

If an effluent filter or screen has been installed in the OWTS, we recommend this filter or screen be cleaned
annually, or as needed. If the OWTS consists of a pressurized pump system, we recommend the laterals be
flushed annually, or as needed.
The homeowner should pump the septic tank every two years, or as needed gauged by measurement of
solids in the tank. Garbage disposal use should be minimized, and non-biodegradable materials should not
be placed into the OWTS. Grease should not be placed in household drains. Loading from a water softener
should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS.
Mechanical room drains should not discharge into the OWTS. The OWTS is engineered for domestic waste
only.
ADDITIONAL CONSTRUCTION NOTES
If design includes a pump, weep holes must be installed to allow pump lines to drain to minimize risk of
freezing. The pump shall have an audible and visual alarm notification in the event of excessively high-
water conditions and shall be connected to a control breaker separate from the high water alarm breaker
and from any other control system circuits. The pump system shall have a switch so the pump can be
manually operated.
Excavation equipment must not drive in excavation of the STA due to the potential to compact soil.
Extensions should be placed on all septic tank components to allow access to them from existing grade.
Backfill over the STA must be uniform and granular with no material greater than minus 3-inch.
LIMITS:
The design is based on information submitted. If soil conditions encountered are different from conditions
described in report, ALL SERVICE septic, LLC should be notified. All OWTS construction must be
according to the county regulations. Requirements not specified in this report must follow applicable
county regulations. The installer should have documented and demonstrated knowledge of the
requirements and regulations of the county in which they are working.

Onsite Wastewater Treatment System (OWTS) Inspection

As requested, CBO Inc. performed an Onsite W astewater Treatment System (OWTS) inspection at 4553
Frying Pan Road, Basalt, Colorado on May 3, 2019. The legal description of the 2.0-acre property is as
follows:

We were provided the attached records on the property from Eagle County Environmental Health
Department. Permit No. 239 was issued for a 4-bedroom duplex. The current residence has 4-
bedrooms and the proposed remodel will have no more than 4-bedrooms.
Other documents indicate issues with the site plan that was provided by the then-property owner. There
is conflicting information on the county documents as to whether the septic tank was 1000 gallons or
1250 gallons. The drawing provided by the county, with the electronic file name “as-built.pdf” notes a
1250-gallon septic tank. The septic permit never received final approval. B&R Septic pumped the septic
tank in June 2016. The invoice was for 1000-gallons; however, this may not be accurate. The difference
in 250 gallons is sometimes difficult to determine in the pump truck.
We were also provided documents for a system installed in 2001, but we believe this is a neighboring
property.
At the time of our inspection, both septic tank lids were accessible from grade. Given the age of the
structure, it is possible that the sewer line from the house to the septic tank could be cast iron. Cast iron
is prone to clogging and deterioration. We recommend this sewer line be explored to assess the
condition of the sewer line.
The subject OW TS consists of a two-compartment concrete septic tank (either 1000-gallons or 1250-
gallons) followed by a soil treatment area (STA). Documents note the STA was to provide a minimum
infiltrative area of 800 square feet and the file “as-built.pdf” notes a 24’ x 42’ STA. If the system was
installed in 1978, we assume the STA consists of pipe and gravel.
At the time of our inspection, both manhole lids were accessible from grade. Three feet of risers were
added to each side of the septic tank. Both inlet and outlet tees were present. The outlet tee is tucked
under the lid of the septic tank, making our typical recommendation of adding an effluent filter not
feasible. The outlet tee is cocked up slightly, indicating possible settling of the pipe outside of the septic
tank. Levels in the septic tank were normal at the time of our inspection.
We walked the area of the STA and no inspection ports were observed. We found no evidence of
surface saturation or failure at the time of our inspection.
This evaluation is not a guarantee of future system performance.

based upon what was observed and the Licensed Inspector’s expertise in onsite wastewater technology.
The Inspector has not been retained to warrant or guarantee the proper functioning of the system for any
period of time in the future. Because of numerous factors which may affect the operation of an OWTS,
as well as the inability of the Inspector to supervise or monitor the use or maintenance of this OWTS, this
report shall not be construed as a warranty by the Inspector that the system will function properly for any
particular prospective buyer, and the Inspector disclaims any warranty, either expressed or implied,
arising from the inspection of the OWTS or this report. The evaluation does not ascertain the impact the
system is having on groundwater.
