Dear Ivan:
At the request, of Ron Book on September 12, 2001, we visited f1je construction site on Lot 15,
Bertrocb Subdivision Filing No. 6, 216 Strohm Circle, Gypsum, Eagle County, Colorado. The
Purpose of our site visits was to inspect the installation of the septic system.
They installed the system. Iri overall cornpliance with the septic system desigil., Drawing
No, OIOISD.DWG, dated Januaty 10, 2001. The system was installed as shown. on the attached,
as -built sketch. Thev installed a 150Q-gallon, two compartments, a precast, concrete septic tank.
A cleanoutA),as installed between the house and the septic tank.
Forty-eight Standard Infiltrator Chambers were installed in four. trenches. The first trench
contained 13 infiltrators, the sc,=nd and third trenches each had 12, and the fourth had 11. The
infiltrators were conriected with serial distribution. In the first and last infiltrator of each trench,
inspection ports were installed.

The Proposed leachfield for the septic system is located in the easement of Holy Cross Energy,
and it is under the existing power lines. Also, thank you for the copy of the letter from Holy
Cross Energy, which allows the installation of the leachfield within their easemeiit.
As stated in the design dxawing, dated January 10, 2001, Item 8 from the notes, Traffic
over the system should be avoided. In. order to protect the leachfield from traffic of mainteilailce,
vehicles, we recommend that a fence be installed at its perimeter.

Dear Mr. Comad:
As requested by Pon Book with Book Construction, a representative of Hepworth-
pawlak Geote&aical, Inc. conducted percolation testing at the subject site to evaluate
the feasibility of an infiltration septic disposal system. The results of our work are
presented in this report. we previously conducted a subsoil study at the site and
presented our findings in a report dated October 17, 20M, Job No. 100 814.
A profile pit and three shallow backhoe pits were excavated at the locations shown on
Fig. 1. Percolation test holes were hand dug in the bottom of the backhoe pits. The test
holes were soaked, one day prior to testing. The holes were covered with rigid foam
insulation to protect against freezing overnight. The soils exposed in the percolation
holes are similar to those exposed in the profile pit and consist of about 1/2 foot of
topsoil and 4 feet of clayey sand and gravel with rock fragments up to boulder size
overlying weathered siltstone/claystane bedrock to the profile pit depth of 81/2 feet.
Groundwater was not observed is the profile pit and the soils were slightly moist to
moist.
Percolation. tests were performed, on December 13, 2000 by a represeatative of
Hepworth-Pawlak Geott--chuical, Inc. The percolation test results are presented on
Table 1. The ground temperature was about 34- at the time of testing. The percolation
test results indicate an infiltration rate bet-,,veen about 5 and 40 minutes per inch. Based

on the shallow bedrock encountered in the profile pit, the town may require that a civil
enineer design the septic disposal system,

Due to the closeness of the weathorred bedrock, each trench will be overexcavated a minimum of two feet.

NOTES:
1. Bends in the building sewer shall be limited to 45 degrees.
2. The septic tank sholl'be installed level. The tank shall have removable covers or manholes to within 8
inches of the finished -grade, for inspection and cleaning.
3. Excavato,-Puree (3) feet wide trenches. The horizontal distance from sidewall to sidewall between the
ttrerchshcll be a minimum of six (6) feet.
4. Place and compact, by wetting, two feet or' sand filter at the bottom of the trenches. The sand filter
'sftll be clean sand, consisting of 88 to 937 sand and 7 to 12% finer grained material with a design
infiltration rate of 1.2 gpd/sq. ft.
5. Assemble and install the Infiltrator Chambers in the trenches according to the manufacturers
recommendations.
6. Backfill the sidewall area with native on —site soil for proper support.
7. Backfill the trenches with a minimum of 12 inches of tamped soil cover.
8. Avoid vehicle traffic over the system.
9. All installations shall meet the rules and regulations of the Eagle County Environmental Health Division,
Chapter 4, Individual Sewage Disposal Systems.
10. The Septic System Installation shall be inspected by the Design Engineer prior to bockfilling.
