Permitted Construction / Details:
Install the OWTS exactly as designed by LKP Engineering, LLC stamped and dated October 3, 2013.
System consists of a 2,000 two compartment, top seam concrete septic tank and 1,688 square feet of
absorption area credit via 85 Infiltrator Quick 4 Plus chambers serially connected in five shallow trenches
of 17 chambers in each trench. Be sure to use the top entry and exit portals on the end plates and affix
the splash guards beneath the entery portals. Do not install in wet weather or in frozen ground.
The system is designed with the capacity to serve a six bedroom residence but will be primarily used by
the employees of a commercial business.
Contact the design engineer for all inspections prior to covering any component of the system and for
certification of proper installation of the new system along with proper abandonment of the existing tank.
The system cannot be used until final certification and as-built drawings are recieved by the design
engineer and Environmental Health provides final documentation to the owner.
The existing tank is to be abandoned in accordance with 4.17.07 Eagle County Land Use Regulations.

Be aware that changes in the use of your property or alterations of your building may
require commensurate changes to,or relocation of,your septic system.Landscape features,
trees with tap roots,irrigation systems and parking areas above the drain field can cause
premature system failure.It is equally important that you notice and immediately repair
dripping faucets and hissing toilets as this will certainly cause the system to fail.By
following a few simple guidelines,like avoiding grease or other additives down the drain and
making sure your septic tank is on a pumping schedule,your system has been designed to
last for many years.

Dear Mr. Daniel:
At your request, on November 1 and 6, 2013, we visited the construction site at 2558
Gypsum Creek Road, Eagle County, Colorado. The purpose of our site visits was to inspect the
installation of the septic system.
They installed the system in overall compliance with the septic system design, Drawing
No. 13-2889OWTS.DWG, dated October 3, 2013, and as shown on the attached as-built sketch.
They installed a new 2000-gallon, two compartments, concrete septic tank. There were two
existing cleanouts between the septic tank and the building, as shown on the attached as-built
sketch. The first cleanout was near the northwest building corner. The building sewer line from
the first cleanout was connected to the north side of the septic tank, into the side inlet. The
second cleanout was found under the deck on the west side of the house, about 15 feet south
from the first cleanout. The building sewer line at this cleanout was connected to the inlet at the
top (east side) of the septic tank.
Eighty-five, Q-4 Plus, Standard Infiltrator Chambers were installed in five trenches.
There were seventeen (17) infiltrators in each trench. There was a minimum of six feet separation
between the trenches. In the first and last infiltrator of each trench, inspection ports were
installed. The T’s in both compartments of the septic tank were at least 14-inches long. In the
inlet chamber two T-s were installed, one for each of the building sewer lines.
There is no other warranty either expressed or implied.
