The OWTS Permit authorizes the repair and partially replacement of the original system serving the home (ISDS 660) installed in 1984 which is presumed to be located under the driveway. The OWTS repair/replacement consists of inspecting the original tank for suitability and if it is in good shape as determined by the design engineer, will be used as second and third compartment storage by installing a one compartment Fralo ST-750 (now Roth RTM-750) ahead of the old tank with new piping. Risers must be added to the existing tank if used to bring access to or above grade. If the existing tank is not usable, the original tank must be abandoned in accordance with Regulation 43.9 (C) and a new 1,250 gallon tank installed. The new soil treatment area has 1,000 square feet of absorption area credit via the installation of 58 Infiltrator Quick 4 Plus chambers serially connected in shallow trenches as shown on the design drawings. The new OWTS is designed to serve a total of four bedrooms. Do not install in wet weather and contact the design engineer to conduct the inspections necessary to certify proper installation. Certification of proper installation along with as-built drawings, photos and documentation of existing tank abandonment (if abandoned) are required to be submitted to Eagle County Environmental Health and approved prior to using the new system. Horn 1st and Main Investment Advisors 97 Main Street, W103 Edwards, CO 81632 RE: Soil and Site Evaluation for a Septic System 449 Meadow Road, Minturn Eagle County, Colorado Project No. 14-2964 On July 24, 2014, we visited your property on 449 Meadow Road, Minturn. According to the topographical survey, the property is 2.135 acres of land. Access to the property from Main Street in Minturn, is via Nelson Avenue to Meadow Road, which is about half a mile. The purpose of our site visits was to select the most suitable site available on the lot for the proposed location of the new septic system. The residence is presently services by an existing septic system. We understand, from the information you provided during our site visit, and from information emailed by the ECEH Department, that the existing septic tank and the leachfield are under the parking area at the top of the driveway. ●Preliminary Investigation □Address: 449 Meadow Road, Minturn, Eagle County, Colorado □Legal Description: A Part of the SE1/4 SW1/4 Section 26, and NE1/4 NW1/4 Section 35, T5S, R81W, of the 6TH Principal Meridian, Eagle County Colorado, from a Partial Topographical Survey by Eagle Valley Surveying, Inc., dated August 27, 2014. □Existing Structures: On the property there is the main residence, couple of detached garage buildings and other ranch structures. □Location of existing and proposed wells on property: The well is on the east side of the main house and it is 100 feet away from the proposed leachfield. □ECEH Department records □Topography: The topography of the property varies from relatively level to moderate to steep. The area of the proposed new leachfield has a slope of 25 to 27%. □Soil Data: The soil encountered in the profile hole consisted of about one foot of topsoil underlain by four feet of light brown gravelly sand with cobbles and boulders over 2 feet of light brown, sandy gravel with cobbles and boulders. Ground water was not encountered. □Location of applicable setbacks in Table 7-1: The leachfield is at least 10 feet away from the west and southern property lines, and 100 feet from the P.O. Box 2837, Edwards, CO 81632 www.lkpengineering.com 449 Meadow Road, Minturn September 15, 2014 Project No. 14-2964 Page 2 of 3 well. It is also over 20 feet away from the residence. The septic tank is 95 feet away from the well, 18 feet away from the main residence, and about 26 feet away from the leachfield. There are no other bodies of water near the septic system. ●Reconnaissance □Landscape position: The proposed leachfield site is located on a south facing slope, south of the parking area and the driveway. □Topography: The topography of the proposed leachfield site is moderate □Vegetation: The vegetation of the proposed leachfield site consisted of wild grass and few trees. □Natural and cultural features: There are no streams or other bodies of water on the lot near the leachfield. □Current and historic land use: Based on the records available, the property is currently part of a residential subdivision. No prior record found. ●Detailed Soil Investigation □Visual and tactile evaluation of two or more soil profile test pit excavation; OR □Percolation tests plus one or more soil profile holes or one or more profile test pit excavations: One profile hole was excavated and three percolation holes were dug, presoaked and a perc test was run the following day. ●Report and Site Plan □Name, address, telephone number, email address, and credentials and qualification of individual who conducted site evaluation: This site evaluation was completed by Luiza Petrovska, PE, of LKP Engineering, Inc. LKP Engineering, Inc. Is located at 275 Main St., Suite C-104, Edwards, Colorado. The phone number is 970-926-9088 and Luiza’s email is luiza@lkpeng.com □Dates of preliminary and detailed evaluation: July 24, 2014 □A graphic soil log, to scale: Attached □Setback distances in Table 7-1: The leachfield is at least 10 feet away from the west and southern property lines, and 100 feet from the well. It is also over 20 feet away from the residence. The septic tank is 95 feet away from the well, 18 feet away from the main residence, and about 26 feet away from the leachfield. There are no other bodies of water near the septic system. P.O. No pond, creek, lake or other water feature. No dry gulch or cut bank, No Ground water or restrictive layer. □A drawing (to scale) with complete property boundary lines on minimum of 8.5 x 11" ■Label profile test pits/holes ■North Arrow ■Graphic scale ■Horizontal and vertical reference points of proposed STA ■Contours OR slope direction and % slope ■Location of any visible or known unsuitable, disturbed, or compacted soils ■The proposed elevation of the infiltrative surface of the STA from benchmark or ground surface will be two feet. •Anticipated construction related issues: The existing septic tank will be evaluated and new lockable risers constructed to bring it to the surface. No other issues anticipated. •Assessment of known or foreseeable land use changes expected to affect the system performance: None known. •Narrative explaining difficulties encountered during site evaluation: No difficulties encountered. Location of setbacks are all shown on the septic system design. The percolation test results and a graphic soil log are appended to this report. Sincerely, LKP Engineering, Inc.
