Install the replacement soil treatment area exactly as described in the All Service Septic, LLC. October 9,
2014 design report dated, stamped and signed by Richard Petz. The original 1,250 gallon septic tank
permitted under ISDS #638 is reported to be in good condition and can be used with the addition of an
effluent filter on the outlet tee and bringing the access risers to or above grade.
The replacement soil treatment area consists of 1,000 square feet of absorption area credit via the
installation of 4 rows of 15 Infiltrator Quick 4 chambers (for a total of 60 chambers) placed on sandy
suitable soils atop the Roaring Fork alluvial materials, kept as shallow as possible. Be sure to remove all
large cobbles from the bottom of the excavation and place the chambers over at least 6 inches of suitable
soil. Inspection portals are required to be installed at both ends of each trench. Effluent will be equally
distributed to each trench via a flow regulated distribution box with access to or above grade. A ball
valve will be installed in the effluent line connected to the old soil treatment area to allow future,
emergency or temporary use.
Contact the design engineer to conduct all inspections necessary to certify the proper installation of the
OWTS. Final certification of proper installation, including as-built drawings and photos are required to be
submitted to Eagle County Environmental Health and approved prior to the use of the new soil treatment
area. Do not install in wet weather and be sure to scarify the sidewalls of the clay toward the bottom of
each excavated trench.

Doug:
This letter is to inform you that the above-referenced septic permit has been inspected and
finalized. This permit was approved and sized for a 3 bedroom dwelling.Additional information
about the maintenance your septic system needs as well as information about private wells can be
accessed through web site links provided on the Environmental Health Department’s home page
at http://www.eaglecounty.us/envHealth/
Be aware that changes in the use of your property or alterations of your building may
require commensurate changes to,or relocation of,your septic system.Landscape features,
trees with tap roots, irrigation systems and parking areas above the drain field can cause
premature system failure. It is equally important that you notice and immediately repair
dripping faucets and hissing toilets as this will certainly cause the system to fail.By
following a few simple guidelines, like avoiding grease or other additives down the drain and
making sure your septic tank is on a pumping schedule, your system has been designed to
last for many years.
If you have any questions regarding this information, please contact us at (970)328-8755 and
reference the OWTS septic permit number.

Subsurface Investigation and Onsite Wastewater Treatment System Design
4-Bedroom Residence
450 Hooks Lane
Eagle County, Colorado
Doug,
ALL SERVICE septic, LLC performed a subsurface investigation and completed an onsite
wastewater treatment system (OWTS) design for the subject residence. The property is located
outside of Basalt, in an area where OW TSs and wells are necessary.
SITE CONDITIONS
A 4-bedroom, single-family residence presently exists and is utilizing an existing OWTS which is
experiencing saturated conditions, evident by the inspection ports in the existing soil treatment area
(STA). There is a private well located on the property located greater than 100-feet from the
proposed STA. In 2000, a French drain was installed up-gradient (south and east) of the residence,
alleviate the property of infiltration of groundwater from flood irrigation of surrounding agricultural
fields. Since that time, groundwater has not been an issue. The French drain is greater than 200-
feet from the proposed STA.
SUBSURFACE
The subsurface was investigated on June 30, 2014 by digging one 8-foot soil profile test pit excavation
(Test Pit) and taking a soil sample from approximately 3 to 4-feet below grade. Only one Test Pit was
excavated because instructions on soil analysis were given to the property owner prior to the effective
date of the current regulation. A gradation analysis was run by HP Geotech and results are detailed in
a report dated July 9, 2014, Job No 114 021M.
The materials encountered in the Test Pit consisted of dark brown, extremely firm, moist, blocky,
clayey, silty sand to 4.5-feet, underlain by medium brown, loose, moist, granular gravelly, silty sand
with 2 to 6-inch cobbles to a maximum depth explored of 8.0-feet. No bedrock or groundwater was
encountered.
The gradation and hydrometer analysis performed by HP Geotech shows the soil consists of 41%
gravel, 43% sand, 10% silt and 6% clay. The USDA soil type is classified as Very Gravelly Loamy
Sand. Removing the gravel from the equation, we can extrapolate the soil material to be
approximately 68% sand, 16% silt, and 10% clay. A long term acceptance rate (LTAR) of 0.6
gallons per square foot will be used to design the OWTS, in accordance with Table 10-1 Soil
Type 2 presented in the Eagle County On-Site Wastewater Treatment System Regulations,
adopted May 13, 2014.
Page 2

Backfill from 0 to 4.5-feet Backfill from depth below 4.5-feet
DESIGN SPECIFICATIONS
The OWTS design is based on 4-bedrooms. An average daily wastewater flow of 600 GPD will be
used. The STA will be sized on a sandy loam material with a granular structure (Soil Type 2),
gravity flow to gravelless chamber trenches.
The existing 1250-gallon, two-compartment septic tank will remain in use. The existing septic tank is
in good condition and burial is shallow with no risers necessary. Current code requires manholes be
accessible from grade. The outlet side of the septic tank may require installation of a riser to bring
access to grade. Inlet and outlet tees will be installed. An effluent filter will also be installed on the
outlet tee of the septic tank.
A ball valve will be installed on the existing sewer line out of the septic tank to the existing STA, as
well as on the new sewer line to the new STA. Access to the valves must be accessible from grade.
This will allow the currently saturated STA to rest, and if properly recovered, may be used in
emergency situations in the future. No reductions have been taken in sizing the new STA as this will
not be considered an alternating system.
A new distribution box will be installed, accessible from grade. The distribution box must be filled
with water at the time of the inspection to assure equal distribution to all trenches. Flow regulators
or levelers must be used to assure flow equalization. Four rows of 15 ‘Quick 4’ Infiltrator® chambers
Page 3
will be installed for a total of 60 chambers and 700 square feet of infiltrative area. Inspection ports
must be placed at the beginning and ends of each row. There must be at least 6-feet of undisturbed
soil between each trench.
The infiltrative area may not exceed 4-feet below native grade. If the extremely firm dark brown
horizon is encountered at a depth exceeding 4-feet, this material must be excavated and
removed. The trenches may NOT be installed in this material. If the depth of the over-excavated
trenches exceed 4-feet, either on-site material with a sandy loam texture (3-inch minus) OR
imported concrete sand must be replaced in the trenches. This material should compacted
in one-foot lifts with the backhoe bucket to establish a base for the chambers and the
infiltrative area.
The component manufacturers are typical of applications used by contractors and engineers in this
area. Alternatives may be considered or recommended by contacting our office. Construction must
be according to Eagle County On-Site Wastewater Treatment System Regulations, the OWTS Permit
provided by Eagle County Environmental Health Department, and this design.
REVEGETATION REQUIREMENTS
An adequate layer of good quality topsoil capable of supporting revegetation shall be placed over the
entire disturbed are of the OWTS installation. A mixture of native grass seed that has good soil
stabilizing characteristics (but without taproots), provides a maximum transpiration rate, and competes
well with successional species. No trees or shrubs, or any vegetation requiring regular irritation shall be
plated over the area. Until vegetation is reestablished, erosion and sediment control measures shall be
implemented and maintained on site. The owner of the OWTS shall be responsible for maintaining
proper vegetation cover.
OPERATION INFORMATION AND MAINTENANCE
The property owner shall be responsible for the operation and maintenance of each OWTS servicing
the property. The property owner is responsible for maintaining service contracts for manufactured
units, alternating STAs, and any other components needing maintenance.
Geo-fabrics or plastics should not be used over the STA. No heavy equipment, machinery, or materials
should be placed on backfilled STA. Livestock should not graze on the STA. Plumbing fixtures should
be checked to ensure that no additional water is being discharged to OWTS. For example, a running
toilet or leaky faucet can discharge hundreds of gallons of water a day and harm a STA.
The homeowner should pump the septic tank every two years, or as needed gauged by measurement
of solids in the tank. Garbage disposal use should be minimized, and non-biodegradable materials
should not be placed into the OWTS. Grease should not be placed in household drains. Loading from
a water softener should not be discharged into the OWTS. No hazardous wastes should be directed
into the OWTS. Mechanical room drains should not discharge into the OWTS. The OWTS is
engineered for domestic waste only.
Page 4
ADDITIONAL CONSTRUCTION NOTES
If design includes a pump, air release valves and weep holes must be installed to allow pump lines
to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of the STA
due to the potential to compact soil. Extensions should be placed on all septic tank components to
allow access to them from existing grade. Backfill over the STA must be uniform and granular with
no material greater than minus 3-inch.
INSTALLATION OBSERVATIONS
ALL SERVICE septic, LLC must view the OWTS during construction. The OWTS observation should
be performed before backfill, after placement of OWTS components. Septic tanks, distribution
devices, pumps, dosing siphons, and other plumbing, as applicable, must also be observed. ALL
SERVICE septic, LLC should be notified 48 hours in advance to observe the installation.
LIMITS:
The design is based on information submitted. If soil conditions encountered are different from
conditions described in report, ALL SERVICE septic, LLC should be notified. All OW TS construction
must be according to the county regulations. Requirements not specified in this report must follow
applicable county regulations. The contractor should have documented and demonstrated
knowledge of the requirements and regulations of the county in which they are working. Licensing of
Systems Contractors may be required by county regulation.
Please call with questions.

Onsite Wastewater Treatment System (OWTS) Installation Observations
450 Hooks Lane
Eagle County, Colorado
Permit No. OWTS-10-14-11609
Doug,
ALL SERVICE septic, LLC observed the installation of the onsite wastewater treatment system (OWTS)
on October 22, 2014 for the subject property.
The existing 1250-gallon, two-compartment septic tank remains in use. An effluent filter was
installed on the outlet tee of the septic tank. This filter must be cleaned annually, or as needed.
A ball valve was installed on the existing sewer line out of the septic tank to the existing STA, as well
as on the new sewer line to the new STA. The valves are accessible from grade. This allows the
currently saturated existing STA to rest, and if properly recovered, may be used in emergency
situations in the future.
A new distribution box was installed and is accessible from grade. The distribution box was filled
with water at the time of the inspection to assure equal distribution t o all trenches. Four rows of 15
‘Quick 4’ Infiltrator® chambers were installed for a total of 60 chambers and 700 square feet of
infiltrative area. Inspection ports were placed at the beginning and ends of each row. At least 6-feet
of undisturbed soil was present between each trench.
For assurance that the installation of the trenches achieved a depth below the clay layer, water was run
through the distribution box for over an hour with a garden hose and no ponding occurred at the
beginning of the trenches.
No irrigation ditches may run within 50-feet of the soil treatment area.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are
made to the OW TS in the future. Any additional OWTS construction must be according to the county
regulations.
LIMITS:
Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent of the expertise of the installer, soil
type, and weather conditions.
Please call with questions.
