of the following Resolution:
BOARD OF COUNTY COMMISSIONERS
COUNTY OF EAGLE, STATE OF COLORADO
RESOLUTION N0.2003_~T
RESOLUTION AUTHORIZING THE DIRECTOR AND THE APPOINTED ACTING
DIRECTOR OF THE EAGLE COUNTY
DEPARTMENT OF HEALTH AND HUMAN SERVICES TO
ENTER INTO AND EXECUTE
DESIGNATED CHILD PLACEMENT AGREEMENTS
WHEREAS, the Eagle County Department of Health and Human Services ("ECDHHS")
does from time to time desire to enter into and execute the following designated child placement
agreements: General Provider Agreement, Individual Authorization for Care, and
Voluntary Placement Contract; and
WHEREAS, in the interests of providing timely child placement and maintaining client
confidentiality, the Director and appointed Acting Director of the ECDHSS seek the authority
from the Board of County Commissioners to enter into and execute these agreements; and
WHEREAS, the Board is desirous of designating the Director and the appointed Acting
Director of the ECDHSS as its authorized representatives to enter into and execute said
agreements:
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF THE COUNTY OF EAGLE, STATE OF COLORADO:
THAT, the Board hereby designates the Director and appointed Acting Director of the
Eagle County Department of Health and Human Services as its duly appointed representatives
and agents to execute said agreements in furtherance of timely child placement and client
confidentiality.
THAT, the Board of County Commissioners finds, determines and declares that this
Resolution is necessary for the public health, safety and welfare of the citizens of the County of
Eagle, State of Colorado.
