One-time funds are available to expand the support of rapid screening for syphilis, HCV, and HIV. This support is made available by offering free rapid test kits to LPHAs and other covered entities via CDPHE’s Rapid Testing Distribution Program. Project Term: 1/1/2025-12/31/2025 Agreement: Under the terms of this agreement, CDPHE will provide access to no-cost rapid HIV/HCV/Syphilis tests to partnering entities. Partnering entities are prohibited from Docusign Envelope ID: 5FC37BC4-F394-4D6F-8488-82724A534669 charging an individual for the cost of the test or billing insurance for any rapid tests distributed through this program. Process and Protocol This protocol is intended to explain the process of ordering HIV/HCV/Syphilis Rapid Tests purchased by CDPHE for qualifying entities in the State of Colorado. Funding is supported by the CDC’s Strengthening STD Prevention and Control for Health Departments (PCHD) and State Opioid Response (SOR). As well as state-funds for syphilis prevention and control. All HIV testing conducted under this protocol will be considered “supported testing events” and follow appropriate CDC reporting requirements. 1. Agencies intending to receive rapid tests purchased by CDPHE must submit a distribution plan prior to placing each order through the CDPHE: Rapid Test Order Form 2. Agencies must submit both a signed MOU and CLIA waiver with their rapid test order form. a. Plans are reviewed within 5 business days of submission. CDPHE will notify the agency if their distribution plan was approved. 3. All orders will be placed by CDPHE and shipped directly to the agency. Expectations Standards and Requirements 1. The Agency shall perform STI screening in accordance with current CDC screening guidelines: https://www.cdc.gov/std/treatment-guidelines/STI-Guidelines-2021.pdf 2. The Agency shall maintain one (1) of the following for STI testing throughout the life of the project: a. Valid, non-expired CLIA waiver certificate b. Valid, non-expired CLIA certificate 3. The Agency shall comply with reporting requirements. The CDPHE Communicable Reportable Conditions Guidelines: https://www.colorado.gov/pacific/cdphe/report-a disease 4. The Agency shall complete the STI testing and treatment locator survey. The agency shall complete this aggregate Rapid Test Monthly Reporting Form within 15 days of the following month. Data reporting for this MOU is not meant to replace any contracted or other data reporting requirements by other projects funded by CDPHE. Agencies receiving PCHD, High Impact HIV Prevention (HIHP), and Colorado HIV Alliance for Prevention, Care, and Treatment (CHAPP), will be expected to input client Docusign Envelope ID: 5FC37BC4-F394-4D6F-8488-82724A534669 level data into the STI/HIV/VH Prevention REDCap project: https://cdphe.redcap.state.co.us/surveys/?s=TECJAF7RKRKM9AA9. Contingency of change or loss of funding The partnering entity acknowledges that funding for this project is based on one-time funds that are available to the Colorado Department of Public Health and Environment and there is no guarantee of continued or additional funding beyond the specified terms. In the event that any of these sources of funding are reduced or eliminated prior to the end of the term, CDPHE has the right to reduce, suspend or eliminate financial support for this project. CDPHE shall endeavor to give the partnering entity as much prior notice as is possible of any such situation.
