Installation of the OWTS exactly as depicted in the LKP Engineering, Inc, design dated, signed and
stamped May 29, 2015. System is designed to serve a 5 bedroom residence.
System consists of a 1,500 gallon, two compartment, Front Range Precast septic tank and 1,250 square
feet of soil treatment area credit (prior to adjustments for using chambers) via 73 Infiltrator Quick 4
chambers installed in 4 shallow trenches as shown on the design drawing.
Be sure to follow the engineer's instructions for trench bottom mitigation should basalt rocks be
encountered during construction.
Contact the design engineer to conduct all inspections necessary to certify the system and to prepare the
as-built drawings. System certification along with the as-built drawing and photos are required to be
submitted to, and approved by, Eagle County Environmental Health prior to the use of the system or
occupancy of the dwelling.

This letter is to inform you that the above-referenced septic permit has been inspected and
finalized. This permit was approved and sized for a total capacity for 5 bedrooms. Additional
information about the maintenance your septic system needs can be accessed through web site
links provided on the Environmental Health Department’s home page at

Be aware that changes in the use of your property or alterations of your building may
require commensurate changes to, or relocation of, your septic system. Landscape features,
trees with tap roots, irrigation systems and parking areas above the drain field can cause
premature system failure. It is equally important that you notice and immediately repair
dripping faucets and hissing toilets as this will certainly cause the system to fail. By
following a few simple guidelines, like avoiding grease or other additives down the drain and
making sure your septic tank is on a pumping schedule, your system has been designed to
last for many years.

Dear Henry:
At the request of Chris Spiegel, on October 1 and 5, 2015 , we visited the construction
site on Lot 13, Block 1, Cordillera Filing No. 36, 343 Pine Marten Way, Eagle County, Colorado.
The purpose of our site visits was to inspect the installation of the septic system.
They installed the system in overall compliance with the septic system design, Drawing
No. 13-2908owts.dwg, last revised May 29, 2015, and as shown on the attached as-built sketch.
At the time of the inspection we observed the following:
They installed a 1500-gallons, two compartments, concrete septic tank. One cleanout was
constructed between the septic tank and the building. The effluent line from the tank to the
leachfield was about 43.5 feet, 4-inch diameter, sdr35 pipe.
Seventy-three, Q-4 Plus, Standard Infiltrator Chambers were installed in four trenches.
There were nineteen (19) infiltrators in each of the first three trenches, and sixteen (16) chambers
in the fourth trench. There was a minimum of six feet separation between the trenches. In the first
and last infiltrator of each trench, inspection ports were installed. The infiltrators were connected
with serial distribution. There is no other warranty either expressed or implied.

RE: Soil and Site Evaluation

On November 18 and 19, 2013, we visited Lot 13, Block 1, Cordillera Filing No. 36
343 Pine Marten Way, Eagle County, Colorado. Lot 13 is a 2.297 acre parcel of land located on
the south side of Pine Marten Way in Cordillera at the Summit, Eagle County, Colorado. The lot
is about 1800 feet northwest from the intersection of Pine Marten Way and The Summit Trail.
The purpose of our site visits was to select the most suitable site available on the lot for the
proposed septic system, down gradient from the proposed building site.
!Preliminary Investigation
G Address: 343 Pine Marten Way, Eagle County, Colorado
G Legal Description: Lot 13, Block 1, Cordillera Subdivision, Filing No. 36
G Existing Structures: There were no existing structures on the property at
the time of our site visit
G Location of existing and proposed wells on property: There were no wells
on the property and none are proposed.
G ECEHD records
G Topography: The topography of the property is moderate with an
average slope of about 8 percent. Drainage is to the west.
G Soil Data: The soil encountered in the profile hole consisted of two feet
of topsoil underlain by 4.5 feet of grayish-brown matrix of slightly clayey
sand with basalt rocks of cobble size. Ground water was not encountered
at the time of our site visit.
G Location of applicable setbacks in Table 7-1
!Reconnaissance
G Landscape position: The proposed leachfield site will be located in a
back yard, northwest of the proposed residence.
G Topography: The topography of the proposed leachfield site is moderate
and the trenches will run along the contours.
G Vegetation: The vegetation of the proposed leachfield site consisted of
sage brush.

G Natural and cultural features: On the south side of the lot there is a golf
course. There are no natural features.
G Current and historic land use: Cordillera is an unincorporated
community. Current use is rural residential. Based on available records,
the property has been used as a ranch for grazing cattle.
!Detailed Soil Investigation
G Visual and tactile evaluation of two or more soil profile test pit
excavation; OR
G Percolation tests plus one or more soil profile holes or one or more profile
test pit excavations: One profile hole was excavated and three
percolation holes were dug, presoaked and a perc test was run the
following day.

rocks. If that happens, remove the rocks and fill the void area with sand or
bypass them with a 6-foot length of a 4-inch sch40 PVC and start the
trench again.
G Assessment of known or foreseeable land use changes expected to affect
the system performance: None known.
G Narrative explaining difficulties encountered during site evaluation:
Several areas were investigated due to shallow basalt rocks.
Location of setbacks, profile hole and percolation holes are shown on the septic
system design. The percolation test results and a graphic soil log are appended to this report.
