Permitted Construction / Details:
Removal and disposal of the existing 1,000 gallon plastic septic tank in accordance with WQCC
Regulation 43.9 (C.)(1.) and the installation of a new 1,250 gallon, two compartment, precast concrete tank
in the exact same location as the old tank as described in the LKP letter signed, stamped and dated June
17, 2015.
The manufacturer and septic tank model to be used must appear on the list of approved septic tanks
which is available on the Water Quality Control Division web site. Risers on the tank must be brought to
or above grade.
Contact the design engineer to certify that the tank replacement and glued connections were done in
accordance with Eagle County Public Health Agency On-site Wastewater Treatment System Regulations
adopted May 13, 2014. Engineer certification including an as-built drawing showing the new tank location
documenting the tank make and model along with photos are required to be submitted to and approved
by Eagle County Environmental Health prior to the use of the new tank. Evidence of proper tank disposal
is also required.
Previous permits serving Unit A include the original 1977 permit ISDS #157 and repair permit ISDS
1861-99 to replace the original failed drain field.

This letter is to inform you that the above-referenced septic tank replacement permit has been
inspected and finalized. This permit was approved and sized for a 3 bedroom dwelling.
Additional information about the maintenance your septic system needs as well as information
about private wells can be accessed through web site links provided on the Environmental Health
Department’s home page at http://www.eaglecounty.us/envHealth/
Be aware that changes in the use of your property or alterations of your building may
require commensurate changes to, or relocation of, your septic system. Landscape features,
trees with tap roots, irrigation systems and parking areas above the drain field can cause
premature system failure. It is equally important that you notice and immediately repair
dripping faucets and hissing toilets as this will certainly cause the system to fail. By
following a few simple guidelines, like avoiding grease or other additives down the drain and
making sure your septic tank is on a pumping schedule, your system has been designed to
last for many years.

Dear Alisa:
On July 31 and August 3, 2015, we visited the construction site at your property, Lot 34,
Block 6, Lake Creek Meadows, 284 Eagle Crest Road, Unit A, Eagle County, Colorado The
purpose of our site visits was to inspect the installation of the new septic tank.
Dave Bronn, installed the new septic tank in overall compliance with the written
recommendations in a letter dated, June 15, 2015, as shown on the attached as-built sketch. They
removed the old septic tank and disposed off it at the Eagle County Dump (per Dave Bronn). In
its place they installed a 1250-gallon, two compartment, concrete septic tank by Front Range
Precast. The tank had risers on both sides and twist on covers with locks. The covers were at
grade. A new cleanout was also constructed on the existing building sewer line. The cleanout was
about 6 feet east of the center of the east septic tank cover and was enclosed in a black, plastic
box.
We recommended that they also put a rock or a curb barrier to protect the east cover of
the septic tank, which was about a foot away from the edge of pavement and about 6 inches
above.
There is no other warranty either expressed or implied.

RE: Consultation on Septic Tank Repair

Dear Alisa:
At your request, today, June 15, 2015, we met on site at your property Lot 34, Block 6,
Lake Creek Meadows, 284 Eagle Crest Road, Unit A, Eagle County, Colorado. The purpose of
our site visit was to find the components of the existing septic system.
We understand from our conversation, emails, the documents received from Terri
Vroman with the Eagle County Environmental Health department, and the LKP Engineering files
(attached) that the existing system was built for a 3-bedroom residence. The existing septic tank
is 1000 gallon, fiberglass tank, it has broken partition, and insufficient covers.
Based on one of the old sketches, it appears that the building sewer line (and the cleanout
that Tim found near the southwest corner of the garage for unit A are connected to the existing
septic tank. The existing leachfield was constructed over an area of the front yard where the
existing soil was excavated out and replaced with imported fill.
We recommend that the old septic tank be removed and disposed off according to the
ECEH Department regulations, OWTS #43.9C.1 - Abandonment of Tank. Once the old tank is
removed the site will be prepared by your licensed septic system installer for the installation of
the proposed new septic tank. The new septic tank will be 1250-gallons, two compartments,
precast, concrete septic tank. Your installer will need to pull a repair permit and will coordinate
the abandonment of the old tank, the installation, connection and backfilling of the new tank, and
will make sure that all the required setbacks are met. Once the new septic tank is installed with
all the connections completed, you or your installer will contact LKP Engineering and the ECEH
Department to schedule and inspection. DO NOT backfill any part of the system prior to the
inspection. If you have any questions, please do not hesitate to call.
