Installation of the OWTS exactly as depicted on the LKP Engineering, Inc. design drawings stamped,
signed and dated by Luiza Petrovska on March 16, 2015. System is designed to serve a five bedroom
residence.
The OWTS consists of a 2,000 gallon Front Range Precast, two compartment septic tank; and 1,250
square feet of soil treatment area (prior to applying the adjustment factor for using chambers) via the
installation of 73 Infiltrator Quick 4 Plus Standard chambers arranged in 3 serially connected, shallow
trenches as shown on the design drawing.
Contact the design engineer well in advance to conduct all inspections necessary to certify the
installation and to prepare as-built drawings. System certification along with the as-built drawings and
photos must be submitted to, and approved by, Eagle County Environmental Health prior to the use of the
system or occupancy of the dwelling.

The OWTS consists of a 2,000 gallon Valley Precast, two compartment septic tank followed by a500gallon Front Range Precast (or equivalent approved by the designed engineer )electric lift stationequipped with an Orenco PF5005 High Head Effluent Pump; and 4,089 square feet of basal area with a750square foot gravel bed on top as shown on the design drawing.Be sure to contact Eagle County BuildingDepartment for necessary electrical inspections associated with the pump and control panel.Thecontrols must be in accordance with Eagle County On-site Wastewater Treatment System Regulations43.9(H.)(7.).
An effluent screen must be placed in the outlet tee of the 2,000 gallon septic tank or the pump must beplaced within a biotube filter assembly.For this determination contact the design engineer .

On October 29 and 30, 2014, and March 11 and 12, 2015, we visited Lot 2, Block 1,
Cordillera Filing No. 34, 1282 Gore Trail, Eagle County, Colorado. The lot is 5.65 acres and it is
located in the Cordillera subdivision at the Summit. From U.S. Highway 6 in Edwards, south, to
Fenno Drive at the entrance to the Cordillera Subdivision at the ranch, is about 3 miles. From
there to the gate is another mile, and from the gate to the lot is about 4.8 miles. The purpose of
our site visits was to evaluate the area to the southwest of the proposed residence for the
proposed location of the septic system.
●Preliminary Investigation
□Address: 1282 Gore Trail, Eagle County, Colorado
□Legal Description: Lot 2, Block 1, Cordillera Filing No. 34
□Existing Structures: There are no structures on the property
□Location of existing and proposed wells on property: There are no wells
on the property.
□ECEHD records
□Topography: The topography of the property varies. The proposed
building site is located on an east-west ridge. From the ridge the
topography is moderate and is sloping to the northwest, towards the
street, and to the south and southwest towards the back of the lot. The
proposed leachfield site is located south of the proposed building site.
□Soil Data: Two profile holes were excavated. The soil encountered in the
first profile hole consisted of two feet of topsoil underlain by four feet of
reddish-brown, volcanic, basalt rocks in a clayey matrix. Ground water
was not encountered.
□Location of applicable setbacks in Table 7-1
●Reconnaissance
□Landscape position: The proposed leachfield site is located in a proposed
back yard, to the south of the proposed residence.
□Topography: The topography of the proposed leachfield site is moderate.
□Vegetation: The vegetation of the proposed leachfield site consisted of
sage brush and grass. The surrounding vegetation also consisted of sage
brush and grass.

□Natural and cultural features: There are no natural and cultural features
on the property.
□Current and historic land use: Presently the property is used as a
residential subdivision. Based on available records, previously it was used
for livestock grazing.
●Detailed Soil Investigation
□Visual and tactile evaluation of two or more soil profiles in a test pit
excavation; OR
□Percolation tests plus one or more soil profile holes or one or more profile
test pit excavations: Two profile holes was excavated and three
percolation holes were dug, presoaked and a perc test was run the
following day.

□Narrative explaining difficulties encountered during site evaluation:
Basaltic rocks were encountered in the profile hole and since the
equipment used for digging the profile hole was a mini excavator, it was
difficult to dig out the rocks. The second profile hole was excavated with a
backhoe and it didn’t have a problem digging to 8 feet.
Location of setbacks are all shown on the septic system design. The percolation
test results and a graphic soil log are appended to this report.
