Permitted Construction / Details:
Connect the plumbing from the half bathroom located in the shed via the installation of a new sewer line
as detailed in the HP Geotech design report dated, stamped and signed May21, 2015. The new sewer line
will connect to the existing sewer originating from the residence with a downstream wye connection just
ahead of the 2,000 gallon septic tank and must also be encased as detailed in the design report. This
OWTS Permit is an alteration to the septic system original permitted under ISDS Permit #2276-03.
Contact the design engineer to conduct all inspections necessary to certify the system and to prepare the
as-built drawings. System certification along with the as-built drawings and photos are required to be
submitted to and approved by Eagle County Environmental Health prior to the use of the half bathroom or
occupancy of the shed.

Subject: As-Built Evaluation of On site Wastewater Treatment System Connection,
Existing Residence, Lot 14, Diamond Star Ranch, 1120 Diamond Star Road,

Dear Mr. Strickland:
Hepworth-Pawlak Geotechnical, Inc. has observed the repair of the Onsite Wastewater
Treatment System (OWTS) connection from the proposed shed to the existing OWTS located
at 1120 Diamond Star Road in Eagle County, Colorado. We previously prepared an As-Built
Evaluation of the OWTS in our report dated October 1,2015. The October 1st report showed
a Femco fitting that was used to connect the sewer line from the residence to the new sewer
line from the shed structure. The fernco fitting is considered unsuitable for the connection.
The Fernco fitting was removed and replaced with an expandable glued repair fitting as
shown in the photograph below.

Mr. Finchem:
This letter is to inform you that the above-referenced septic permit has been inspected and
finalized. This permit was approved for the addition of a pool house/workout area to be
connected to the existing OWTS system. Additional information about the maintenance your
septic system needs as well as information about private wells can be accessed through web site
links provided on the Environmental Health Department’s home page at
http://www.eaglecounty.us/envHealth/
Be aware that changes in the use of your property or alterations of your building may
require commensurate changes to, or relocation of, your septic system. Landscape features,
trees with tap roots, irrigation systems and parking areas above the drain field can cause
premature system failure. It is equally important that you notice and immediately repair
dripping faucets and hissing toilets as this will certainly cause the system to fail. By
following a few simple guidelines, like avoiding grease or other additives down the drain and
making sure your septic tank is on a pumping schedule, your system has been designed to
last for many years.
