the Upper Eagle Regional Water Authority
Pursuant to section 8(a) of the Authority Agreement Amending and Restating the Agreement Establishing the Upper Eagle Regional Water Authority and the Master Services Contract (“Authority Agreement.), the governing body of each Contracting Party to the Authority Agreement shall appointment one principal member to the Board of Directors of the Upper Eagle Regional
Water Authority (“Authority Board”) and one or more alternate members to act in the absence of the principal member.
This certificate should be completed by an officer or representative of the governing body who is authorized to certify the Contracting Party’s appointment to the Authority Board.

I, the undersigned officer/representative of the abovementioned Contracting Party, certifies that the following principal member and alternate member(s) are hereby appointed to serve on the Authority Board, effective immediately and until such time as the Contracting Party revokes the appointment
by submitting a new Certificate of Appointment to the Recording Secretary of the Authority Board.
