RECITALS:
Whereas the Nurse-Family Partnership®Program (the "Program") is an evidence-based
program developed on the basis of randomized controlled trial research to yield certain benefits
for low-income, first-time mothers and their children (“Program Benefits”).
Whereas Invest in Kids (“IIK”) provides certain services, largely funded by the Colorado

Network Partners. IIK also provides services to the Network Partners on NFP's behalf to ensure
Colorado Network Partners implement the Program with fidelity to the Nurse-Family Partnership
Model Elements.
Whereas IIK funding is dependent upon it receiving NFP reports containing outcomes and other
data obtained from Network Partners, and using the reports to provide data to CDEC.
Accordingly, the Parties are entering into this Agreement to memorialize the consent from the
Network Partner to release to IIK data reported to NFP by Network Partner.
