GRANT AGREEMENT BETWEEN
EAGLE COUNTY, COLORADO
AND
MOUNTAIN PRIDE
THIS GRANT AGREEMENT (“Agreement”) is effective as of July 1, 2024 by and between
Mountain Pride (hereinafter “Grant Recipient”) and Eagle County, Colorado, a body corporate and
politic (hereinafter “Grantor” or “County”).
RECITALS
WHEREAS, Eagle County works to promote the health, safety, and welfare of Eagle County
citizens of all ages through its Department of Public Health and Environment; and
WHEREAS, the County uses outside providers and professionals to enhance the ability of County
to promote such health, safety and welfare; and
WHEREAS, Eagle County Public Health & Environment (“ECPHE”) has identified a significant
community need for funding and services dedicated to the ‘promotion’ phase of care, which
addresses root causes of local health disparities, which lead to a myriad of individual high-risk
behaviors; and
WHEREAS, the observed and reported fragmentation of community organizations and non-
profits leads to a lack of strategy in provision of services and programming across systems of care;
and
WHEREAS, Eagle County Public Health & Environment would like to extend staff knowledge
and experience to offer organizational support while administering funds with the intent of infusing
equity and inclusion from a systemic level; and
WHEREAS, the parties acknowledge and agree that the programs and services detailed herein
promote the health, safety and welfare of the people of Eagle County and are intended to serve a
valid public purpose and provide services that could otherwise be performed by County directly;
and
WHEREAS, Grant Recipient has elected to perform services that help create and sustain a healthy,
environmentally and economically vital and socially diverse community in Eagle County on its
own and County has, therefore, elected to compensate Grant Recipient to perform the Services for
the public to fulfill an important public purpose; and
WHEREAS, Grant Recipient is authorized to do business in the State of Colorado and has the
time, skill, expertise, and experience necessary to provide the services as defined below in
paragraph A hereof; and
WHEREAS, this Grant Agreement shall govern the relationship between Grant Recipient and
County in connection with the services.

GRANT AGREEMENT
NOW, THEREFORE, in consideration of the foregoing premises and the following promises,
Eagle County and Grant Recipient enter into this Agreement for payment of the grant award in the
amount of seventy one thousand dollars ($71,000, the “Grant Award”) to Grant Recipient.
A. Grant Recipient agrees that funds awarded by Grantor may be used and expended solely
for services and programs related to building equitable systems for LGBTQ+ empowerment by
focusing on projects that build community, offer opportunities for education, advocacy, and
resource connection. as specified in its 2024 Advancing Systems Change Grant Application
attached hereto as Exhibit A, and incorporated herein by reference, and Grant Application Budget
Section. All funds will be expended in the one-year grant period, with the grant period terminating
exactly one-year from contract execution (the “Grant Period”). The Grant Award set forth herein
is for the specific Grant Period and the Grantor makes no representations, nor should Grant
Recipient rely on this award as a representation of any future awards for other grant periods.
B. In return for the Grant Award, Grant Recipient agrees to the terms of funding, deliverables,
and deadlines outlined in the Scope of Work attached hereto as Exhibit B, and incorporated herein
by reference. Grant Recipient shall submit a report discussing how the funds were expended,
deliverables as outlined in the Grant Application, and how they tie back to creating and sustaining
a healthy environmentally and economically vital and socially diverse community. The report will
be due no later than July 1, 2025.
C. Payment of the Grant Award will be made in one installment.
D. Grant funds under this Grant Agreement are to be used and expended within the Grant
Period. Any funds not used or expended during the Grant Period must be returned to Grantor.
E. If, after payment of the Grant Award, Eagle County reasonably determines that payment
was improper because the purposes for which the payment was made were misrepresented, or the
terms of this Agreement were breached, then upon written notice of such determination and request
for reimbursement from Eagle County, Grant Recipient shall forthwith return such payment to
Eagle County.
F. Grant Recipient shall comply at all times and in all respects with all applicable federal,
state and local laws, resolutions and codes; and specifically, with the requirements of the Civil
Rights Act of 1964 and Section 504, Rehabilitation Act of 1973, concerning discrimination on the
basis of race, color, sex, age, religion, political beliefs, national origin or handicap.
G. Grant Recipient shall not assign any of its rights or duties under this Grant Agreement to a
third party without the prior written consent of the Grantor. The Grantor shall terminate this Grant
Agreement in the event of any assignment without its prior written consent, in which case, any
unused Grant Funds must be returned to Grantor.

H. Grant Recipient agrees that no agent, employee or volunteer of Grant Recipient shall be
deemed an agent, employee or volunteer of County.
I. Grant Recipient shall maintain, for a minimum of three (3) years, adequate financial and
programmatic records for reporting to Grantor on performance of its responsibilities hereunder.
Grant Recipient shall be subject to financial audit by federal, state or county auditors or their
designees. Grant Recipient authorizes Grantor to perform audits or to make inspections during
normal business hours, upon 48 hour notice to Grant Recipient for the purpose of evaluating
performance under this Grant Agreement. Grant Recipient shall cooperate fully with authorized
Eagle County representatives in the observation and evaluation of the program and records. Grant
Recipient shall have the right to dispute any claims of misuse of funds and seek an amicable
resolution with Grantor.
J. This Agreement shall be governed by the laws of the State of Colorado. Jurisdiction and
venue for any suit, right or cause of action arising under, or in connection with this Agreement
shall be exclusive in Eagle County, Colorado.
K. This Grant Agreement supersedes all previous communications, negotiations, and/or
agreements between the respective parties hereto, either verbal, or written, and the same not
expressly contained herein are hereby withdrawn and annulled.
L. This Grant Agreement does not, and shall not be deemed or construed to confer upon or
grant to any third party or parties any right to claim damages or to bring any suit, action or other
proceeding against either Grant Recipient or Grantor because of any breach hereof or because of
any of the terms, covenants, agreements and conditions contained herein.
M. Invalidity or unenforceability of any provision of this Grant Agreement shall not affect the
other provisions hereof, and this Grant Agreement shall be construed as if such invalid or
unenforceable provision was omitted.
N. To the extent required to perform under this Grant Agreement, Grant Recipient represents
and warrants that it has the expertise and personnel necessary to properly perform the Grant
Agreement and its professional personnel are duly licensed to perform.
O. No failure or delay by either party in the exercise of any right hereunder shall constitute a
waiver thereof. No waiver of any breach shall be deemed a waiver of any preceding or
succeeding breach.
P. The signatories to this Grant Agreement aver to their knowledge, no employee of the
County has any personal or beneficial interest whatsoever in the matters described in this Grant
Agreement. The Grant Recipient has no beneficial interest, direct or indirect, that would conflict
in any manner or degree with the performance of the Grant Agreement and Grant Recipient shall
not employ any person having such known interests.
Q. Grant Recipient shall be responsible for the completeness and accuracy of its
performance under the Grant Agreement, including all supporting data or other documents

prepared or compiled in performance of the Grant Agreement, and shall correct, at its sole
expense, all significant errors and omissions therein. The fact that the Grantor has accepted or
approved the work performed shall not relieve Grant Recipient of any of its responsibilities.
Grant Recipient shall perform the Grant Agreement in a skillful, professional and competent
manner and in accordance with the standard of care, skill and diligence applicable to Grant
Recipients performing similar services. This paragraph shall survive termination of this Grant
Agreement.
S. Grant Recipient shall carry workers compensation insurance as required by law and
automobile and commercial general liability insurance as necessary and appropriate during the
term of the Grant Agreement.
T. The Grant Recipient shall indemnify and hold harmless Grantor, and any of its officers,
agents and employees against any losses, claims, damages or liabilities for which Grantor may
become subject to insofar as an such losses, claims, damages or liabilities arise out of, directly or
indirectly, this Grant Agreement, or are based upon any performance or nonperformance by
Grant Recipient or any of its sub-recipients hereunder including claims for bodily injury or
personal injury including death, or loss or damage to tangible or intangible property; and Grant
Recipient shall reimburse Grantor for reasonable attorney fees and costs, legal and other
expenses incurred by Grantor in connection with investigating or defending any such loss, claim,
damage, liability or action. This indemnification shall not apply to claims by third parties
against the Grantor to the extent that Grantor is liable to such third party for such claims without
regard to the involvement of the Grant Recipient. This paragraph shall survive expiration or
termination hereof.
U. Grant Recipient certifies that it has read the Grant Agreement, understands each and every
term and the requirements set forth herein, and agrees to comply with the same.

Advancing Systems Change Grant: Program
Application 2024
Eagle County Public Health & Environment (ECPHE) is thrilled to announce the 2024 Advancing Systems
Change Grant opportunity. Involvement will be highly interactive and adaptable to the grantee's needs,
goals, and limitations, with the intent of fostering a supportive funding relationship. This one-year grant
cycle will begin August 2024, with an application period beginning Monday, March 25th, 2024, and
closing Monday, May 6th, 2024. Virtual information sessions will be held on Thursday, April 11th &
Wednesday, April 17th at 4pm to offer applicants an oppor tunity to ask questions and learn more detail
about the grant. Any additional submitted questions will be publicly posted with coinciding answers.
The Advancing Systems Change grant program offers funding toward projects that address root causes of
health disparities, such as the promotion of social connectedness*, enhancement of inter-agency
partnerships & collaboration, and incorporation of community-based decision-making in operations. A
component of this work is intended to address needs identied in the 2024 Public Health Improvement
Plan*, specically Priority 2: Promoting Social Connectedness. These projects must directly serve
populations who experience disadvantages due to inequities in policies, practices, social norms, and the
distribution of resources. It is through dedication to these efforts that gradual system-level change can be
accomplished.
*The 2024-2028 Eagle County Public Health Improvement Plan (available through this link) is a ve-year
plan directly informed by Eagle County residents. By facilitating conversations with community members,
Eagle County Public Health and Environment was able to build a deeper understanding of needs, and
potential interventions, that can change the root causes of health disparities. This assessment led to the
determination of three priority areas, one of which being the Promoting Social Connectedness. This
priority encompasses the crucial need for the improvement of social infrastructure throughout Eagle
County, such as equal access to physical gathering spaces, programs, organizations, public
involvement, and even policies.
Eligible use of funds include:
Community outreach, engagement, and building efforts
Community building projects which create belonging
Direct voice and involvement from populations served through programs and projects
Community member empowerment (training, strengthening connections, advocacy)
Equity-focused professional development - staff and board
Strategic planning and organizational development; board (re)development, capacity building, or
sustainable funding
Recruitment of a Board of Directors or staff that represents the diverse populations of the
community
Enhancing or establishing safe gathering spaces to better accessibility and belonging
Supporting the development, or strengthening of, community-based social support groups

Eligibility and Requirements
Eligibility:
To be considered for funding, organizations must provide information identifying the following:
Registration as a nonprot with the State of Colorado and provide evidence of local programming or
projects which fosters community building and root cause work
Project applied for will directly impact residents of Eagle County, with the applicant’s service area
focused on the Eagle County and/or Roaring Fork Mid-Valley
Familiarity with unique needs, culture, and present services available within Eagle County and/or
Roaring Fork Mid-Valley
Awareness of the organization's role and function within the applicable systems
Requirements:
1. Sign a standard Eagle County Contract, including a Scope of Work
2. Par ticipation in coordination meetings: Minimum of three (3) meetings with ECPHE staff,
attendance in existing applicable collaboration meetings relevant to the proposed
project (ex: youth-focused organizations group convenings)
3. Data reporting: Mid-grant and post-grant outcome reporting as outlined in reporting template with
outcomes and indicators identied by grantee, annual report sharing as published in standard format
4. Evaluation: Participation in pre-grant and post-grant evaluation and quality measurement, as
identied by ECPHE staff

Just as there is no singular LGBTQIA+ experience, there is no singular rural experience. However, many
people in rural par ts of the country describe their communities in similar ways: built around family and
close-knit community; centered around strong social institutions such as churches, schools, and local
businesses; deeply connected to place and the environment; and based in a sense of efficacy and self-
reliance to make change in their own communities. Cer tain challenges and experiences are also
increasingly common in rural America, including the ongoing economic hardships; addiction and substance
abuse, including the opioid epidemic; fewer or more distant options for quality health care; and more. The
challenges of rural life often lead to different consequences for LGBTQ+ people, and can amplify LGBTQ+
people’s experiences of both acceptance and rejection.
Though rural America is home to a robust population of LGBTQIA+ people—as many as 3.8 million or some
20 percent of the country’s total LGBTQIA+ population—discrimination and isolation are still par ticularly
severe challenges for queer residents in America’s rural areas, like Eagle County. Those conditions are
interwoven with exclusion from employment, housing, access to community, and other oppor tunities, and
with marked health disparities.
This very real need led to the founding of what is now known as Mountain Pride. In 2020, Britny Rose, Eagle
County Pride's - Pride in the Park inaugural organizer, inquired about Eagle County not having a pride
festival. Within seven days of asking that question, she organized a successful pride festival attended by
hundreds. This resulted in the formation of a "Pride Committee '' composed of Eagle County residents
committed to growing and strengthening the LGBTQIA+ community and its allies in Eagle County. Eagle
County Pride hosted a variety of social events, meet-and-greets, and self-care activities such as yoga and
dancing classes.
Through intentional community outreach and listening, we have strived to understand and address the
diverse needs of LGBTQIA+ individuals across these regions. Which led us to develop our four strategic
program areas; community, education, advocacy, and activism, and resources. In the past year alone, we've
engaged with over 3,000 individuals through our four strategic program areas, resulting in more than 5,000
meaningful touchpoints.
While the organization's origins may have been rooted in creating visibility and connectedness, its mission
has expanded to encompass broader systemic issues affecting the LGBTQIA+ community. The grant
opportunity presents a strategic avenue for Mountain Pride to deepen its impact through advocacy,
education, and policy reform.

Mountain Pride has an inclusive, coalition-building strategy aimed at establishing trust and credibility in
order to secure the organization's long-term success. Mountain Pride is constantly expanding its activities
based on community needs, seeking collaborations with prospective organizational partners who share our
mission and vision, and referring individuals to partner groups that can assist them with their needs. To
achieve our goal of better serving the LGBTQ+ communities, we will implement the following strategies:
Community
A sense of belonging is a strong safety net that can support positive health outcomes, particularly those
belonging to marginalized communities. LGBTQ+ individuals exist in every community across the mountains
of Colorado. Community events seek to grow connections, lift our allies, enhance visibility and celebrate the
LGBTQ+ individuals through fun events. Events will benefit a wide range of people, including BIPOC, youth,
adults, and families.
Provide monthly activities and events for our diverse LGBTQIA+ communities to connect and foster
relationships
Implement Pride in the Park, a resource that celebrates our LGBTQIA+ community, while providing resources
and connections to attendees
Host the regional community-wide Gender & Sexuality Alliance meetings
Education
Promote acceptance and understanding of the LGBTQ+ community by delivering training, workshops, and
speaking engagements to the community, organizations, health care workers, providers, schools, and more
on various topics to engage in conversations that will help ensure welcoming and thriving environments for
all.
Training for allies on language, pronouns, vocabulary, gender-affirming practices, support, and more
Engage community par tners in building capacity through comprehensive training and consulting services on
LGBTQ+ best practices, equity, intersectionality, and inclusion
Provide training and resources which help educate and empower our LGBTQ+ individuals
Advocacy & Activism
Mountain Pride will support LGBTQ+ individuals with difficult situations like discrimination and harassment
and help navigate suppor t services. We will advocate for reform, connecting with local and state
representatives, encouraging civic engagement, and making investments for the betterment of our
communities.
Cultivate greater LGBTQIA+ civic representation and public presence/ participation
Proclaim June as pride month with the Eagle County Commissioners
Resources
We seek to provide information related to the community's informed needs, such as sexual or reproductive
health services, suicide and violence prevention, mental health resources, peer suppor t, gender-affirming
resources, coming out, HIV & STI, and more. Peer suppor t groups will be available year-round to assist
individuals in speaking with specialists to reduce anxiety, improve self-esteem, and help individuals' overall
well-being.

HIV and Other STI Testing
Peer Support Groups & 1:1 Peer Suppor t
Our approach encompasses a multifaceted strategy focused on promoting social connectedness,
enhancing par tnerships and collaboration, incorporating community-based decision-making into our
operations, and fostering community-based education for allyship.
According to Envision You’s State of the State Survey, approximately a third of respondents did not feel that
they have a community to which they belong. We recognize the significant impact of this kind of social
isolation and disconnectedness on health outcomes for LGBTQ+ community members. To address this, we
have implemented various programs and initiatives aimed at fostering social connectedness within our
community. These include regular (multiple times per month) community events, monthly suppor t groups,
and other outreach programming that creates meaningful interactions and connections among community
members. By promoting social connectedness, we aim to mitigate the adverse effects of isolation and
create a supportive environment conducive to overall health and well-being.
We understand that addressing disparities requires a collaborative effor t involving multiple stakeholders
and agencies. Mountain Pride has actively sought to strengthen par tnerships with local organizations
(SpeakUp ReachOut, Eagle Valley Community Foundation, Your Hope Center, Mountain Valley Horse Rescue,
Habitat for Humanity), healthcare providers (Vail Health), government agencies (Eagle County School
District, Town Government, and Eagle County Government), and other relevant stakeholders (Business
Owners). Through collaborative effor ts, we leverage collective exper tise, resources, and networks to develop
comprehensive solutions to health disparities. These par tnerships enable us to pool resources, share best
practices, and coordinate services more effectively.
We firmly believe in the importance of engaging the community served in decision-making processes that
affect them. We actively solicit input from community members through various channels, including
community forums, surveys, and our advisory committee. This community-driven approach ensures that our
programs and initiatives are responsive to the needs and priorities of the LGBTQ+ community and those
who experience multiple marginalizations that we serve.
We recognizes the impor tance of allyship in creating inclusive and suppor tive environments for all
community members, par ticularly marginalized populations like the LGBTQ+ community. To promote
allyship, we offer community-based education initiatives that raise awareness about social justice issues,
privilege, and systemic barriers. These initiatives include public events, workshops, workplace specific
training sessions, and educational materials designed to empower community members to become
effective allies in the fight against injustice. By fostering a culture of allyship, we aim to create a more
inclusive and equitable community where all members feel valued and suppor ted.

“Building Equitable Systems for LGBTQ+ Empowerment '' is a transformative initiative aimed at catalyzing
systems change to address the root causes of social isolation and inequity within LGBTQ+ communities in
Eagle County. By strategically allocating funds across our four program areas; community, education,
advocacy, and resources, we will implement a cohesive strategy that leverages community-based decision-
making, fosters collaboration, and promotes social connectedness to drive meaningful change at the
systemic level.
The first priority of this grant is to continue our foundation for community-building to ensure that everything
we do is guided with the community at the center. Through funding we will facilitate monthly, collaborative
community events providing inclusive spaces where LGBTQ+ individuals and allies can gather, exchange
experiences, and collectively advocate for change. These events will be co-designed with community input,
ensuring they authentically represent the diverse voices and needs of our population. In regards to co-
designed, there will be specific opportunities for youth to design events for youth to foster belonging within
the youth segment of the community.
Education serves as a cornerstone for challenging systemic discrimination and fostering understanding and
acceptance. We will offer comprehensive training sessions, workshops, and speaking engagements to equip
community members, organizations, healthcare providers, and educators with the knowledge and skills
needed to cultivate inclusive environments and challenge systemic injustices. By fostering collaboration
among stakeholders and facilitating shared learning experiences, we empower individuals to become
advocates for systemic change within their spheres of influence.
Advocacy is essential for dismantling systemic barriers and advancing LGBTQIA+ equity. We will collaborate
with local businesses, organizations, and policymakers to advocate for policy reforms that foster equity and
inclusion for LGBTQIA+ communities. This includes initiatives such as establishing "Safe Space" programs,
cultivating LGBTQ+ civic representation, and advocating for the adoption of inclusive policies and practices
across sectors. By amplifying the voices of LGBTQ+ individuals and allies, we promote community-based
decision-making and mobilize collective action to drive systemic transformation.
Access to resources is critical for supporting the well-being of LGBTQ+ individuals and addressing systemic
disparities. Through this grant, we will provide vital resources and support through our initiatives of Gender
Affirming Closet, HIV testing services, behavioral health support, and peer suppor t groups. These resources
will be continuously evaluated and adapted based on community feedback, ensuring they remain responsive
to evolving needs and contribute to lasting systemic change.

Mountain Pride is committed to the operation and funding of our programs into perpetuity through a
thoughtful, strategic, and multifaceted approach to organizational accountability and structure. We have a
solid Board of Directors who is a committed group of individuals who hold the space for both being a
founding board and identifying as LGBTQIA+ community members. Each member has verbalized and
actualized a willingness to step into varying and more hands-on roles while building the organizations
(fundraising suppor t, event management, etc.). 100% of Board Members have financially contributed to the
organization. Another impor tant note on sustainability is Mountain Pride’s role in the renewed and
reinvigorated community commitment to behavioral health through the Eagle Valley Behavioral Health “It
Takes a Valley Campaign''. As a community partner, we are encouraged through financial suppor t,
professional mentoring, and more. Lastly, in achieving 501c3 status, we have been able to continue to
diversify our funding by applying to grants more broadly, growing our existing “Pride in the Park'' business
sponsorship program, and continuing our individual fundraising campaigns around Colorado Gives Day and
other effor ts. In the last year, we have received generous funding from various grant sources as well
including but not limited to The Colorado Health Foundation, Colorado Parks and Wildlife Outdoor Equity
Grant Program, Colorado Department of Public Health and Environment Sexual Violence Prevention

Mountain Pride was created by our LGBTQ+ community growing out of our Pride in the Park committee
following our 2020 Pride in the Park. A grassroots organization of local people, we are uniquely positioned
to serve as a community convener. We pull together LGBTQ+ individuals and groups for brainstorming,
planning, implementation, and program assessments to address the needs of our rural LGBTQ+ community.
The original Pride in the Park group is still in formation and serves as the foundation for our organization.
This entire group identifies as par t of the LGBTQ+ community and includes individuals from all walks of life
and school and professional circles so they serve as our listening ears to the entire community. Out of this
group, our Executive Director stepped up first in a volunteer capacity to formalize the organization and then
eventually as the first hired staff. During this time, a Board of Directors was recruited from a diverse
convergence of skills and experiences, focusing on diversity within the LGBTQ+ community. The Pride in the
Park committee is now the Pride committee and works year round to gather feedback from the community
and plan events accordingly while the Board serves an advisory role specifically focusing on fiduciary
responsibility. Our Board of Directors is also composed of majorly queer individuals, 88% to be exact.
Our school based GSAs are student-run and lead. Students meet and connect with other trans and queer
students in their school. They work to create safe spaces and talk about the various issues they face in
school or their broader communities, such as discrimination from teachers or school administrators.
Students take a leadership role to improve school climate through campaigns and events that raise
awareness and change policies or practices in their schools.
Our community based GSAs is another way in which we are encouraging leadership and soliciting direction
from our community. They are structured similarly in our in schools programs but focus more on community
issues impacting adults and families outside of the school setting.

The core of Mountain Pride is a steadfast commitment to justice, equity, diversity, and inclusion, specific to
the LGBTQ+ community. All staff identify at LGBTQ+ and share many lived experiences with those that we
serve and seek to serve. We recognize however, the complexity of identifying with multiple marginalized
identities including but not limited to gender expansiveness, race, ethnicity, language, documentation status,
ability, and religion.
100% of staff are members of the LBGTQ+ community and over 90% of the Board of Directors is as well.
In Eagle County, where over 60,000 people reside, 31.6% are Latinx, with 51% Latinx student enrollment and
56% representing minority groups in the school district. Notably, 20% of residents are foreign-born
compared to Colorado's 9.7%, and 31% speak a language other than English at home, nearly double the
state's 16.7%. Specifically addressing the JEDI needs in our community and the complexity of multiple
marginalized identities, one of our foci is on the queer Hispanic community. Community-based leaders,
identifying as queer and Hispanic, lead effor ts including events like Hispanic Drag Shows, tailored
communication on Spanish radio stations and bilingual social media posts. Spanish language peer support
groups are also established.
Another area of focus is addressing the needs and challenges of the gender expansive community.
Throughout queer history this group has been erased from the narrative and thus the resources. We have
Board leadership that reflects this community, bring education specific to the issues this community faces
to our community including but not limited to focus on athletics and medical transition, as well as have
offered specific peer support groups led by and for this community.

Scope of Work: Terms of Funding, Deliverables, and Deadlines
These provisions are to be read and interpreted in conjunction with the provisions of the
Grant Agreement specified above.
I. Entity Name: Mountain Pride
II. Project Description: This project serves to build equitable systems for LGBTQ+
empowerment by focusing on projects that build community, offer opportunities for
education, advocacy, and resource connection.
III. Definitions:
Community Voice: An approach to decision-making, project management, and strategic
planning which incorporates direct feedback and redistributes power to community
members most impacted by the project or programming.
Root cause: A descriptor for the underlying reasons that create the differences seen in
health outcomes which originates from the community in which people live, and the
systems that dictate accessibility to resources and opportunities necessary for survival.
Systems change/systems-level-work: The shift of responsibility for poor health outcomes
from individual behavior, to the community systems which individuals rely on, where
inequities in access to opportunities for success exist. This addresses population health
measures on social, racial/ethnic, economic, and physical factors- often referred to as social
determinants of health.
Health Equity Framework: A Public Health Framework developed to collect data,
establish and evaluate projects/resources, and approach strategic planning in a manner that
includes and addresses health disparities and inequities.
Social Connectedness: A continuum of the size and diversity of one’s social network and
roles, the functions these relationships serve, and their positive or negative qualities”,
demonstrating that social connection, and its adverse social isolation, are significant
predictors of lifespan and quality of health. The quality of social connection influences the
physical, behavioral, and economic health of a person and a community.

V. Monitoring:
Eagle County Public Health and Environment will monitor this Grant Agreement for
compliance with performance requirements throughout the Grant Period. Methods used will
include progress meetings with ECPHE as needed/requested with required documentation
provided by the Grant Recipient.
VI. Resolution of Non-Compliance:
The Grant Recipient will be notified in writing within 15 calendar days of discovery of a
compliance issue. Within 30 calendar days of discovery, the Grant Recipient and Eagle
County Government will collaborate, when appropriate, to determine the action(s) necessary
to rectify the compliance issue and determine when the action(s) must be completed. The
action(s) and timeline for completion will be documented in writing and agreed to by both
parties.
