GRANT AGREEMENT BETWEEN
EAGLE COUNTY, COLORADO
AND
EAGLE VALLEY COMMUNITY FOUNDATION
THIS GRANT AGREEMENT (“Agreement”) is effective as of July 1, 2024 by and between
Eagle Valley Community Foundation (hereinafter “Grant Recipient”) and Eagle County,
Colorado, a body corporate and politic (hereinafter “Grantor”)
RECITALS
WHEREAS, Eagle County works to promote the health, safety, and welfare of Eagle County
citizens of all ages; and
WHEREAS, the County uses outside providers and professionals to enhance the ability of County
to promote such health, safety and welfare; and
WHEREAS, Eagle County Public Health & Environment (“ECPHE”) has identified a significant
community need for funding and services dedicated to the ‘promotion’ phase of care, which
addresses root causes of local health disparities, which lead to a myriad of individual high-risk
behaviors; and
WHEREAS, the observed and reported fragmentation of community organizations and non-
profits leads to a lack of strategy in provision of services and programming across systems of care;
and
WHEREAS, Eagle County Public Health & Environment would like to extend staff knowledge
and experience to offer organizational support while administering funds with the intent of infusing
equity and inclusion from a systemic level; and
WHEREAS, the parties acknowledge and agree that the programs and services detailed herein
promote the health, safety and welfare of the people of Eagle County and are intended to serve a
valid public purpose and provide services that could otherwise be performed by County directly;
and
WHEREAS, Grant Recipient has elected to perform services that help create and sustain a healthy,
environmentally and economically vital and socially diverse community in Eagle County on its
own and County has, therefore, elected to compensate Grant Recipient to perform the Services for
the public to fulfill an important public purpose; and
WHEREAS, Grant Recipient is authorized to do business in the State of Colorado and has the
time, skill, expertise, and experience necessary to provide the services as defined below in
paragraph A hereof; and

WHEREAS, this Agreement shall govern the relationship between Grant Recipient and County
in connection with the services.
GRANT AGREEMENT
Now, therefore, in consideration of the foregoing premises and the following promises, Eagle
County and Grant Recipient enter into this Agreement for payment of the grant award in the
amount of six thousand dollars ($6,000, the “Grant Award”) to Grant Recipient.
A. Grant Recipient agrees that funds awarded by Grantor may be used and expended solely
for services and programs related to its ‘Empowerment Through Elevar: Strengthening Authentic
Leadership and BIPOC Entrepreneurship in Eagle County’ project as specified in its 2024
Advancing Systems Change Grant Application, attached hereto as Exhibit A and incorporated
herein by reference, and Grant Application Budget Section. All funds will be expended in the one-
year grant period, with the grant period terminating exactly one-year from contract execution (the
“Grant Period”). The Grant Award set forth herein is for the specific Grant Period and the Grantor
makes no representations, nor should Grant Recipient rely on this award as a representation of any
future awards for other grant periods.
B. In return for the Grant Award, Grant Recipient agrees to the terms of funding, deliverables,
and deadlines outlined in the Scope of Work attached hereto as Exhibit B, and incorporated herein
by reference. Grant Recipient shall a report discussing how the funds were expended, deliverables
as outlined in the Grant Application, and how they tie back to creating and sustaining a healthy
environmentally and economically vital and socially diverse community. The report will be due
no later than July 1, 2025.
C. Payment of the Grant Award will be made in one installment.
D. Grant funds under this Grant Agreement are to be used and expended within the Grant
Period. Any funds not used or expended during the Grant Period must be returned to Grantor.
E. If, after payment of the Grant Award, Eagle County reasonably determines that payment
was improper because the purposes for which the payment was made were misrepresented, or the
terms of this Agreement were breached, then upon written notice of such determination and request
for reimbursement from Eagle County, Grant Recipient shall forthwith return such payment to
Eagle County.
F. Grant Recipient shall comply at all times and in all respects with all applicable federal,
state and local laws, resolutions and codes; and specifically, with the requirements of the Civil
Rights Act of 1964 and Section 504, Rehabilitation Act of 1973, concerning discrimination on the
basis of race, color, sex, age, religion, political beliefs, national origin or handicap.
G. Grant Recipient shall not assign any of its rights or duties under this Grant Agreement to a
third party without the prior written consent of the Grantor. The Grantor shall terminate this Grant
Agreement in the event of any assignment without its prior written consent, in which case, any
unused Grant Funds must be returned to Grantor.

H. Grant Recipient agrees that no agent, employee or volunteer of Grant Recipient shall be
deemed an agent, employee or volunteer of County.
I. Grant Recipient shall maintain, for a minimum of three (3) years, adequate financial and
programmatic records for reporting to Grantor on performance of its responsibilities hereunder.
Grant Recipient shall be subject to financial audit by federal, state or county auditors or their
designees. Grant Recipient authorizes Grantor to perform audits or to make inspections during
normal business hours, upon 48 hour notice to Grant Recipient for the purpose of evaluating
performance under this Grant Agreement. Grant Recipient shall cooperate fully with authorized
Eagle County representatives in the observation and evaluation of the program and records. Grant
Recipient shall have the right to dispute any claims of misuse of funds and seek an amicable
resolution with Grantor.
J. This Agreement shall be governed by the laws of the State of Colorado. Jurisdiction and
venue for any suit, right or cause of action arising under, or in connection with this Agreement
shall be exclusive in Eagle County, Colorado.
K. This Grant Agreement supersedes all previous communications, negotiations, and/or
agreements between the respective parties hereto, either verbal, or written, and the same not
expressly contained herein are hereby withdrawn and annulled.
L. This Grant Agreement does not, and shall not be deemed or construed to confer upon or
grant to any third party or parties any right to claim damages or to bring any suit, action or other
proceeding against either Grant Recipient or Grantor because of any breach hereof or because of
any of the terms, covenants, agreements and conditions contained herein.
M. Invalidity or unenforceability of any provision of this Grant Agreement shall not affect the
other provisions hereof, and this Grant Agreement shall be construed as if such invalid or
unenforceable provision was omitted.
N. To the extent required to perform under this Grant Agreement, Grant Recipient represents
and warrants that it has the expertise and personnel necessary to properly perform the Grant
Agreement and its professional personnel are duly licensed to perform.
O. No failure or delay by either party in the exercise of any right hereunder shall constitute a
waiver thereof. No waiver of any breach shall be deemed a waiver of any preceding or
succeeding breach.
P. The signatories to this Grant Agreement aver to their knowledge, no employee of the
County has any personal or beneficial interest whatsoever in the matters described in this Grant
Agreement. The Grant Recipient has no beneficial interest, direct or indirect, that would conflict
in any manner or degree with the performance of the Grant Agreement and Grant Recipient shall
not employ any person having such known interests.

Q. Grant Recipient shall be responsible for the completeness and accuracy of its
performance under the Grant Agreement, including all supporting data or other documents
prepared or compiled in performance of the Grant Agreement, and shall correct, at its sole
expense, all significant errors and omissions therein. The fact that the Grantor has accepted or
approved the work performed shall not relieve Grant Recipient of any of its responsibilities.
Grant Recipient shall perform the Grant Agreement in a skillful, professional and competent
manner and in accordance with the standard of care, skill and diligence applicable to Grant
Recipients performing similar services. This paragraph shall survive termination of this Grant
Agreement.
S. Grant Recipient shall carry workers compensation insurance as required by law and
automobile and commercial general liability insurance as necessary and appropriate during the
term of the Grant Agreement.
T. The Grant Recipient shall indemnify and hold harmless Grantor, and any of its officers,
agents and employees against any losses, claims, damages or liabilities for which Grantor may
become subject to insofar as an such losses, claims, damages or liabilities arise out of, directly or
indirectly, this Grant Agreement, or are based upon any performance or nonperformance by
Grant Recipient or any of its sub-recipients hereunder including claims for bodily injury or
personal injury including death, or loss or damage to tangible or intangible property; and Grant
Recipient shall reimburse Grantor for reasonable attorney fees and costs, legal and other
expenses incurred by Grantor in connection with investigating or defending any such loss, claim,
damage, liability or action. This indemnification shall not apply to claims by third parties
against the Grantor to the extent that Grantor is liable to such third party for such claims without
regard to the involvement of the Grant Recipient. This paragraph shall survive expiration or
termination hereof.
U. Grant Recipient certifies that it has read the Grant Agreement, understands each and every
term and the requirements set forth herein, and agrees to comply with the same.

Advancing Systems Change Grant: Program
Application 2024
Eagle County Public Health & Environment (ECPHE) is thrilled to announce the 2024 Advancing Systems
Change Grant opportunity. Involvement will be highly interactive and adaptable to the grantee's needs,
goals, and limitations, with the intent of fostering a supportive funding relationship. This one-year grant
cycle will begin August 2024, with an application period beginning Monday, March 25th, 2024, and
closing Monday, May 6th, 2024. Virtual information sessions will be held on Thursday, April 11th &
Wednesday, April 17th at 4pm to offer applicants an oppor tunity to ask questions and learn more detail
about the grant. Any additional submitted questions will be publicly posted with coinciding answers.
The Advancing Systems Change grant program offers funding toward projects that address root causes of
health disparities, such as the promotion of social connectedness*, enhancement of inter-agency
partnerships & collaboration, and incorporation of community-based decision-making in operations. A
component of this work is intended to address needs identied in the 2024 Public Health Improvement
Plan*, specically Priority 2: Promoting Social Connectedness. These projects must directly serve
populations who experience disadvantages due to inequities in policies, practices, social norms, and the
distribution of resources. It is through dedication to these efforts that gradual system-level change can be
accomplished.
*The 2024-2028 Eagle County Public Health Improvement Plan (available through this link) is a ve-year
plan directly informed by Eagle County residents. By facilitating conversations with community members,
Eagle County Public Health and Environment was able to build a deeper understanding of needs, and
potential interventions, that can change the root causes of health disparities. This assessment led to the
determination of three priority areas, one of which being the Promoting Social Connectedness. This
priority encompasses the crucial need for the improvement of social infrastructure throughout Eagle
County, such as equal access to physical gathering spaces, programs, organizations, public
involvement, and even policies.
Eligible use of funds include:
Community outreach, engagement, and building efforts
Community building projects which create belonging
Direct voice and involvement from populations served through programs and projects
Community member empowerment (training, strengthening connections, advocacy)
Equity-focused professional development - staff and board
Strategic planning and organizational development; board (re)development, capacity building, or
sustainable funding
Recruitment of a Board of Directors or staff that represents the diverse populations of the
community
Enhancing or establishing safe gathering spaces to better accessibility and belonging
Supporting the development, or strengthening of, community-based social support groups

Eligibility and Requirements
Eligibility:
To be considered for funding, organizations must provide information identifying the following:
Registration as a nonprot with the State of Colorado and provide evidence of local programming or
projects which fosters community building and root cause work
Project applied for will directly impact residents of Eagle County, with the applicant’s service area
focused on the Eagle County and/or Roaring Fork Mid-Valley
Familiarity with unique needs, culture, and present services available within Eagle County and/or
Roaring Fork Mid-Valley
Awareness of the organization's role and function within the applicable systems
Requirements:
1. Sign a standard Eagle County Contract, including a Scope of Work
2. Par ticipation in coordination meetings: Minimum of three (3) meetings with ECPHE staff,
attendance in existing applicable collaboration meetings relevant to the proposed
project (ex: youth-focused organizations group convenings)
3. Data reporting: Mid-grant and post-grant outcome reporting as outlined in reporting template with
outcomes and indicators identied by grantee, annual report sharing as published in standard format
4. Evaluation: Participation in pre-grant and post-grant evaluation and quality measurement, as
identied by ECPHE staff

EVCF is a young, entrepreneurial organization founded to address the greatest needs of the marginalized
people living and/or working within the Eagle River Valley and beyond. EVCF is not a traditional community
foundation, rather we work in conjunction with community par tners to both deliver direct service programs
and suppor t the impact and capacity building of other nonprofit organizations.
EVCF is a convener, inspiring collective action to local challenges, always keeping our ear to the ground and
providing responsive solutions to the most emerging needs of Eagle Valley community members. Current

Wellness Resources Navigation and Referrals (MIRA), Community Capacity and Emerging Leadership

At the Community Market (TCM), we believe access to food is a right and not a privilege. TCM’s objective is
to provide low-income communities with healthy food with no barriers to access as well as to prevent food
from going to waste. Through two storefront markets, located in Gypsum and Edwards, and pop-up Mobile
Markets available throughout the Eagle River Valley, The Community Markets are addressing food insecurity
by providing a variety of fresh produce, dairy, meats, and bread as well as shelf-stable grocery items for
customers to shop on a regular basis.Today, we continue to feed over 4,800 people per week. As we
develop our organizational strategic plan for the next 3 years, we plan to hone in on local sourcing and food
recovery systems as well as sustaining existing community par tnerships with senior and youth meal and
snack programs.
MIRA is a single-stop, comprehensive and culturally relevant approach to resource connection and service
delivery. MIRA brings resources directly to neighborhoods and workplaces to enable government and
community services, resources, and programming to go directly into neighborhoods, schools, community
places, and other gathering/event locations with culturally relevant branding, recognized and trusted
leadership, and consistent data collection. The vision of MIRA is that all community members will be able to
access resources in a way that is cohesive and streamlined, in an asset-based, culturally relevant and whole
family approach. MIRA travels to each of the six largest mobile home developments in our county 1-3 times
per month serving over 4,100 people each year. The next 3 years of MIRA will focus on expanding our
offerings as described above and expanding our annual health and wellness fair and monthly health
education series. Each month, we have at least one event that focuses on bringing resources to the
community in a more engaging way.
Elevar is an emergent leadership development program and social impact fund that provides community
investments through grantmaking and program-related investments to emerging Latinx leaders and people
of color. Elevar creates the connection point between existing education and leadership training programs
to the resources needed for emerging leaders to make their dreams a reality, while building economic power,
and creating a more empowered community. Elevar has 11 steering committee members and a founding
cohor t of 7 entrepreneurs. We are just getting started as this program continues to grow and evolve.
Entrepreneurs will receive 6-month training on business management, finances, and marketing to suppor t
their journey.
We also have a fiscal sponsorship program. Our Foundation provides fiduciary oversight, financial
management, and other administrative services to help build the capacity of these charitable projects.
Currently, we have 7 fiscal sponsorship projects. We also host an annual nonprofit leadership gathering.

EVCF has a robust history of engaging in root-cause analysis to address complex social issues and drive
meaningful change within the community. Through our various programs and initiatives, we have developed
a deep understanding of the underlying factors contributing to the challenges faced by individuals and
communities in Eagle Valley.
Our approach to root-cause work is characterized by:
Community and Stakeholder Engagement: We recognize the impor tance of engaging with diverse
stakeholders, including community members, nonprofit organizations, government agencies, and other key
partners, in the root-cause analysis process. Through collaborative discussions and participatory
approaches, we ensure that a wide range of perspectives are considered and that solutions are informed by
the lived experiences of those most affected by the issues at hand.
Systems Thinking: Our root-cause work is guided by a systems thinking approach, which acknowledges the
interconnectedness of various social, economic, and environmental factors that shape individual and
community well-being. By examining the underlying systemic structures and dynamics that perpetuate
inequities and barriers to oppor tunity, we are better equipped to develop holistic solutions that address root
causes rather than symptoms.
Data-Driven Analysis: We place a strong emphasis on data collection and analysis to identify patterns,
trends, and disparities within the community. By analyzing quantitative data such as demographic
information, socioeconomic indicators, and service utilization rates, as well as qualitative data gathered
through interviews, surveys, and focus groups, we gain valuable insights into the root causes of social
problems. We par tner with many organizations on their Community Needs Assessments as a way to analyze
data and summarize the needs of the community.
Continuous Learning and Adaptation: Root-cause analysis is an iterative process that requires ongoing
learning, reflection, and adaptation. We are committed to continuously refining our understanding of
complex social issues and refining our strategies based on evidence and feedback. By embracing a culture
of learning and innovation, we strive to remain responsive to the evolving needs and dynamics of the
community.
Overall, our experience with root-cause work has underscored the importance of taking a comprehensive
and collaborative approach to addressing social challenges. By delving beneath the surface to uncover the
underlying drivers of inequity and injustice, we can develop more effective and sustainable solutions that
promote positive change and improve the lives of individuals and families in Eagle Valley.

Empowerment Through Elevar: Strengthening Authentic Leadership and BIPOC Entrepreneurship in Eagle
County
Elevar Steering Committee Development: This investment will enable committee members to assume
additional leadership roles within Elevar and extend their impact to the broader community. Steering
Committee Members are in the process of delineating their responsibilities and organizing specialized
committees tailored to their individual strengths and preferences. These may include a Finance Committee,
a Communication and Marketing Committee, alongside other committees designed to address additional
vital aspects.
Elevar Entrepreneurial Cohor t Expansion: Funds will suppor t the expansion of the Elevar Entrepreneurial
Cohor t, welcoming new members and fostering the growth of the founding cohor t. By providing mentorship,
training, and resources, we aim to empower entrepreneurs to realize their full potential and contribute to the
economic vitality of Eagle Valley. We aim to establish a partnership with the Rocky Mountain Microfinance
Institute for an intensive six-month curriculum, wherein Steering Committee Members will undergo training
as trainers. By fostering this capacity within the bilingual community, we maintain the curriculum's relevance
and impact.
Community Round Tables: Funds will be used to organize and facilitate 7-8 roundtable discussions on
various themes relevant to the Elevar community. These roundtables will serve as forums for connecting
subject matter exper ts with Elevar Steering Committee members and entrepreneurs, fostering two-way
education and collaboration.
Mentorship Program: We will establish a mentorship program within Elevar, pairing experienced Steering
Committee members with entrepreneurs to provide guidance, suppor t, and networking oppor tunities.
System Reform: Effor ts to reform existing systems within partner organizations like VVP, CMC, Bravo Vail!,
TV8, and Speak Up Reach Out, among others, through initiatives spearheaded by Steering Committee
Members. For instance, we've initiated collaborations with CMC and VVP, facilitating focus groups to gather
insights on SC experiences within their courses and advising on strategies to enhance inclusivity for BIPOC
communities.
Additionally, we aim to establish resources such as an internship directory connecting local youth from
universities with businesses for summer oppor tunities, while another initiative involves compiling a roster of
BIPOC-owned small businesses.
EVCF Staffing Capacity Enhancement: We will focus on enhancing EVCF staffing capacity to provide
consistent suppor t to Elevar par ticipants. This includes hiring Contracted Community Engagement Manager
and Entrepreneur in residence to complement existing full-time staff, ensuring that par ticipants receive the
guidance and assistance they need to succeed.

Steering Committee:

Ensure equitable access to resources and support services provided by par tner organizations, prioritizing

Eagle Valley Community Foundation receives funding from diverse sources including grants from private
foundations and local municipalities, businesses, board members, individuals, and community-based
grassroots donors. This diverse range of funding ensures we are not overly reliant on any one source of
revenue, and we can adapt should funding fall through. We operate a lean and balanced budget to suppor t
the needs of the community and rely 100% on funding from our suppor ters and funders each year to fulfill
our mission. Specific to this grant request, Eagle Valley Community Foundation has secured a grant from
the Colorado Health Foundation for three years to suppor t the development of Elevar. We also have
generous individuals who have designated their suppor t to the Elevar program. We continue to develop a
diverse grant por tfolio and see oppor tunities for Elevar to secure additional justice, equity, diversity, and
inclusion funding. In addition to cash funding, we have many in kind par tnerships with Alpine Bank, private
law firms, VVP, Latinx House, Bravo! Vail, CMC, Vail Daily, and more.

Through intentional effor ts to build community and forge meaningful connections, Elevar addresses the
imperative need for social suppor t networks, par ticularly among historically marginalized populations.
At the hear t of Elevar's mission is the empowerment of BIPOC individuals, including entrepreneurs and
members of the steering committee. By providing a platform for collaboration, skill-building, and resource-
sharing, Elevar cultivates a vibrant community where BIPOC entrepreneurs can thrive and suppor t one
another. Through workshops, networking events, and mentorship opportunities, Elevar facilitates the
exchange of knowledge and fosters a sense of solidarity among par ticipants, thereby strengthening social
ties and enhancing community resilience.
Elevar focuses on the creation of inclusive spaces where the BIPOC community can foster genuine
connections and feel a sense of safety, fostering a profound sense of belonging among both the Steering
Committee and Entrepreneurs. Encouragingly, 100% of the Steering Committee has expressed a keen
interest in sustaining our collaborative efforts. As one member aptly stated, “While we acknowledge areas
of exper tise that may pose challenges, our strength lies in our unity and mutual support.”
In addition to fostering connections within the BIPOC community, Elevar serves as a bridge for building
connections with a diverse array of stakeholders, including Board of Directors members, community leaders,
and representatives from passion area nonprofits. Through collaborative initiatives and shared endeavors,
Elevar facilitates meaningful interactions and cross-sector par tnerships that transcend traditional
boundaries. By bringing together individuals and organizations with varied backgrounds, perspectives, and
exper tise, Elevar promotes collective action and creates opportunities for mutual learning and growth.
Elevar's commitment to inclusivity and collaboration extends beyond superficial interactions to foster
genuine relationships grounded in trust, respect, and reciprocity. By actively engaging with diverse

Objectives:
#1: Allocate resources to empower and invest in individuals, par ticularly those from diverse BIPOC

#5: Allocate funds to develop a comprehensive professional development program in collaboration with
partners such as Rocky Mountain Microfinance Institute, Colorado Mountain College, Vail Valley

EVCF adopts a comprehensive approach to evaluation, integrating various methodologies to assess the
effectiveness and impact of its programs, notably the Elevar initiative. At the core of our evaluation strategy
is a commitment to understanding the nuanced needs of our diverse community and fostering sustainable,
long-term change.
To ensure inclusivity and cultural responsiveness, we prioritize engaging with stakeholders through surveys
tailored to the specific demographics we serve. For instance, we implement surveys targeting The
Community Market (TCM) customers, aiming to glean insights into their cultural preferences, identify
potential barriers, and ascer tain additional needs. Similarly, surveys are deployed for MIRA clients.
The success of our programs, particularly TCM, MIRA, and Elevar, is multifaceted and extends beyond
numerical metrics. While quantitative data, such as entry survey responses collected by volunteers and
staff, are vital in tracking trends and identifying community needs, we recognize the impor tance of
qualitative indicators in assessing impact. Exit data from the MIRA program, for instance, illuminates
increased awareness and utilization of essential services, including health, education, housing, and food
assistance.
Elevar is evaluated through a combination of quantitative and qualitative measures aimed at capturing both
individual outcomes and broader community impacts. Pre- and post-surveys administered to Elevar steering
committee members and cohor t par ticipants enable us to track behavioral changes, knowledge acquisition,
and the development of supportive relationships over time.
Beyond shor t-term metrics, EVCF is committed to fostering enduring community transformation. This
necessitates a holistic evaluation approach that encompasses behavioral changes, shifts in attitudes,
knowledge acquisition, and the cultivation of meaningful relationships. Through periodic surveys, focus
groups, and ongoing dialogue with stakeholders, we seek to measure not only the scale of our impact but

Our focus has always been to meet basic needs but in the last few years we have realized we need to do
more to create systematic change, we have to change the economic landscape of our community.
Our vision is to collectively build a community that embraces diversity, honors inclusivity and actively works
to dismantle the systems and institutions that prevent communities of color from having a voice and
leadership roles across all aspects of society. We will listen, learn, and collaborate to activate community
power, create space for community voice, and give the communities we serve an oppor tunity to create and
implement ideas with, by and for communities of color throughout Eagle County.
This program addresses root causes by empowering community members to identify their own solutions
and oppor tunities while providing the learning and support necessary to succeed and the funding needed to
get star ted. Often BIPOC community members are unable to attain economic prosperity or star t their own
businesses because they do not have traditional sources of funding or wealth such as generational wealth,
credit or equity to take out business loans, or the knowledge necessary to begin these financial processes
and engage in our country’s capitalistic economic system.
Limited access to networks and social capital can make it difficult for individuals to connect with others
who can help them advance in their careers and succeed in their endeavors. Elevar is creating network
opportunities for potential leaders in our community who do not have access to those groups.
For those who are not fluent in English, language barriers can create additional challenges and make it
difficult to access cer tain oppor tunities and succeed in cer tain fields. All Elevar programming is conducted
bilingually and continues to work to advocate for bilingual programming across the community. In addition,
individuals who are not citizens, or who are not authorized to work in the US, their immigration status can
create additional barriers to social mobility, especially in star ting their own businesses, because Elevar has
a mentoring and 1:1 approach EVCF is able to walk alongside individuals through these barriers in order to
achieve economic success.
We could continue to list these root causes, but know that they are endless, reiterating the impor tance of a
cohor t model to provide peer support, mentoring, etc. when addressing systemic barriers.

Specific to this grant request, The Elevar program uniquely redefines typical decision making. Elevar boasts
its own Steering Committee, or decision making group, composed of 100% Latinx individuals. These 11
community leaders are a force to be reckoned with and represent the diversity of workforce options within
our community. To remove barriers to par ticipation and ensure equitable representation, Steering
Committee members are compensated for their time. Each Steering Committee member has also been
given a scholarship to obtain self-identified skills by taking courses through Colorado Mountain College,
attending events hosted by Vail Valley Partnership, and achieving cer tifications that range from
interpretation and marketing to Board Service and Community Leadership. In their inaugural year, the
Steering Committee developed the grant application process, assisted with recruitment, and evaluated all 32
applications received. In the next year, we are excited to identify growth oppor tunities for each Steering
Committee Member to extend their service with Elevar through new roles. The Entrepreneur Cohor t group
will receive an intensive 6-month training program that will enhance and unite them with their mentors who
are par t of the Steering Committee.
Eagle Valley Community Foundation’s (EVCF) role is to gather people together who are doing good work and
listen intently to needs and gaps that exist in our community, working collectively on solutions. As a
community convener and trusted resource, staff, leadership, and volunteers are integrated into the
neighborhoods in which we serve and have deep understandings of barriers to health and social equity in
our community. Many staff are people introduced to EVCF first as customers and clients of our programs.
Since launching The Community Market in 2018, six customers have advanced into full-time staff roles
within Eagle Valley Community Foundation. 100% of our Executive leadership team reflects the diverse
populations that we serve and over 60% of our staff identify as people of color and/or come from
historically marginalized communities. Our diverse staff have experiences as customers, volunteers, and as
donors. These perspectives help to shape how we serve our community, while recognizing that we are
neither the exper ts nor the authority on the issues in which we work. We embrace people with diverse
perspectives and experiences to join us and engage in organizational decision-making. All programs that
have emerged from EVCF are a result of community informed and community-driven needs. We enter every
partnership and conversation with compassion, humility and a spirit of learning.

III. Definitions:
Community Voice: An approach to decision-making, project management, and strategic
planning which incorporates direct feedback and redistributes power to community
members most impacted by the project or programming.
Root cause: A descriptor for the underlying reasons that create the differences seen in
health outcomes which originates from the community in which people live, and the
systems that dictate accessibility to resources and opportunities necessary for survival.
Systems change/systems-level-work: The shift of responsibility for poor health outcomes
from individual behavior, to the community systems which individuals rely on, where
inequities in access to opportunities for success exist. This addresses population health
measures on social, racial/ethnic, economic, and physical factors- often referred to as social
determinants of health.
Health Equity Framework: A Public Health Framework developed to collect data,
establish and evaluate projects/resources, and approach strategic planning in a manner that
includes and addresses health disparities and inequities.
Social Connectedness: A continuum of the size and diversity of one’s social network and
roles, the functions these relationships serve, and their positive or negative qualities”,
demonstrating that social connection, and its adverse social isolation, are significant
predictors of lifespan and quality of health. The quality of social connection influences the
physical, behavioral, and economic health of a person and a community.

with the population(s) most impacted by the organization’s programming/funded project.
Objective IIIa: Grant Recipient will connect directly with community members of the
population most impacted by the organization with the purpose of establishing trusted
relationships and authentic community-voice inclusion in operations/decision-making.

groups, individual interactions, participation in
community events, and/or similar methods of direct

an effort to establish, determine, and/or strengthen the Organization’s role within the
population’s systems/system of care.
Objective IVa: Grant Recipient will communicate/collaborate with key community partners in
assessing the needs of the population most impacted by the project.
Deliverables Deadlines
1. Grant Recipient will provide ECPHE information
regarding EVCF role in participation of regular
(monthly) Latine-serving coalitions, groups,
strategies, and/or similar convenings (e.g.: District
Wellness Committee).

V. Monitoring:

Eagle County Public Health and Environment will monitor this Grant Agreement for
compliance with performance requirements throughout the Grant Period. Methods used will
include progress meetings with ECPHE as needed/requested with required documentation
provided by the Grant Recipient.
VI. Resolution of Non-Compliance:
The Grant Recipient will be notified in writing within 15 calendar days of discovery of a
compliance issue. Within 30 calendar days of discovery, the Grant Recipient and Eagle
County Government will collaborate, when appropriate, to determine the action(s) necessary
to rectify the compliance issue and determine when the action(s) must be completed. The
action(s) and timeline for completion will be documented in writing and agreed to by both
parties.
