EAGLE COUNTY, COLORADO
AND
LA COCINA
THIS SECOND AMENDMENT (“Second Amendment”) is effective as of ____________ by
and between La Cocina, a Colorado Non-Profit Corporation (hereinafter “Consultant” or
“Contractor”) and Eagle County, Colorado, a body corporate and politic (hereinafter “County”).
RECITALS
WHEREAS, County and Consultant entered into an agreement dated the 8th day of June, 2023,
for certain Services (C23-195, the “Original Agreement”); and
WHEREAS, the Original Agreement contemplated that the Consultant would perform certain
Services with compensation in an amount not to exceed $35,000; and
WHEREAS, County and Consultant entered into an agreement dated the 9th day of December,
2023, for certain Services (C23-390, the “First Amendment”); and
WHEREAS, the First Amendment contemplated that the Consultant would perform certain
additional Services with compensation in an amount not to exceed $10,000; and
WHEREAS, the County desires to have Consultant perform additional Services for additional
compensation as set forth below; and
WHEREAS, the term of the First Amendment expires on the 31st day of May, 2024, and the
parties desire to extend the term for an additional year.
FIRST AMENDMENT
NOW THEREFORE, in consideration of the foregoing and the mutual rights and obligations as
set forth below, the parties agree as follows:
1. The Original Agreement shall be amended to include additional Services as described
in Exhibit 1, which is attached hereto and incorporated herein by reference.
2. The compensation for the additional Services set forth in Exhibit 1 shall not exceed
$45,000 for a total maximum compensation under the Original Agreement, the First
Amendment and this Second Amendment of $90,000.
3. The term of the Original Agreement is hereby extended to the 31st day of May, 2025.
4. Capitalized terms in this Second Amendment will have the same meaning as in the
Original Agreement and First Amendment. To the extent that the terms and
provisions of the Second Amendment conflict with, modify or supplement portions of

the Original Agreement, the terms and provisions contained in this Second
Amendment shall govern and control the rights and obligations of the parties.
5. Except as expressly altered, modified and changed in this Second Amendment, all
terms and provisions of the Original Agreement shall remain in full force and effect,
and are hereby ratified and confirmed in all respects as of the date hereof.
6. This Second Amendment shall be binding on the parties hereto, their heirs, executors,
successors, and assigns.
IN WITNESS WHEREOF, the parties hereto have executed this Second Amendment to the
Original Agreement the day and year first above written.

Services for Early Head Start:
Monthly Group LiBRC for EHS
Manager/Supervisor 12 sessions @ $220/hr
Monthly Group LiBRC for EHS/DHS
Managers/Supervisors 12 sessions @ $220/hr
Monthly Group LiBRC for Home Visitors
and Family Advocates 12 sessions @ $220/hr
Half-day training (four-and-one half hours)
delivered by two La Cocina
expert trainers; may be requested in
English or Spanish; cost for
20-participants.
$1600 per single

Individual LiBRC for EHS staff (as needed) @ $220/hr up to 20 sessions/year
Bilingual Infancy and Early Childhood
Mental Health (IECMH)
consultation for families
@ $200/session up to 5/year
Mileage Reimbursement for any in-person
trainings/ meetings .67 per mile (2024 rates)
*Subject to change: ECDHS will use federal
guidelines/ rates for mileage reimbursement
Mental Health Services for CORE Services program:
Bilingual counseling or psychotherapy services $150 per hr
Bilingual children, youth and young adult’s life adversity
and/or trauma screenings Flat Rate of $900
Bilingual early childhood neurodevelopmental
(psychological) evaluation Flat rate of $3,000
Bilingual children, youth and young adult’s psychological
assessment, including neuropsychological assessment Flat rate of $3,000
Bilingual group counseling or psychotherapy services $75 per session “episode”
Case management (report writing/ written or verbal
caseworker updates, sending invoices, etc) $35 per hour

Attendance at family engagement meetings $50 per hour (typical family
engagement meetings last two
hours)
Travel Time for CORE Services-- Invoices for this can be
sent directly to dhsacctpayable@eaglecounty.us
$35 per hour (Maximum of 30
hours per contact year).
