As requested, we observed the installation of the onsite wastewater system for the subject site on April
22, 2004. The system was designed under our Job No. 14618W, dated December 11, 2001and revised
August 5, 2002.
The system includes the installation of a 1500-gallon, two -compartment, concrete septic tank with a
pump in a BiotubeTM pump vault, and a mounded chamber -trench drain field. Each trench had one row of
12 chambers for a total of 48 chambers and of 744 ft2 of infiltrative area. An as -built drawing is,included
as Figure 1.
The components of the OWS design were installed in conformance with our plans and specifications.
Final electrical connections must be made by a licensed electrician. The construction foreman may
activate the pumping system at his discretion.

The location of the cleanout in the sewer line was not noted during our site visit on April 22, 2004. We
understand the cleanout is located under the deck and have received pictures verifying the location. These
pictures have been forwarded to the Eagle County Department of Environmental Health. Included is an
updated As -Built Drawing.
The components of the OWS design were installed in conformance with our plans and specifications. Final
electrical connections must be made by a licensed electrician. The construction foreman may activate the
pumping system at his discretion.

As requested, we have designed an onsite wastewater system (OWS) for the proposed residence. The
subsurface investigation was performed by Hepworth-Pawlak Geotechnical, Inc. on September 28, 2001.
SITE CONDITIONS
The property is in a rural area in Eagle County, where OWS are required. The proposed drain field area has
a 10% slope to the south-southeast and a dense cover of native grasses and sagebrush. The residence will be
served by the local water district.
PROPOSED CONSTRUCTION
A four -bedroom residence with a living area of 3000 square feet (SF) is proposed as presented on Figure 1.
The OWS was sized based on 4 bedrooms and 600 Gallons Per Day (GPD) using Eagle County Environ-
mental Health Department loading of 75 gallons per day per person. The sewage loading with a peaking
factor of 1.5 is 900 GPD. This loading includes washing machines and garbage grinders.
SUBSURFACE CONDITIONS
H.P. Geotech performed a subsurface investigation by digging one profile pit and three percolation holes, at
the locations indicated on Figure 1. Subsurface materials consisted of 6 inches of topsoil, underlain by sandy
clay to 4 feet, underlain by sandy silt with basalt fragments and boulders to the maximum depth explored of
8 feet. No ground water was encountered. Percolation tests indicate percolation rates of 20 to 60 minutes
per inch (MPI). The average percolation rate is 42 MPI. Percolation data is presented as Appendix A.
RECOAUdENDATIONS
The installation must include a septic tank and a mounded chamber -type drain field installed at the locations
presented on Figure 1. The mounding is based on Eagle County wanting 4-feet to the basalt boulders. We C 0-6

have to be authorized by Eagle County. The OWS design is based on an average sewage load f 6
and a percolation rate of 42 MPI. The installation must include one 1500-gallon, two-compartm , ecast
concrete septic tank with a pump in a Biotub-im vault, and four mounded chamber wrenches, each with 1
row of 10 chambers. Six inches of topsoil must be removed beneath trench area and replaced with six inches
of imported sand filter material. The topsoil should be stockpiled and replaced on top of mound to promote

re -vegetation. A 1.25-inch PVC line must be suspended- within chambers with 1/8-inch holes drilled on
three-foot centers, with holes upward and every fifth hole facing downward. A pump with a total flow rate
of 36.6 gallons per minute and total dynamic head of 20.2 feet is required to pressurize the drain field. Pump
specifications are included as Figure 4. p r,
Vehicle movement over the field area during construction should be kept at a minimum to reduce
compaction of the soils. The trench area should be scarified prior to installation of sand and chambers.
The surface of the field should be seeded after installation of the trenches. A native seed mix should be sown
at the trench area to revegetate the surface. These mixes do not require irrigation and develop a growth 10
to 15 inches high. No automatic sprinkler system should be installed over the fields.
OPERATION AND MAINTENANCE
The owner must realize an OWS is different from public sewer service. The owner must be aware of
and assume responsibility for maintenance of the system. In the absence of a monitoring program, the
septic tank should be pumped every two years. Effluent filter and screened vaults, as applicable, .are to
be cleaned during pumping. There are daily considerations such as not putting plastic or other nonbio-
degradable materials into the septic system. Garbage disposal use should be minimized.
The backwash of a water softener may be harmful to the OWS. A separate drywell should be constructed
for the backwash waste, if a softener is installed. Chemically treated water from a swimming pool or spas
should not be introduced to the OWS.
LIMITATIONS
Our investigation, layout, design and recommendations are based on data submitted. If subsurface condi-
tions considerably different from those described in this report are encountered, we should be notified to
evaluate the effect on the proposed OWS. If the health department makes modifications to this design, we
should be contacted to evaluate the impact on the performance of the OWS. The county and our firm must
be contacted upon completion of the OWS --- -�o
All construction is to be in accordance with the ISDS regulations. Pipe type and size, burial requirements,
septic tank construction, a -Lid other specifications, which are not depicted in our report, are to conform to
the requirements of the ISDS regulations. The installer of the system is to have demonstrated knowledge of
the Eagle County Environmental Health Department ISDS regulations and requirements.

recommend a minimain slope of 12 inches- in the first 10 feet in unpaged,
areas and a minimum slope of 3 inches in the first 10 feet in pavement
and walkway areas. A swale may be needed 'uphill to direct surface
runoff around the residence..
4) Roof downspouts and drains should discharge well beyond the limits of
all backfill.
S) Landscaping which requires regular heavy. irrigation should be located at
least 5 feet from the building. Consideration should be given to the use
of xeriscape to limit potential wetting due to irrigation-.
Percolation Testing.-.- Percolation. tests were. conducted_ on -September 21,.2001 to
evaluate the feasibility of an infiltration septic disposal system at the site. One profile
pit and three percolation holes were dug at the locations shown on Fig. 1. The test
holes (nominal 12 inch diameter by 12 inch deep) were hand dug at the bottom of
shallow backhoe pits and were soaked with water one day prior to testing. The soils
exposed in the percolation- holes are- similar to- those exposed- irk Pit 2 excavated near the
septic disposal area and shown on Fig. 2 and consist of 1/2 foot of topsoil and 31h feet of
stiff to very stiff sandy clay overlying relatively dense basalt fragments up to boulder
size in a calcareous sandy gilt matrix to the pit depth of S feet. The percolation test
results are presented in Table H. The percolation test results indicate an infiltration rate
between 20 and 60 minutes per inch with an average of 42 minutes per inch.
Limitations: This stud} has been conducted in accordance with gene -ally accepted
geotechnical engineering principles and practices in this area at this time. We make no
warranty either expressed or implied. The conclusions and recommendations submitted
in this report are based upon the -data. obtained- from the, exploratory pits excavated at the
locations indicated on Fig. 1, the proposed type of construction and our experience in
the area. Our findings include interpolation and extrapolation of the subsurface
conditions identified at the exploratory pits. and variations in the subsurface conditions
may not become evident until excavation is performed. if conditions encountered
during construction appear different from those described in this report, we should- be
notified at once so re-evaluation of the recommendations may be made.
This report -has been -prepared for the -exclusive use by our client for design purposes_
We are not responsible for technical interpretations by others of our information. As
the project evolves, we should provide continued consultation and field services during
construction to review and monitor the implementation of our recommendations,. and to

Per discussion with Laura Fawcett, we are providing the attached addendum of our December 11, 2001
OWS Design. Eagle County has requested the OWS be sized on long term acceptance rates (LTAR) per
the EPA Design Manual.
Based on the EPA Design Manual the LTAR for a 42 MPI percolation rate is 0.6 gallons per square foot
per day. Using the LTAR, a minirrlum of 48 chambers is requirzd. Instead of four trenches with 10
chambers, the installation will require four trenches, each with 12 chambers, as presented in Revised Figures
1 and 2_ The trenches must be mounded, pressure distributed, and have 6 inches of imported sand filter
material Alt other recommendations in our report dated November 11, 2001 must be followed..
