As requested, we observed the installation of the onsite wastewater system for the subject site on August
7 and August 12, 2002. The system was designed under our Job No. 13498W, dated April 20, 2001 and
revised May 6, 2002.
The system includes a 2000-gallon, two -compartment, precast concrete septic tank with an effluent filter
on the outlet, and five chamber -trenches with a concrete distribution box and serial overflow distribution.
Three trenches had 1 row of sixteen chambers each, one trench had 1 row of 15 chambers, and one trench
had 1 row of 10 chambers for a total of 73 chambers and of 1131.5 square feet of infiltrative area. An as -
built drawing is included as Figure 1.
The components of the OWS were installed in conformance with our plans and specifications.

prepared a revised onsite wastewater system (OWS) for the proposed residence. The revision is based on a
change in the drain fend configuration frorn our original design, dated April 20, 2001. The percolation test
was performed on April 16, 2001. Our original design should be. discarded to avoid confusion.

The lot is located in a residential area in the subdivision of Cordillera, in Eagle County, Colorado. The
proposed dmLn fold area has a -moderate- to steep slope to the north. 'i hm are scattered aspen and
evergreen trees in the proposed field area.
r—ROPOSED COINST"RUCTION
A six-hedroom_ residence is proposed at the location presented on Figure 1. In accordance with Individual
Sewage Disposal System (IMS) reguMons; the average wwage loaIng for a to residence is 900

F Geotech performed a subsurface investigation by digging one profile pit, at the location indicated on
Figure 1. Subsurface materials reported consisted of 2.5 feet of topsoil, underiann by sand. and ro&

prevent compaction of the receiving soils. We recommend the bottom of each trench be scarified with
a pick, shovel, or packhoe bucket teeth prior to the addition of dispersal gr .dell.
OPERATION AND ALUV MANCE
The distribution device (Flow Splitter Basin or distribution box) must be observed at end of one year to
confzrr_n or assure even disthbution. The owner must realize an OWS is different ?torn public sewer service,
and must assume the responsibility for maintenance of the OWS. The system is relatively maintenance free,
but the over trust have the septic tams pumped. In the absence of a monitoring program, we rfonimend
;.optic tank be pumped and the effluent filter cleaned every two years. There are daily considerations,
such as not putting plastic or other nonbiodegradabie material into the OWS. rater use must be monitored
so toilets are not allowed to nut when seals malfunc-tiori. To illustrate the point;. a running toilet can
consume in excess of 1000 GPD. A loading of 1000 GILD could flood and irreparably harm the OWS.

be constricted for the backwash waste, if a softener is installed: Chemically treated water from a swimming
poor or spas should not be introduced to the O "J.
An OWS design requires installation'by a contractor who is experienced in its installation. Our imr-estig-atimn,
layout, design and reeomtuendations are based on data submitted. If subsurface conditions considerably
different from those described m tills report are encountered; we should be notified to evaluate the effect on
the proposed OWS: Vthe health department makes modifications to this desiQ,xi, we should be contacted to
evaluate the impact on the perffinTtarice of"the OWS.
All construction is to be in accordance with the ISDS regulations. Pipe type and size, burial requirements,
septic tank constniction, and other specifications, which are not depicted in our report, are to conform to
the requirements of the ISDS regulations. The installer of the system is to have demonstrated knowledge of
the Eagle County ISDS regulations and requirements.

Dear Tim:
I spoke with Ed Church about this OWS design a few weeks ago on the phone. I gave him
the following design concerns at that time:
The system design from Church & Associates is based upon 900 gpd which is the
maximum loading rate for a 4-bedroom house. The maximum daily loading rate for a
six -bedroom house according to Eagle County ISDS regulations and State of Colorado
guidelines is 1350 gpd. Assuming the HP average perc rate of 28 min/inch is accurate,
the LTAR is 0.6 gal/sq.ft/day. Therefore, 1350/0.6 = 2250 sq. ft. 2250sq/ft/31 = 73
infiltrator chambers. The Church design specifies 48 chambers for a trench
configuration.
2. Split flow devices and/or distribution boxes are not recommended by Eagle County ISDS
Regulations. However, they may be acceptable to resolve a site specific condition; i.e.,
isolation distances, grade elevations, soil types, etc. The Hlavacek property does not
appear to have site conditions that require the use of a split flow device.
Upon further review I also have concern with the profile hole and perc tests performed by HP
GeoTech in 1998. Eagle County prefers to observe a profile hole and recommends that Church
& Associates perform a perc test in accordance with procedures outlined in a 2001 letter to HP
GeoTech which is enclosed for your reference or by using another professionally recognized
methodology. The percolation test can be conducted by Eagle County EH for conventional
systems after adequate site preparation. Please call and schedule an appointment at least 48
hours in advance.

Per your request this letter shall serve to clarify Eagle County's instructions to engineers performing
percolation and soil profile test holes on properties intending to design individual sewage disposal
systems. The following specifications are located in the Eagle County How To Obtain an Individual
Sewage Disposal Permit (ISDS) handout included with each application form:
• Soil Profile Hole: The 8 feet soil profile examination should be located in an area directly.
adjacent to the proposed absorption area. Soil profiles within a proposed
absorption field are discouraged due to changes in soil texture. If your
absorption area is located on a slope, dig your soil profile hole down
slope of the proposed area.
• Pere test holes: Backhoe cuts impact the upper natural soil strata leaving depressions
within the absorption field area. The percolation test holes should be dug
with a post hole digger or spade. The dimensions of the holes should be
approximately six (6) inches in diameter and three (3) feet deep. The
holes cannot be more than twelve (12) inches in diameter. Accepted
percolation test methodologies can be found in many technical
publications, including the EPA design manual. This office would be
happy to provide this information, if necessary.
This department would actually prefer engineering firms replace the traditional percolation tests results
with a soil profile hole report that includes a long term soil acceptance rate (LTAR) recommendation. A
representative from this department will observe the soil profile hole during the required site evaluation
of the property.
