2. THIS PERMIT IS VALID ONLY FOR CONNECTION TO STRUCTURES WHICH HAVE FULLY COMPLIED WITH COUNTY ZONING AND BUILDING
REQUIREMENTS. CONNECTION TO, OR USE WITH ANY DWELLING OR STRUCTURE NOT APPROVED BY THE ZONING AND BUILDING DEPARTMENTS SHALL

RE: Issuance of Individual Sewage Disposal System Permit No.2134-01. Tax Parcel
# 2111-201-01-010. Property Location: 661 Amhearst Drive, Gypsum, CO.,
Brode residence.
Enclosed is your ISDS Permit No. 2134-01 and a field copy of the engineer design stipulated on
the permit. It is valid for 120 days, or for the duration of your current building permit for this
property. The enclosed copy of the permit must be posted at the installation site. Any changes
in plans or specifications invalidates the permit unless otherwise approved. Please note any
special requirements that may have been added to the design by this department.
Systems designed by a Registered Professional Engineer must be certified by the Engineer
indicating that the system was installed as specified. Eagle County Environmental Health
must also view the installation prior to back filling. Please call well in advance for your
final inspection. Your TCO will not be issued until our office receives this certification
from the engineer, and views the installation.
Due to the onset of inclement weather., Eagle County will not perform final
inspections after December 1, 2001. All field work will resume, weather
permitting, on March 15, 2002.
Permit specifications are minimum requirements only, and should be brought to the property
owner's attention.
This permit does not indicate conformance with other Eagle County requirements. Please
notify this office if you have not been contracted to perform this installation.

At your request, we visited the construction site on Lot 27, The Coyote Ranch, Gypsum
Eagle Couaty, Colorado. The purpose of our site visit was to inspect the installation of the septic
system.
They installed the system in overall compliance witb the septic system design, Drawing number
01152SD_DWG, dated.fanuaty 04, 2002. They installed a 1500-gallon, two compartments, a precast,
concrete septic tanks, as shown on the attached sketch. A cleanout was installed between the house and
the septic tatak.
The leachfield was constructed on the west side of the natural gully. An additional profile hole
and a percolation test were done in the new leachfield area. The percolation test results and the log of the
profile hole area attached to this letter. Sixty-five, Standard Infiltrator Chambers were installed in
trenches, as shown on the attached as -built sketch. The first two trenches had twenty-two infiltrators
separated by six feet with a serial distribution after the first sixteen infiltrators. The third trench also bad
sixteen infiltrators on the south end of the trench, and five infiltrators after the serial distribution.
Inspection, ports were installed in the first and last infiltrator of each of the sections with 16 infiltrators,
and in the last acid the third an:61trator from the north end in the sections with five and six infiltrators. All
piping was 4-inch SDR 3.5 PVC.
