permit is valid for 120 days or for the duration of your current building permit for this property.
The enclosed copy of the permit must be posted at the installation site. Any changes in plans or
specifications invalidate the permit unless otherwise approved. Please note any special
requirements that may have been added to the design by this Department.
Systems that are designed by a Registered Professional Engineer must be certified by the
engineer indicating that the system was installed as specified. Eagle County Environmental
Health must also view the installation prior to back filling. Please call well in advance for
your final inspection. Your TCO will not be issued until our office receives this certification
from the engineer and views the installation.
Please note that our ISDS Permit specifications are typically minimum requirements only and
should be brought to the property owner's attention in case they may wish to install a system capable
of handling larger wastewater flows.
This permit does not indicate conformance with any other Eagle County requirements. Please

Dear Licensed Systems Contractor,
Please find enclosed ISDS permit number 2348-04 to conduct the construction exactly as indicated on the
permit. It remains valid for 120 days or for the duration of a current building permit. The enclosed copy
of the permit must be posted at the installation site. Any changes in plans or specifications must be pre -
approved by this department. If you are receiving this permit and have not been contracted to perform the
installation, please contact us immediately.

Also enclosed is a final inspection completeness check list to assist you in making sure all of the detailed
points of an inspection have been properly addressed before calling for your final inspection. It is
important to pay close attention to the special conditions which may have been included to address site
specific issues. All components of the system must be exposed for our inspection. Projects that require
multiple inspections will be assessed a re -inspection fee of $55.00 payable before or at the time of
inspection. It is extremely important to remember to rake all infiltrative surfaces and avoid installing
systems in wet or frozen ground.
In addition to avoiding installations during unfavorable site conditions, please be advised of our annual
time frame for ISDS installations, unless otherwise specifically approved by this department. Permits
issued on or before November 15 must be installed prior to December 1. Spring field activities typically
resume around mid March. Avoiding the winter months promotes better site evaluations, designs and
installations resulting in the best private wastewater system for our customers. If unfavorable conditions
prevail between the installation and the final inspection (such as snow) special precautions must be taken
to protect the drain field area from the elements.
Please keep this office informed regarding the progress of your installation so we can provide responsive
customer service when scheduling your final inspection. The Temporary Occupancy Permit (TCO)
associated with the building permit cannot be issued without the septic system being finally inspected.
Systems designed by a registered professional engineer must be inspected and certified by the design
engineer as well as Eagle County Environmental Health.

This letter is to inform you that the above referenced ISDS Permit has been inspected and
finalized. Your septic system has been sized to accommodate five (5) bedrooms. Enclosed is a
copy to retain for your records along with a brochure regarding the care of your septic system.
Additional information about the maintenance of your septic system as well as information about
private wells can be accessed through our web site links provided in the Environmental Health

Be aware that changes in the use of your property or alterations of your building may require
commensurate changes to, or relocation of, your septic system. Landscape features, trees with
tap roots, irrigation systems and parking areas above the drain field can cause premature system
failure. It is equally important that you notice and immediately repair dripping faucets and
hissing toilets as this will certainly cause the system to fail. By following a few simple
guidelines, like avoiding grease or other additives down the drain and making sure your septic
tank is on a pumping schedule, you system has been designed to last for many years.

As requested, we observed the installation of the onsite wastewater system for the subject site on
August 6, 2005 and August 13, 2005. The system was designed under our Job No. 16592, dated
August 2, 2004 and revisions of June 1, 2005 and August 9, 2005.
The system includes the installation of a 2000-gallon, two -compartment, precast concrete septic tank
with a Biotube Pump Vault and a 1200 SF semi -mounded sand filter drain field. Two feet of concrete
sand filter material was installed. There is pressurized distribution of effluent to the sand mound. An
as -built drawing is included as Figure 1.
The components of the OWS were installed in general conformance with plans and specifications. Our
installation observations do not imply a guarantee or warranty of materials or workmanship.

As requested, we have investigated subsurface conditions and designed an onsite wastewater system
(OWS) for the proposed residence. The subsurface investigation was performed on July 21, 2004.
SITE CONDITIONS
The lot is located in a rural residential area in Eagle County, Colorado, where OWS are required. The
proposed drain field area has a 20% slope to the southwest. There is a moderate cover of native grasses,
sagebrush, scrub oak, and basalt boulders. Water is provided to the site by an onsite well located greater
than 114 feet north of the proposed drain field area.
PROPOSED CONSTRUCTION
A five -bedroom residence is proposed at the location presented on Figure 1. The OWS was sized based on
5 bedrooms and 750 gallons per day (GPD) using Eagle County Department of Environmental Health
(ECDEH) loading of 75 gallons per person per day. The sewage loading with a peaking factor of 1.5
is 1125 GPD. The loading includes washing machines and garbage grinders.
If additional bedrooms in unfinished areas are possible; it is suggested that the OWS be constructed to
handle the additional loading. The installation of a properly engineered OWS to serve future build -out can
be cost effective.
SUBSURFACE CONDITIONS
The subsurface investigation was performed by digging four percolation holes, 23 to 51 inches deep, and
one profile pit at the locations indicated on Figure 1. Subsurface materials in the profile pit consisted of 1
foot of sandy topsoil underlain by silty, sand, with basalt cobbles and boulders to the maximum depth

explored of 9.5 feet. No ground water or bedrock was encountered. Percolation tests indicate percolation
rates of 11 to 16 minutes per inch (MPI). The average percolation rate is 15 NIPI, which indicates a Long -
Term Acceptance Rate (LTAR) of 0.80 gallons per day per square foot (gal/day/ft). Percolation data is
presented in Table 1.
RECOMMENDATIONS
The installation must include a septic. tank and a gravelless chamber -trench drain field, at the locations
presented on Figure 1. ECDEH regulations require the drain field area to be sized using the LTAR of
the receiving soils. The OWS design is based on an average sewage load of 750 GPD and an LTAR of
0.80 gal/day/ft2. The installation must include a 1500-gallon, two -compartment, precast concrete septic
tank, with an effluent filter on the outlet and a total of four chamber -trenches using serial overflow
distribution. Each trench must have one row of 12 chambers for a total of 48 chambers and 744 ft2 of
infiltrative area. Trenches can be modified to fit topography and preserve vegetation. Details of the
design are presented on Figures 1 through 3.
Movement over the drain field trench during construction should be kept at a minimum to prevent
compaction of the receiving soils. The bottom of each trench must be scarified with a pick, shovel, or
backhoe bucket teeth prior to the addition of chambers.
OPERATION AND MAINTENANCE
The owner must realize an OWS is different from public sewer service and must assume the responsibility
for maintenance of the OWS. The system is low maintenance, but the owner must have the septic tanks
pumped. In the absence of a monitoring program, the septic tank should be pumped and the effluent filter
cleaned every two years. There are daily considerations, such as not putting plastic or other nonbiode-
gradable material into the OWS. Water use must be monitored so toilets are not allowed to run when seals
malfunction. To illustrate the point, a running toilet can consume in excess of 1000 GPD. A loading of 1000
GPD could flood and irreparably harm the OWS.
The loading from the backwash of a water softener may be harmful to the OWS. A separate drywell should
be constructed for the backwash waste, if a softener is installed. Chemically treated water from a swimming
pool or spas should not be introduced to the OWS.

OWS designs require installation by a licensed contractor. Our investigation, layout, design and recom-
mendations are based on data submitted. If conditions different from those described in this report are
encountered, we should be notified to evaluate the effect on the proposed OWS. If the health department
modifies this design, we should be contacted to evaluate the impact on the performance of the OWS.
All construction is to be in accordance with the ISDS regulations. Pipe type and size, burial requirements,
septic tank construction, and other specifications, which are not depicted in our report, are to conform to
the requirements of the ISDS regulations. The installer of the system is to have demonstrated knowledge of
the Eagle County ISDS regulations and requirements.

The onsite wastewater system (OWS) design for the subject property has beenrevised due to the presence
of large boulders encountered during excavation ofthe drain field. The originaldesign was presented in a
report dated. August 2, 2004 under our Job No. 1.6592.
RECOMMENDATIONS
The OWS should be installed as indicated on. the revised Figures 1. and 2. The revised OWS designincludes
an additional. 500-gallon septic tank: with a BiotubeTM pump -vault, followed by a I2-foot by 100-foot
"pressurized -mounded" drain field :For a total absorptive area of 120W. The drain field will require
imported sand filter material as indicated or, the revised Figure 3. The installation. for the residence must
include one 1500-gallon, two -compartment, precast concrete septic tank followed by one 500-gallon,
precast concrete septic tank with an effluent pump in a Biotubdm pump vault. The pump curve is
included with this report.
The surface ofthe mounded drain field should be seeded after installation ofthe system. A native seed mix
from the area available at seed stores should be used. These mixes do not require irrigationand develop a
growth. 1.0 to 15 inches .high. No automatic irrigation system should be installed over.the field_

The engineer redesign for the above mentioned wastewater system (OWS) has been
received by this department for review and permitting. The new design is being
submitted to overcome the presence of large boulders encountered during excavation of
the conventional system permitted last fall. The new design is a pressurized, sand mound
drain field of 100' x 12', providing 1,200 sq.' of absorption area.
In our opinion, the extent of the large boulders present in the geology has not been clearly
described to assure the design has enough basal area and is adequately separated from the
large boulders encountered if they were characterized as bedrock. We notice a
percolation rate of 15 minutes per inch has been used for basal area calculation resulting
in the basal area being the same dimension as the mound. Typically, the basal area is a
larger footprint which contemplates wastewater will be dispersed laterally from the sand
mound in all directions with a toe extending down the slope. Furthermore, if the
subsurface consisted of bedrock, the acceptance rate of wastewater into this strata would
define the necessary basal area and the nature of the bedrock would need to be evaluated
to assure wastewater would continue to flow through fractures or mitigation may be
necessary to avoid day -lighting.
We ask that you provide additional information regarding the extent as well as depth of
the large boulders and the ability of the subsoil to accept wastewater at a conservative
long-term rate. We also ask that more detail be included in the top and end views
showing the basal area extending away from the mound and further down hill based on
the evaluation of the acceptance rate of the subsoil. It would be helpful to the installer to
also have piping details and sequential procedures to follow for proper construction. You
may want to double check the pump curve as it appears the pump specification

calculations include overcoming 15 feet of head. The site plan appears to indicate a drop
in elevation from the dose chamber rather than pumping up hill. Will the system receive
several doses per day based on actual anticipated wastewater flows or are the floats set to
dose larger volumes every day or two?

This letter is in response to the June 16, 2005 Eagle County letter with respect to the subject site requesting
information presented in our letter or June 1, 2005. The June 16 letter requests information on: 1)
subsurface conditions concerning the nature of the boulders and percolation rate, 2) the basal infiltration
rate and 3) the lift on the pump curve chart. Our office received the June 16 letter, today August 9.
During construction, the installer "Dig It" able to excavate to a depth of 4 feet at the east end of the field as
indicated on the attached photo and did not encounter refusal. It is believed that with the excavation
equipment used, the field could be excavated to the depth of 4 feet, which indicates the boulders were not
bedrock, but boulders that are porous. The west end of the field was dug only to 1.5 feet. The depth was
controlled at keeping the excavation level, not refusal due to bedrock. It is believed that the percolation
rate of 15 MPI is valid.
The vertical nature of the sand filter is due to the percolation rate of 15 MPI of the basal material requires
less area than the application rate to the sand filter material. This would result in a possible smaller area for
the basal area than the top of the mound, which we would not do, so the sands of the mound were made
vertical. Because of pressure distribution there should be no, to little lateral migration. With the use of the
sand filter material, treatment is assured, which would remove nutrient so that clogging would not occur at
the basal area. The long-term acceptance rate of 0.80 is very conservative and not believed to be valid
considering the sand filter treatment and pressure dosing. The 3:1 slopes recommended for cover of the
mound in the design is valid.
With respect to the pump curve, an additional pump curve is attached that has a lift of 5 feet instead of 15
feet. The same pump is specified. The 15 feet was not correct. It is recommended that the floats be set for
100 gallons per dose, which should be 1 to 3 doses per day for a typical family.
Per telephone discussions on June 29th and August 1 with Ray Merry and Victor Crocco, respectively, the
subsurface conditions of no refusal to 4 feet were described and it was agreed that 2 feet of sand was
correct rather than 4 feet of sand. Without being aware of the June 16 letter, 2 feet of sand was installed.

Re: Summary of sequence of events
The original permit to serve this dwelling was a conventional, gravity fed series of
shallow trenches designed by E.0 Church & Associates and the permit was issued in
August of 2004. During the excavation, the presence of large cobbles or boulders
necessitated the engineer to provide mitigation measures by proposing 6" of sand be
placed on the trench bottoms. Since the systems was not installed in 2004, this office
followed up with the owner to verify the project was moving forward with the same
Licensed Systems Contractor (Dig It) to assure a current 2005 license and that the
engineer was still under contract to conduct necessary inspections. The permit was re-
issued to Dig It in May 2005 to allow them to proceed and E.O. Church confirmed the
contract was still valid.
During the installation of the permitted, conventional, shallow trench system with a layer
of sand in late May or early June 2005, large boulders or bedrock prevented the
installation of this system and the engineer needed to submit another design for our
review. A sand filter design was faxed to us on or about June 2, 2005 for our review and
re -permitting. A letter was sent to the engineer from this office on June 16, 2005 with
requests for further information. On or about August 8, 2005 a request for a final
inspection was called in to this office by the installer who installed the re -designed, sand
filter system without an amended permit. A follow up document was faxed to this office
on August 9, 2005 by the engineer addressing the comments outlined in the June 16,
2005 letter. The permit was re -issued after -the -fact and faxed to the installer on August
10, 2005 and an inspection scheduled for August 11, 2005. The installer was notified to
have the engineer certify the system.

As requested, we observed the installation, of the onsite Wastewater system for the subject site on
August 6, 2005 and August 13, 2005. The system was designed under our Job No. 1.6592, dated
August 2, 2004 and revisions of June 1, 2005 and August 9, 2005.
The system includes the installation of 2000-gallon, two -compartment, precast concrete septic tank
with. a Biotube Pump Vault and a 1200 SF semi -mounded sand filter drain field. Two feet of concrete
sand filter material was installed_ There is pressurized distribution of effluent to the sand mound_ An
as -built drawing is included as Figure 1.
The components ofthe OWS were installed in general conformance withplans and installation observations do not imply a guarantee or warranty, of materials or specifications. Our
