GRANT AGREEMENT BETWEEN
EAGLE COUNTY, COLORADO
AND
MY FUTURE PATHWAYS
THIS GRANT AGREEMENT (“Agreement”) is effective as _______________ by and between
My Future Pathways, a Colorado nonprofit corporation (hereinafter “Grant Recipient”) and Eagle
County, Colorado, a body corporate and politic (hereinafter “Grantor” or the “County”) through
its Department of Public Health (“ECPHE”).
RECITALS
WHEREAS, the County works to promote the health, safety, and welfare of Eagle County citizens
of all ages; and
WHEREAS, ECPHE has identified a significant community need for funding and services
dedicated to the ‘promotion’ phase of care, which addresses root causes of local health disparities,
which lead to a myriad of individual high-risk behaviors; and
WHEREAS, the observed and reported fragmentation of community organizations and non-
profits leads to a lack of strategy in services and programming across systems of care; and
WHEREAS, ECPHE would like to extend staff knowledge and experience to offer organizational
support while administering funds with the intent of infusing equity and inclusion from a systemic
level; and
WHEREAS, among the services Eagle County provides in order to promote such health, safety,
and welfare are grants-in-aid to local organizations whose work assists the County in the
accomplishment of the goal to create and sustain a safe, healthy, environmentally and
economically vital and socially diverse community; and
WHEREAS, the County introduced the 2023 Pilot Advancing Systems Change Grants (the “ASC
Grants”), which intend to fulfill capacity-building needs that support selected grant recipients’
projects which lead to expanding the ability to advance efforts to address root causes, enhance
inter-agency partnerships and collaboration, and incorporate community-based decision-making
in operations; and
WHEREAS, the County issued a Request for Proposals on June 26, 2023 to determine appropriate
entities to receive the ASC Grants. Grant Recipient submitted a proposal and was selected on July
31, 2023 as an appropriate entity to receive the Grant. Grant Recipient’s proposal is attached
hereto as Exhibit A and incorporated herein by this reference; and
WHEREAS, Grant Recipient has agreed to provide services that create and sustain a healthy
environmentally and economically vital and socially diverse community in Eagle County; and

WHEREAS, this Agreement shall govern the relationship between Grant Recipient and County
in connection with the Services.
GRANT AGREEMENT
Now, therefore, in consideration of the foregoing premises and the following promises, Eagle
County and Grant Recipient enter into this Agreement for payment of the grant award in the
amount of sixty-nine thousand, six hundred and fifteen dollars ($69,615) (the “Grant”) to Grant
Recipient.
A. Grant Recipient agrees that funds awarded by County may be used and expended solely to
to build the internal capacity necessary for equitable and secure data collection by providing My
Future Pathways staff with adequate technology necessary for operations, thus enhancing the
security of personal identifying information of clients, collecting more accurate data to
representative programming, and enhancing the impact of collaborations with community partners
as specified in its 2023 Community Service Grant Application and Grant Application Budget
Section. All funds will be expended in the specified grant period as outlined in the Scope of Work
(the “Grant Period”), which is attached hereto and incorporated herein as Exhibit B. The Grant
Award set forth herein is for the specific Grant period and the Grantor makes no representations,
nor should Grant Recipient rely on this award as a representation of any future awards for other
grant periods.
B. In return for the award of the Grant, Grant Recipient agrees to submit required
documentation discussing how the funds were expended, deliverables as outlined in the Scope of
Work, and how they tie back to creating and sustaining a healthy environmentally and
economically vital and socially diverse community.
C. Payment of the Grant will be made in two installments, with 70% of funds paid at upon
execution of this Agreement, and the final 30% paid in December of 2024 following the County’s
satisfactory review of deliverables as outlined in the Scope of Work.
D. Grant funds under this Grant Agreement are to be used and expended within the Grant
Period. Any funds not used or expended during the Grant Period must be returned to Grantor.
E. If, after payment of the Grant Award, Eagle County reasonably determines that payment
was improper because the purposes for which the payment was made were misrepresented, or the
terms of this Agreement were breached, then upon written notice of such determination and request
for reimbursement from Eagle County, Grant Recipient shall forthwith return such payment to
Eagle County.
F. Grant Recipient shall comply at all times and in all respects with all applicable federal,
state and local laws, resolutions and codes; and specifically, with the requirements of the Civil
Rights Act of 1964 and Section 504, Rehabilitation Act of 1973, concerning discrimination on the
basis of race, color, sex, age, religion, political beliefs, national origin or handicap.

G. Grant Recipient shall not assign any of its rights or duties under this Grant Agreement to a
third party without the prior written consent of the Grantor. The Grantor shall terminate this Grant
Agreement in the event of any assignment without its prior written consent, in which case, any
unused Grant Funds must be returned to Grantor.
H. Grant Recipient agrees that no agent, employee or volunteer of Grant Recipient shall be
deemed an agent, employee or volunteer of County.
I. Grant Recipient shall maintain, for a minimum of three (3) years, adequate financial and
programmatic records for reporting to Grantor on performance of its responsibilities hereunder.
Grant Recipient shall be subject to financial audit by federal, state or county auditors or their
designees. Grant Recipient authorizes Grantor to perform audits or to make inspections during
normal business hours, upon 48 hour notice to Grant Recipient for the purpose of evaluating
performance under this Grant Agreement. Grant Recipient shall cooperate fully with authorized
Eagle County representatives in the observation and evaluation of the program and records. Grant
Recipient shall have the right to dispute any claims of misuse of funds and seek an amicable
resolution with Grantor.
J. This Agreement shall be governed by the laws of the State of Colorado. Jurisdiction and
venue for any suit, right or cause of action arising under, or in connection with this Agreement
shall be exclusive in Eagle County, Colorado.
K. This Grant Agreement supersedes all previous communications, negotiations, and/or
agreements between the respective parties hereto, either verbal, or written, and the same not
expressly contained herein are hereby withdrawn and annulled.
L. This Grant Agreement does not, and shall not be deemed or construed to confer upon or
grant to any third party or parties any right to claim damages or to bring any suit, action or other
proceeding against either Grant Recipient or Grantor because of any breach hereof or because of
any of the terms, covenants, agreements and conditions contained herein.
M. Invalidity or unenforceability of any provision of this Grant Agreement shall not affect the
other provisions hereof, and this Grant Agreement shall be construed as if such invalid or
unenforceable provision was omitted.
N. To the extent required to perform under this Grant Agreement, Grant Recipient represents
and warrants that it has the expertise and personnel necessary to properly perform the Grant
Agreement and its professional personnel are duly licensed to perform.
O. No failure or delay by either party in the exercise of any right hereunder shall constitute a
waiver thereof. No waiver of any breach shall be deemed a waiver of any preceding or
succeeding breach.
P. The signatories to this Grant Agreement aver to their knowledge, no employee of the
County has any personal or beneficial interest whatsoever in the matters described in this Grant
Agreement. The Grant Recipient has no beneficial interest, direct or indirect, that would conflict

in any manner or degree with the performance of the Grant Agreement and Grant Recipient shall
not employ any person having such known interests.
Q. Grant Recipient shall be responsible for the completeness and accuracy of its
performance under the Grant Agreement, including all supporting data or other documents
prepared or compiled in performance of the Grant Agreement, and shall correct, at its sole
expense, all significant errors and omissions therein. The fact that the Grantor has accepted or
approved the work performed shall not relieve Grant Recipient of any of its responsibilities.
Grant Recipient shall perform the Grant Agreement in a skillful, professional and competent
manner and in accordance with the standard of care, skill and diligence applicable to Grant
Recipients performing similar services. This paragraph shall survive termination of this Grant
Agreement.
S. Grant Recipient shall carry workers compensation insurance as required by law and
automobile and commercial general liability insurance as necessary and appropriate during the
term of the Grant Agreement, proof of which is attached hereto as Exhibit C.
T. The Grant Recipient shall indemnify and hold harmless Grantor, and any of its officers,
agents and employees against any losses, claims, damages or liabilities for which Grantor may
become subject to insofar as an such losses, claims, damages or liabilities arise out of, directly or
indirectly, this Grant Agreement, or are based upon any performance or nonperformance by
Grant Recipient or any of its sub-recipients hereunder including claims for bodily injury or
personal injury including death, or loss or damage to tangible or intangible property; and Grant
Recipient shall reimburse Grantor for reasonable attorney fees and costs, legal and other
expenses incurred by Grantor in connection with investigating or defending any such loss, claim,
damage, liability or action. This indemnification shall not apply to claims by third parties
against the Grantor to the extent that Grantor is liable to such third party for such claims without
regard to the involvement of the Grant Recipient. This paragraph shall survive expiration or
termination hereof.
U. Grant Recipient certifies that it has read the Grant Agreement, understands each and every
term and the requirements set forth herein, and agrees to comply with the same.

Advancing Systems Change Grant: Pilot
Program Application 2023
Eagle County Public Health & Environment is thrilled to introduce the Pilot 2023 Advancing Systems
Change Grant! Involvement will be highly interactive and adaptable to the grantee's needs, goals, and
limitations, with the intent of fostering a supportive funding relationship. This 9-month grant cycle will
begin September 2023, with an application period beginning Monday, June 26th, 2023, and closing
Friday, July 21st, 2023. A virtual information session will be held virtually on July 6th, 2023 at 4pm to
offer applicants an opportunity to ask questions and learn more detail about the grant. Any additional
submitted questions will be publicly posted with coinciding answers.
This program wishes to fulll capacity-building needs that support applicants’ projects which leads to
expanding ability to advance root cause efforts, enhance inter-agency partnerships & collaboration, and
incorporate community-based decision-making in operations. These capacity-building efforts must be
directly linked to the development or support of projects which serve populations in our community that
experience disadvantages due to inequities in policies, practices, social norms, and the distribution of
resources. It is through dedication to these efforts that gradual system-level change can be
accomplished.
Eligible use of funds include:
Community outreach, engagement, and building efforts
Community member empowerment (training, strengthening connections, advocacy)
Equity-focused professional development - staff and board
Strategic planning and organization development for board (re)development, capacity building,
fundraising, and other services
Recruitment of a Board of Directors or staff that represents the diverse populations of the
community
Projects which address/pursue: Strengthening partnerships with coinciding groups, organizations,
and strategies, strengthening community trust and relationships, and/or enhancing community
involvement in the development of projects and identication of need

Eligibility:
To be considered for funding, organizations must provide information identifying the following:
Registration as a nonprot with the State of Colorado and provide evidence of local programming or
projects which fosters community building and root cause work
Project applied for will directly impact residents of Eagle County, with the applicant’s service area
focused on the Eagle County and/or Roaring Fork Mid-Valley
Familiarity with unique needs, culture, and present services available within Eagle County and/or
Roaring Fork Mid-Valley
Awareness of the organization's role and function within the applicable systems
Requirements:
1. Sign a standard Eagle County Contract, including a Scope of Work
2. Participation in coordination meetings: Minimum of three (3) meetings with ECPHE staff,
attendance in existing applicable collaboration meetings relevant to the proposed
project (ex: youth-focused organizations group convenings)
3. Data repor ting: Mid-grant and post-grant outcome reporting as outlined in repor ting template with
outcomes and indicators identied by grantee, annual report sharing as published in standard format
4. Evaluation: Par ticipation in pre-grant and post-grant evaluation and quality measurement, as
identied by ECPHE staff

The rural resor t community of the Eagle River Valley has 50,000 residents, 29% Hispanic — accounting for
50% of local students, not including many undocumented families. Approximately 34% of our community is
“low-income” (<80% of Area Median Income) without accounting for the exorbitant cost of living, which
creates relatively more low-income families.
In 2018, the graduation rate for Hispanic boys in Eagle County Schools was 56.9% — limiting economic and
social mobility and driving a multi-generational pover ty trap. My Future Pathways began in 2019 to help
~100 students in grades 6th - 12th stay in school and graduate. We have grown twelvefold in less than four
years, serving 1,200 students annually, more than 20% of all Eagle County School District (ECSD) students.
MFP has evolved from a mentoring program into a community-wide powerhouse empowering local families
and students by providing everything from post-secondary scholarships to direct services, professional
tutoring, in-school guidance sessions, parent peer-support programming, summer camp, youth centers,
career pipelines, and more. Our social-emotional engagement model helps local individuals become the
best versions of themselves without sacrificing scarce and hard-won resources.

Since 2019, MFP has grown twelvefold, serving over 20% of ECSD students; 1,200 6th-12th grade kids
annually. MFP’s 2022 high-school graduation rate was 100%, and its post-secondary completion rate was
98%. Our two-generational approach suppor ts children in school and helps parents meet immediate needs.
We provide year-round peer-to-peer mentoring, professional tutoring, social-emotional suppor t, physical and
cultural activities, and two youth centers. Twenty-eight graduates now attend Colorado Mountain College on
MFP scholarships, five more attend our pilot technical training program at Western Colorado Community
College, and thirteen receive Guardian Scholars (our sister program) awards to attend Mesa State
University. Working with Vail Health, we also created a Master’s in Social Work scholarship program to meet
local behavioral health needs. Five students have completed Master’s degrees and are working towards full
LCSW credentials; two more graduated in May 2023, and three new students joined the program.
Our other offerings include Summer Pathways, which has over 50 kids registered in 2023, with many on the
waitlist. Offered at no cost to local families, Summer Pathways provides unique oppor tunities to local
students, Wednesdays through Fridays. We’ve taken trips to the Denver Aquarium and have had multiple
biking trips with the Vail Valley Mountain Trail Alliance (VVMTA). Upcoming excursions include the Denver
Ar t Museum, Water World, and a Rockies game.
MFP is also expanding parent and adult engagement oppor tunities this summer. Star ting with a “Breakfast
Club” on Mondays during the summer months, adults (16 currently registered) meet in the morning for
breakfast (provided by MFP) and peer-support training. On Tuesdays, local parents and adults can play
volleyball from 5-8 pm. We’ve had an excellent turnout for volleyball, with 35 regular par ticipants so far. On
Wednesdays, adults can join the youth for mountain biking, and on Thursdays, they can par ticipate in boxing
classes (also open to all).

My Future Pathways addresses our community's alarming Hispanic student graduation rate — 61.5% overall
and 56.9% for boys in 2017-2018. Immigrant families move to Eagle County for oppor tunities unavailable in
their home countries. However, financial tribulations, language barriers, lack of social capital, and
acculturation stressors raise the oppor tunity cost of education. Low-skilled hourly jobs fill immediate needs,
taking precedence over the unguaranteed, long-term oppor tunities yielded by education, perpetuating a
multi-generational pover ty trap.
In response to ECSD’s quick recognition of our ability to engage with Hispanic students, the schools
requested that we increase our effor ts. MFP responded with new staffing and began in-school
programming, growing to involve over 20% of all county students. Our two-generational approach suppor ts
children in school and helps parents meet immediate needs while providing crucial oppor tunities and social
capital frequently in shor t supply for more vulnerable community members. MFP believes that empowering
students and parents in programming decisions engenders trust and representation.
Students and parents are dynamically engaged in our work; MFP provides peer mentoring and study halls at
the Edwards and Gypsum youth centers, along with additional in-school time — college-aged students
mentor high-school students, who then mentor middle-school students. Par ticipants are also active in
deciding on enrichment activities and youth-center programming. Fur thermore, parents work with adult peer
leaders to determine the classes and activities they wish to pursue in the adult peer groups. Creating girls'
programming and adding summer offerings at the behest of community members demonstrate further
community engagement.
MFP has successfully built a coalition of businesses and non-profits dedicated to helping EC’s most
vulnerable population. It works actively with its key par tners to marshal community resources and address
systemic drivers of local inequality.
My Future Pathways’ board has ten members, containing a mixture of community leaders, including MFP
founder Ron Davis, under the direction of Toby Seay, a local businessman and philanthropist. The board
helps the organization maintain fiscal responsibility and adhere to the organizational mission and vision.

Project Funding Request
Title — A Pen and Paper Only Goes So Far: Empowering MFP staff, students, partners, and EC community
members through technology infrastructure, training, outreach, and responsible data practices.
MFP requests $88,389.76 in Eagle County’s Advancing Systems Change funds to implement and update
tech infrastructure and train staff members to become savvy operators and responsible custodians of
sensitive community Personally Identifiable Information (PII). Award dollars would suppor t equipment
purchases, including tablets and iPhones for all seven full-time staff members (with enough to circulate
among our many par t-timers), as well as extra CommunityPass software licenses, security hardening, and
necessary training in Google Workspace, CommunityPass, and responsible data practices.

My Future Pathways is at an inflection point. Due to intense community needs and overwhelming demand
from students, parents, and the Eagle County School District, My Future Pathways grew twelvefold from 100
students in 2019 to 1,200 students annually — more than 20% of all ECSD students. We must transition from
an organization operated solely on the hear t and sweat of a few dedicated staff and volunteers to a
professional, sustainable organization capable of helping EC students and families long-term. Much of this
will depend on the quality of our systems, including data practices, software, and tech infrastructure. By
shoring up our software, data, and tech infrastructure, we believe that My Future Pathways’ model can be
replicable in other communities beset by educational attrition and unsatisfactory graduation rates for
traditionally underserved youth.
Currently, MFP employees use their personal devices to communicate about organizational matters,
representing an unfair, individually resource-intensive expectation and a security concern. Staff typically
record meeting data manually (on their own devices or with pen and paper), and we have only one staff
member using CommunityPass software to enter the relevant data. Staff also need to be more engaged on
social media, which affects how they can market organizational services and advocate on behalf of MFP.
We want to purchase iPhones and tablets for all full-time and par t-time staff members and train them in
Google Workspace, CommunityPass, and data repor ting practices. Funds will also facilitate security-
hardening.
We desperately need more accurate data and efficient repor ting practices, robust security protocols, and
staff development to handle community PII responsibly and evaluate the strength and relevance of our
programs. Also, having access to high-end MFP hardware would reduce the resource burden on staff and
empower them to take an even more active role in marketing and community advocacy through social
media.
Purpose: Describe the issue and/or opportunity addressed in pursuing this project, including

We’ll purchase our initiative's equipment and software licenses within a month of the award date. The
equipment and service subtotal is $45,839.76. The CommunityPass core software is $6,550.00. We expect
the staff training and security rollouts to take longer, but we should complete both within six months of the
prize. Security hardening and maintenance will be $27,500.00, including our first phishing test. Technical
training classes will be $2,500 for two data and repor ting classes, $2,500 for two Google Workspace
classes, and $3,500 for two CommunityPass classes with a custom T&M configuration. The total outlay for
the project is $88,389.76.
Between months six and nine of the pilot grant, we expect to start measuring MFP program attendance,
evaluating staff engagement, and analyzing the relevance of our programming, with the option to add
additional CommunityPass communications modules.
Our budget is approximately $1.3 million annually. Most of our funds come from foundation star t-up grants
and individual benefactors to seed our programming and expand our reach to more students and families.
Within a shor t time (nine months for the pilot grant), a donation of roughly $88,000 would help us
implement the necessary software and tech infrastructure to take our operations to the next level and
ensure our success for many years.
With attention to our software, data, and tech infrastructure and training our staff with the requisite skills,
we can continue operating at scale and safely expand in the future. The initial EC investment would allow us
to buy all needed equipment, purchase CommunityPass software licenses, improve systems security, and
train all staff members within the duration of the pilot grant. Eagle County’s investment would be unique in
its ability to enhance every other function within the organization, many of which target root causes.
To ensure the sustainability of the tech initiative, MFP will make room in its budget for recurring software
costs, future staff training on an as-needed basis, and equipment upgrades over time. We estimate the
regular expense of the initiative to be about $33,000 annually. We actively pursue grants and donor funding
to shore up operations and fund new projects. We hope to reduce the long-term impact of our recurring tech
costs through programs that suppor t general operating expenses, and we expect to have applied for several
by the end of 2023.

The organization can only continue operating at its current level with significant additional technological
investments. Better data and improved efficiency will allow for more accurate and representative
programming and thoughtful, impactful collaborations with community par tners. Empowering our staff to
be responsible custodians of sensitive community PII will help increase their organizational engagement
level, develop their skill sets, and elevate the care they can provide to the individuals in our programs. An
increased social media presence from direct staff engagement on MFP devices will give staff members
another channel for community outreach, elevating the organizational profile and empowering staff
members to communicate with wider audiences. Over time, we can focus on increasing followers,
improving content creation, and evaluating sentiment. The data initiatives, social media outreach, and staff
training will necessitate hardware purchases.
By the end of the nine-month pilot grant period, we expect to have completed training, completed all
software purchases, begun the security-hardening process, and actively used CommunityPass and Google
Workspace to measure program impact.
My Future Pathways began as a means of helping marginalized first-generation students in school to
provide them with future oppor tunities beyond the pover ty trap of low-skilled service jobs. Safety, belonging,
self-confidence, perseverance, and leadership skills are among the attributes we promote. To measure our
effectiveness towards these effor ts, we track the following:

-The graduation rate of MFP students
-Graduation rates of Hispanic students from Eagle County School District (female, male, combined)
-Number of students attending college or trade schools
-Number of college students continuing with MFP to provide mentoring
-Total number of MFP students

Our process works to identify and dynamically address evolving community needs but could benefit from
improved software, tech infrastructure, and staff training. Through the above metrics, as well as monitoring
individual student attrition as our program matures through our rapid growth phase, we can assess whether
we are maintaining our relevance to students and parents, as well as if we are meeting our ultimate goal of
keeping students in school to provide them with education and career pathways that move them out of
pover ty, reaching their full potential.

Outputs include staff-engagement hours, real-time data entry (by meeting), staff technical course
comprehension, social media outreach, and security competency. We will begin with staff training and
engagement on the newly purchased devices. Courses will cover using CommunityPass, Google Workspace
and understanding responsible data practices. We will track staff attendance and par ticipation during the
learning process by evaluating click-through rates and recording the completion of the required modules.
We will also follow staff presence on social media to increase our marketing footprint and the growth of our
audience. We can scale our content output according to need.
We will increase the number of staff members interfacing with CommunityPass from one to a target of five
to seven, entering meeting information in real-time, promoting data efficiency, and reducing our reliance on
manually recorded metrics and personal devices. To improve security and develop responsible data
practices, we’d also like to train staff to recognize phishing, performing mock attempts every six months.
We will likely hire a consultant to generate a vulnerability repor t and initiate remediation.
Once staff members complete training, we will use CommunityPass to accurately evaluate our
programming par ticipation and assess our offerings' relevance; continuous attendance data (recorded at
meetings on MFP tablets and phones) will give us a more comprehensive picture of which programs
resonate with our population.
MFP can add communications modules in CommunityPass, allowing us to survey community members.
Surveys would add depth to our program design, allowing us to keep our finger on the pulse of stated
community needs while reliably measuring the frequency of those sentiments. Survey feedback would
empower community members to effect change in MFP programming directly. A reliable system for
community outreach and recording input will pay dividends well beyond EC’s initial investment, enabling
MFP to respond to changing needs within the community. Quality data will open the door to impact studies,
more comprehensive analytics, and more efficient par tnerships.

MFP’s tech initiative will empower staff through technical training, improving their concrete capacities while
adjusting to the on-the-go needs of the team (mobile platforms vs. desktops). Increasing the security of our
systems to help protect our vulnerable populations and reduce the resource demand on staff members and
their families. Better data and improved efficiency will allow for more accurate and representative
programming and thoughtful, impactful collaborations with community par tners. Elevating our staff to be
responsible custodians of sensitive community PII will help increase their organizational engagement level,
develop their skill sets, and boost the care they can provide to the individuals in our programs. Also, having
access to high-end MFP hardware would reduce the resource burden on staff and empower them to take an
even more active role in marketing and community advocacy through social media. The sum of the pilot
program pieces will allow MFP to continue vigorously addressing root causes for our most vulnerable
community members over many years to come.
Partnerships and Collaboration
Our key par tners are the Eagle County School District (referral of students, providing student grade &
behavioral data if consented to, reserving school time and space for on-campus mentoring), Eagle Valley
Behavioral Health (funding and behavioral health care for students and staff), Vail Health (Fundraising and
marketing), Mind Springs (LSW provides behavioral health psychoeducation and prevention), Howard Head
Sports Medicine (health and fitness program, and Mountain Pride (LGBTQIA+ training and support). Our
other partners include Your Hope Center, Mind Springs, SOS Outreach, Bright Future Foundation, Eagle
Valley Community Foundation, and Guardian Scholars. These relationships enhance My Future Pathways’
suppor t for underserved youth and families in our community.

MFP has a deep network of existing partnerships, and the tech initiative will allow more meaningful
collaborations. Our improved measurements will be par ticularly useful to ECSD as we have expanded our in-
school programs. Better data will enable MFP to work with local schools to determine which offerings
resonate most with vulnerable students, maximizing our in-school contact hours and helping those kids to
make the most of our mentoring. EVBH, which provides a significant por tion of our funding, must evaluate
the returns on its considerable investments and determine which behavioral health outcomes we can
realistically influence. More targeted work with EVBH around repor ting and analysis could lead to
longitudinal data sets and impact studies, further helping all community par tners in our quest to address
root causes in Eagle County. CommunityPass will open the door to augment partnership efficiency,
empowering more students and their families to seize educational oppor tunities and break the pover ty trap.
Increased staff presence on social media will benefit all par tners by promoting collaborations and direct
community engagement around critical issues.

We can determine which programs resonate with our community members by measuring real-time program
attendance through the CommunityPass software. After adding future comms modules with surveys, we

My Future Pathways staff are bilingual, bicultural community members — mainly Hispanic youth with lived
experience in our schools. Our team has a natural connection with local kids, maintaining trust established
with families, community partners, and Eagle County School District over many years. Executive Director
Bratzo Horruitiner, a long-time youth development professional with decades of work in our Hispanic
communities, developed My Future Pathways. Bratzo assembled a team of Hispanic youth leaders in our
community, many from under-resourced, first-generation families, and tasked them with keeping kids
engaged in school. They include Yesenia Briones, David Garcia, and Gerry Lopez, who immigrated to our
community as children and attended Eagle County School District schools before college. These younger
staff members work directly in student programming to promote an organic and representative connection
with local youth.
Additionally, Bratzo recruited Pedro Alvarez, a local father with decades of experience as a spor ts and
academic coach for Eagle County kids in after-school programs. This team of local youth leaders has been
successful due to the trust established with families, community par tners, and Eagle County School District
over the years and decades. Through this model, My Future Pathways’ youth leaders with lived experience
are creating solutions for our local communities. MFP recently won the Vail Valley Par tnership’s Excellence
in DEI award, proof of our commitment to representation and equity within the community.
EC’s award would help our culturally-competent staff members become even better community advocates
by increasing their technical skills, social media presence, providing concrete IT resources, and developing
them into responsible custodians of delicate PII.

Health Equity Framework:A Public Health Framework developed to collect data,

Eagle County Public Health and Environment will monitor this Contract for compliance
