This permit is valid only for connection to structures which have fully complied with County zoning and building requirements.

shall automatically be a violation of a requirement of the permit and cause for both legal action and revocation

Dear Mr. Boles:
Your individual sewage disposal system permit for the above
referenced property is enclosed. You should attach this letter
to your permit and consider the stipulation stated herein as
condition of the permit.
CONDITIONS FOR PERMIT #766:
1. The applicant must submit an as -built site plan
showing the exact location of the sewage disposal
system and an alternative or backup leach field
site. The site plan must be prepared and signed by a
registered enc ink, r architect or surveyor.
2. A lea h field easement -Just be created on Lot 6 for
the co on disposal o !sewage effluent from Lot 6 &
7. In ad i ' , each field maintenance agreement
must be approved by this office and recorded with the
above referenced leach field easement. The leach
field easement and associated maintenance agreement
must be approved and recorded prior to issuance of a
certificate of occupancy for either lot.

3. This permit is approved based on the building plans
drawn by Snowden and Hopkins Architects - Job #8601,
dated 9-4-86. A new individual sewage disposal
system permit shall be required for expanded use
beyond the designed capacity as stated in ISDS
Application #3070 and the above referenced plans.
4. Water conserving appliances and fixtures are highly
recommended.

Thank you for pointing out the discrepancy with the
conditions of.the "footing and foundation permit" and the
individual sewage disposal system permit". _Upon recon-
sideration of the conditions, it has been determined that
the leach `field easement and associated maintenance agree-
ment must be submitted for review with your building permit
application. The leach field easement and.maintenance
agreement must be recorded with the Eagle County Clerk and
Recorder prior to final inspection of the individual sewage
disposal system.
The other conditions referenced in my letter to you
dated September 22, 1986, remain in effect. I hope this
discrepancy did not cause you delay or hardship

At the request of Billy Morgari of Double M Co ruction on June 25 through the 29,
1999, we visited the construction site on Lots 6 & 7, Block 6, Fake Greek Meadows, 201 & 225
Eagle Crest Drive, Eagle County, Coiarado. The purpose.of our site visits was to observe the
installation of the septic system.
They installed the systerh in overall compliance *ith the septic system design, Drawing
No. 9983SD.DWf i, dated May 11,1999. The system was.insWed, as shown on the as -built
sketch, attached. During constmaction it was discovered that the outlet pipe from the existing
septic tanks was lower than the.inlet to -the new leachfield. For d is reason a new, 500-gallon, lift
stationwas installed. A new clamout was also, installed; ii. the south side of the driveway. . sriygn -
xee, Standard. Infiltrator Chambers were installed as shown on the sketch. The infiltrators
were connected with serial..tIistribution. In t&-last infiltrator of each trench, inspection ports
were installed. Horizontal. distaff from center to -renter of the trenches, was 9 feet or more. All
piping was SDR-35, 4-inch`PVC. .The depth of the trenches Was two to.three feet. The soil profile
in the trenches was consistent With the soil logged'in the profile hole. An as -built sketch of
the system, prepared by tht -installer, is included. The
site over the leacbfield should be graded to divert all runoff away from the leach£.eld. Water
from an irrigation system or roof runoff can satin the leachfield and reduce its life. If

Dear John:
At the request from Jake Schwaiger, on September 26 and 29, 2012, we visited the
construction site at the Lot 7, Block 6, Lake Creek Meadows, 225 Eagle Crest Road, Eagle
County, Colorado. The purpose of our site visits was to inspect the installation of the septic
system.
They installed the system in overall compliance with the septic system design, Drawing
No. 12037SD.DWG, dated August 7, 2012, and as shown on the attached as-built sketch. They
installed a 1500-gallon, two compartments, plastic septic tank by Roth MultiTanks. There was an
existing cleanout between the septic tank and the building, and a second cleanout between the
tank and the leachfield. The second cleanout was on the effluent line, and was about 9 feet east
from the tank.
Seventy-five, Q-4, Standard Infiltrator Chambers were installed in four trenches. There
were twenty-five (17) infiltrators in the first trench. In the second and third trenches there were
twenty-two infiltrators each, and in the last trench there were fourteen infiltrator chambers. There
was a minimum of six feet separation between the trenches. In the first and last infiltrator of each
trench, inspection ports were installed. The T’s in both compartments of the septic tank were at
least 14-inches long.
They also disconnected and capped both ends of the old building sewer line, thus
separating the two septic systems. The old system will service Lot 6 and the new Lot 7.
There is no other warranty either expressed or implied.
