and finalized. This OWTS permit was approved and sized to serve a total of five (5) bedrooms
Additional information about the maintenance your septic system needs can be accessed through
our website links, provided on the Environmental Health Department’s septic system resource page.
Be aware that changes in the use of your property or alterations of your building may require
commensurate changes to, or relocation of, your septic system . Landscape features, trees with tap
roots, irrigation systems and parking areas above the soil treatment area can cause premature
system failure. It is equally important that you notice and immediately repair dripping faucets and
hissing toilets as this will certainly cause the system to fail.

Subject: As-Built Evaluation of On-Site Wastewater Treatment System (OWTS), Proposed
Residence, Lot R-7, South Forty Subdivision, 37 Lariat Court, Eagle County,
Colorado.
Dear Carolyn:
As requested, Kumar & Associates, Inc. (K+A) has observed the installation of the Onsite
Wastewater Treatment System (OWTS) for the proposed residence at the subject property. Our
observations and recommendations are presented in this report. The services are supplemental
to our agreement for professional services to you dated June 12, 2017, proposal number P6-17-
188.
We previously designed the OWTS for the project and presented our recommendations in a report
dated April 24, 2018, and revised on May 7, 2018, under the above Project Number. The system
was designed for five bedrooms. The OWTS components observed included: sewer pipe, septic
tank/dose tank, pump and pump operation, alarm, pump vault, effluent transport pipe, soil
treatment area (STA), cleanouts, inspection ports and the locations of components. The installer
was Edwards Excavating.
We performed our As-Built site evaluation on August 29, 2019 and September 27, 2019. The
installation and locations of system components were verified as shown on Figure 1. Photographs
of the OWTS installation are provided in the attached Photograph Log.
It is our opinion that the OWTS was installed in general conformance with our OWTS design for
the project, dated April 24, 2018 and Revised May 7, 2018. The soils observed at the base of the
STA were consistent with the soils encountered during our field exploration for the project,
consisting of loamy sand with scattered gravel, and no sizing modifications to the design were
recommended. All potable water system, property and other required system setbacks, as noted
in our report, should be verified by the contractor during construction.
The observations and recommendations presented above are based on our site observations and
our experience in the project area. We make no warranty either express or implied. Alterations
to the system or improper backfill and site grading could affect the operation and effectiveness of
the OWTS.
