Installation of the OWTS was completed as depicted in the LKP Engineering Inc, design stamped signed and dated
October 20, 2020 by Luiza Petrovska, PE. The septic was designed to serve the existing 3 bedrooms residence and
one bedroom ADU and completely replace the systems permitted #IS-266-78, #1190-92IS, #1732-97IS.
Abandonment of the existing tank was to be completed in conformance with ECPHA OWTS Regulation 43.9.C.
System installation was completed without permit in hand, but design was completed in compliance with current
ECPHA OWTS Regulations, and installation was observed in compliance with aforementioned regulations and PE
design.
THIS PERIMT EXPIRES BY TIME LIMITATION AND BECOMES NULL AND VOID IF THE WORK AUTHORIZED BY THE PERMIT IS NOT COMMENCED WITHIN 120 DAYS OF

Dear Jose,
On September 9 and October 5, and 19, 2020, we visited the John Horan-Kates Property on Lot
20 Block 6, Lake Creek Meadows, 57 Elk Place, Eagle County, Colorado. The purpose of our site visit
on September 9, 2020 was to observe the site of the existing septic system and select an area for the
proposed new Soil Treatment Area for the replacement of the old leachfield. During the following site
visit, you excavated two profile holes. On the last site visit, on October 19, 2020, per the owner’s
request we selected a portion of the previous STA for a bed configuration layout with a sand filter and
pressure dosing in order to reduce the size of the STA and prevent more trees from being cut on the site.
From the information provided by Claire Lewandowski, REHS, Environmental Health Specialist
III with ECEHD, there are records on file for the original septic system for the property from 1978 for 2-
bedroom residence, the next one is from 1992 for a 3-bedroom residence, and another record is for an
alteration in 1997, also for a 3-bedroom residence.
The OWTS replacement is for a 4-bedroom residence that combines the main residence and an
ADU, and it will consist of a 1500-gallons, two-compartments concrete septic tank by valley Precast
with an effluent filter, with a 500-gallons, one-compartment concrete septic tank for the pump and
dosing chamber, and Soil Treatment Area that will consist of 56 Quick 4 Plus, Standard Infiltrator
Chambers in a bed configuration with a 2-foot sand filter and pressure dosing.
The old leachfield is on the east side of the property, while the selected new site for the STA
replacement is on the west side of the property. A topographical survey prepared by Sam Ecker with
Gore Range Surveying, LLC, was used as a basis for the septic system design. Directions to the site are
from US Hwy 6 in Edwards to Lake Creek Road to West Lake Creek Road to Eagle Crest Road to Elk
Place. The driveway is west off of the cul-de-sac.
! Preliminary Investigation
G Address: The property address is 57 Elk Place, Edwards, Eagle County,
Colorado.
G Legal Description: Lot 20, Block 6, Lake Creek Meadows, Eagle County,
Colorado.
G Existing Structures: There was an existing residence and a detached garage.
G Location of existing and proposed wells on property: There is no well on the
property. It is services by a community well for the subdivision, located on the
north side of Angela Lane in Lake Creek.

G ECEHD (Eagle County Environmental Health Department) records: Old record
card was received and reviewed. The old system was built in 1978 and altered in
1992 and 1997.
G Topography: The topography of the leachfield site is flat to moderate with an
average slope of about 15 percent. Drainage is to the north and northeast.
G Soil Data: The profile holes were observed on October 5, 2020. The soil
exposed in the profile holes was as follows:

A soil sample taken from Profile Hole # 2 at 48-inches, was test for tactile
test and classified as Sandy Clay Loam, Type 3, with less than 35% Rock
(>2mm)
LTAR = 0.35 gpd/sf,
In order to reduce the size of the STA and to save the trees on the
property, the STA will be in a bed configuration with 4 rows of 14
infiltrators in each, over a 2-foot sand filter and with pressure dosing.

G Landscape position: The proposed STA will be located in the Foot/Toe
Slope Position north from the existing residence (down gradient).
G Topography: The topography of the proposed leachfield site is moderate.
The rows of infiltrators in the bed will run along the contours.
G Vegetation: The vegetation within the proposed leachfield site consisted of
aspen forest with thick underbrush. The site surrounding the leachfield
was similar.

from Lake Creek and about 2.7 miles south from Eagle River. It is also
about 2.5 miles south from US Hwy 6.

ranching.
