The system is designed to be the complete replacement of the existing failed OWTS, permit #1368-94IS, and serve the existing 5 bedroom single family residence. Abandonment of the existing tank must be completed in conformance with ECPHA OWTS Regulation 43.9.C. The OWTS consists of a 1530 gallon two-compartment Infiltrator poly tank followed by a IM-540 Infiltrator poly dosing tank equipped with an effluent filter and high head pump, with floats set to deliver effluent to an Orenco V6404A Automatic Distribution Valve (ADV), set at the high point of the system and made accessible at grade. The ADV will alternate pressure dosed effluent between four equal trenches, each consisting of 21 Quick4 Plus Standard chambers, for a total of 84 chamber equivalent to 1000 square feet of soil treatment area. The chambers are to be placed atop four feet of over excavated soil media that is removed, broken down, mixed, and replaced. Maintain all setbacks and elevations as shown on the design drawing. Contact Eagle County Environmental Health and the design engineer well in advance of requesting inspections prior to backfilling any component of the OWTS. The design engineer is responsible for conducting all inspections necessary to certify the installation and assure the functionality of the system. System certification, along with photos and a record drawing is required to be submitted to and approved by, Eagle County Environmental Health prior to the use of the system or occupancy of the dwelling. Box 3005 Edwards, CO 81632 bmazz68@comcast.net RE: Inspection of Septic System Installation OWTS-Replacement – 5-Bedroom Lot 12, Block 6, Lake Creek Meadows 38 Idlewild Place Eagle County, Colorado Project No. 20-3405– OWTS-019059-2020 Dear Butch: At the request of the installer, Travor Dunsdon, Altitude Septic LLC, on July 22 and 30, 2020, we visited your property at 38 Idlewild Place, Eagle County, Colorado. The purpose of our site visits was to inspect the installation of the new septic system that replaces the existing, failed septic system, for the existing 5-bedroom residence. The system was installed in overall compliance with the septic system design, Drawing No. 20-3405OWTS.dwg, dated June 24, 2020. At the time of the inspection we observed the following: They installed 1530-gallons, two-compartments, Infiltrator Poly Tank by Valley Precast with an effluent filter in the second compartment and IM-540, one compartment, Infiltrator Poly Tank with an Orenco PF500511 high-head pump assembly. The inlet of the tank had the Tee installed. The tank access ports had corrugated, plastic risers with secured, green, plastic covers. The covers were insulated and will be secured with nuts, bolts and screws. All compartments had 36 inches risers. The building sewer line was connected to the existing sewer line on the north side of the residence at about 11.3 feet NW from the NE building corner, as shown on the attached as-built sketch. A cleanout was constructed on the building sewer line about 4 feet away from the new septic tank. The building sewer line was 4-inch diameter SDR35 PVC pipe. The effluent line from the pump chamber to the Orenco, Automatic Distribution Valve, V6404A, was about 66 feet, 1.5-inch diameter, sch40 pvc pipe. The Distribution valve was also enclosed in a 24-inch diameter, corrugated plastic riser, 36-inch high, with secured, insulated cover. The soil treatment area consisted of infiltrator chambers, arranged in four trenches with 21 infiltrators in each. Four feet of the onsite silty-clayey, gravelly sand to sandy gravel with cobbles and boulders were overexcavated from the bottom of the trenches. The large boulders were removed from the overexcavated soil, the remaining soil was mixed and placed back in the trenches. The pressure dosing system consisted of 84-ft long, 1.5-inch sch40 pvc laterals perforated at the top with 5/32-inch holes at 4-ft intervals, suspended from the west side of each row of infiltrator chambers. All inspection ports were installed (in the end caps of the first and last chamber of each row of infiltrators) opposite the pressure distribution laterals. The transport lines from the Automatic Distribution Valve to each of the trenches of the STA (Soil Treatment Area) were 1.5-inch diameter sch40 pvc. The septic tank installation and startup of the pump system were done by Valley Precast. The control panel for the pump was on the south side of the Pump Tank. The old, concrete septic tank was abandoned by breaking and burying in place. The Inspection ports, the cleanout, and the covers of the tanks and the distribution valve were located by the surveyor, Ted Archibeque, PLS, Archibeque Land Consulting, Ltd. The purpose of our site visits was to observe the site and select an area for the replacement onsite wastewater treatment system. There is an existing residence on the property. The following evaluation was prepared for the replacement of the existing septic system for the existing 5-bedroom residence. From the information provided, we understand that the existing system consists of 1250gallon, septic tank and 22 infiltrator chambers. The existing, failing system was constructed in 1994, with Eagle County ISDS Permit Number 1368. The proposed new OWTS was designed for a 5-bedroom residence. During the first site visit, we met with you and your previous installer, Jose Gonzales. During the second site visit, your new installer, Travor Dunsdon with Altitude Septic, dug two profile holes within the selected new soil treatment area. We logged the profile holes and took soil samples for evaluation and tactile testing. The property is a 1.768-acre parcel of land, according to the Eagle County Assessor’s Records and according to the survey prepared by Ted Archibeque with Archibeque Land Consulting, Ltd. Directions are from U.S. Highway 6 in Edwards to Lake Creek Road to West Lake Creek Road, left to Eagle Crest Road and straight (right) to Idlewild Place. The property is west and southwest of the Idlewild Place cul-de-sac. ! Preliminary Investigation G Address: The property address is 38 Idlewild Place, Eagle County, Colorado, 91632. G Legal Description: Lot 12, Block 6, Lake Creek Meadows, Eagle County, Colorado. G Existing Structures: There is an existing residence on the site. G Location of existing and proposed wells on property: The property is serviced by public water. No wells.
