Install the OWTS exactly as depicted in the LKP Engineering design dated March 8, 2019, stamped and signed by
Luiza K. Petrovska P.E. The system is designed to serve a five bedroom single family residence.
The OWTS consists of a 1530 gallon, two-compartment, Infiltrator Poly Tank and IM540 Infiltrator Poly Tank,
one-compartment with an Orenco Pump Assembly by Valley Precast equipped with an Orenco head head pump
(PF5005) and effluent filter in the outlet tee. Set the floats to pressure dose 90 gallons approximately 62 feet to a
center manifold, connected to six, 41 foot distribution laterals. The laterals are set to distribute effluent across a
mounded soil treatment area consisting of an aggregate distribution cell 84'l ong x 9' wide, set atop 2-2.8'of
secondary sand filter media 111' long x 38' wide (area includes the 3-1' slope on all sides) as shown in the design
drawing. Clear and rake the ground surface immediately prior to placement of aggregate sand. Construction must
only be started when the soil site is suitably dry. Maintain all elevations as shown of the engineer's site plan.
Contact the design engineer and Eagle County Environmental Health well in advance to conduct of requesting
inspections. All inspections necessary to certify proper installation must be performed by the design engineer.
System certification along with photos and a record drawing are required to be submitted to, and approved by,
Eagle County Environmental Health prior to the use of the system and occupancy of the residence.
THIS PERIMT EXPIRES BY TIME LIMITATION AND BECOMES NULL AND VOID IF THE WORK AUTHORIZED BY THE PERMIT IS NOT COMMENCED WITHIN 120 DAYS

Arena Excavating, identified on the permit, is not licensed. Arena Excavating must become licensed prior to
installation or a licensed installer should be identified. No person shall install, renovate, or repair an OWTS in Eagle
County (EC) without a valid EC Systems Contractor License. Any person that conducts a business as a systems
contractor without having obtained a valid license section commits a Class 1 petty offense as defined in section
18-1.3-503, C.R.S. Please notify the department when an installer is identified, prior to installation of the OWTS.

Dear Mark:
At the request of the installer, Travor Dunsdon of Alpha Excavation LLC, on October 31,
2022 we visited your property on Lot 4, Block 5, Cordillera Filing No. 34, 602 Granite Springs Trail,
Eagle County, Colorado. The purpose of our site visit was to inspect the installation of the OWTS –
consisting of pressure-dosed, mound system.
The system was installed in overall compliance with the septic system design, Drawing No.
18-3303OWTS.dwg, last updated on August 19, 2022. At the time of the inspection, we observed the
following:
They installed 1530 gallon, two-compartment, Infiltrator Poly Tank and IM540 Infiltrator
Poly Tank, one-compartment with an Orenco PF500511 high-head pump assembly by Valley Precast.
An effluent filter was installed in the second compartment of the 1530-gallon poly tank. The inlet of
the tank had the Tee installed. The tank access ports had corrugated, plastic risers with secured, green,
plastic covers. The covers were insulated and will be secured with nuts, bolts, and screws.
The building sewer line was connected to the back of the residence, as shown on the attached
as-built sketch. A cleanout was constructed on the building sewer line about 1.6 feet away from the
house. The building sewer line was 4-inch diameter SDR35 PVC pipe.
The two-compartment septic tank and the pump tank, were connected with about 44 feet of 4-
inch diameter, SDR35 PVC pipe.
The effluent line from the pump chamber to the center manifold of the mound system, was
1.5-inch diameter, SCH40 PVC pipe.
The soil treatment area consisted of a mound system, consisting of a sand fill with a gravel
distribution cell. The sand was from United Gravel Pit, was less than 30-days old, and it passed the
classification for a secondary sand. The pressure dosing system consisted of six, 41-feet long, 1.25-
inch diameter SCH40 pvc, distribution laterals, perforated at their inverts with 1/8-inch holes at 3-

foot intervals, arranged on the gravel bed with three laterals on each side of a center manifold. The
center manifold 6 feet, 1.25-inch, SCH40 pvc. The ends of the laterals were equipped with flushing
assemblies. A squirt test was performed during the inspection. Two inspection ports were installed,
one on each side of the distribution cell, at about the middle between the laterals.
The OWTS installation and the system start-up was done by Travor Dunsdon. The control
panel for the pump was installed in the utility box attached to the foundation wall on the north side of
the residence, as shown on the as-built sketch.
There is no other warranty either expressed or implied.

Dear Mark:
The following report was revised to replace the concrete septic tank with Infiltrator septic
tanks, due to supply shortage.
On October 27, 2018, we visited the subject property on Lot 4, Block 5, Cordillera Filing
No. 34, 602 Granite Springs Trail, Eagle County, Colorado near Edwards, Eagle County,
Colorado. The purpose of our site visit was to observe the site and select an area for the new,
Onsite Wastewater Treatment System (OWTS). Buz Reynolds, The Reynolds Corporation, dug
the profile holes.
The proposed Onsite Wastewater Treatment System (OWTS) will consist of 1530 gallon,
two-compartment, Infiltrator Poly Tank and IM540 Infiltrator Poly Tank, one-compartment with
an Orenco Pump Assembly by Valley Precast. The septic tank will have an effluent filter in the
second compartment. The Soil Treatment Area (STA) will consist a mound system. The mound
system was designed partially in compliance with the Technical Guidance for Mounded
Wastewater Treatment Systems and partially with the Wisconsin Mound System Guidance.
According to the guidelines of the Technical Guidance for Mounded Wastewater Treatment
Systems the maximum recommended linear loading rate for soil types 3A through 5 is in the
range of 3.0 to 5.0 gpd/lf. With this rate the length of the proposed mound system was greater
than the site. Due to the size of the site and relatively gentle to moderate slope of 8 percent we
decided to use the Wisconsin Mound System Guidance, where the area of the distribution cell
remained the same but the dimensions changed, thus allowing the mound system to fit on the
site. The linear loading rate used in the final design was 7.2 gpd/lf.

SITE PREPARATION
STAKE OUT THE MOUND PERIMETER. CUT AND REMOVE ANY EXCESSIVE VEGETATION.
INSTALL THE DELIVERY PIPE FROM THE DOSING CHAMBER TO THE MOUND. BACKFILL AND COMPACT THE PIPE
TRENCH.
FILL PLACEMENT
PLACE THE FILL MATERIAL OVER THE PREPARED SITE.
THE FILTERING MATERIAL USED IN A SAND FILTER MUST BE CLEAN, COARSE SAND, CLASSIFIED AS PRIMARY SAND OR
SECONDARY SAND. THE ABOVE DESIGN WAS PREPARED BASED ON SECONDARY SAND WITH SAND FILL LOADING RATE
OF 0.8 GPD/SF.
THE CLASSIFICATION OF THE SAND FILL TO BE USED IN THE MOUND SYSTEM MUST BE LESS THAN 30 DAYS OLD AND
SUBMITTED TO THE ENGINEER PRIOR TO THE SAND FILL PLACEMENT. THE RESULTS OF THE SAND FILL SPECIFICATION
MIGHT REQUIRE A REVISION OF THE SYSTEM.
THE HEIGHT OF THE SAND FILL SHOULD BE THE ELEVATION OF THE TOP OF THE ABSORPTION BED. SHAPE THE SIDES
OF THE MOUND TO THE RECOMMENDED SLOPE, AS SHOWN ON THE DETAILS.
DISTRIBUTION NETWORK PLACEMENT
CAREFULLY PLACE THE COARSE AGGREGATE ON THE SAND BED. THE COARSE AGGREGATE WILL CONSIST OF 3/4 TO
2-1/2 INCH CLEAN, CRUSH ROCK. LEVEL THE AGGREGATE TO A MINIMUM DEPTH OF 6 INCHES. ASSEMBLE THE
DISTRIBUTION NETWORK OVER THE AGGREGATE. THE DISTRIBUTION NETWORK WILL CONSIST OF EIGHT 1.25-INCH
DIAMETER SCH40 PVC, 41-FEET LONG LATERALS, (THREE ON EACH SIDE OF A 1-1/4-INCH DIAMETER CENTER
MANIFOLD). THE INVERTS OF THE LATERALS SHALL BE PERFORATED WITH 1/8-INCH HOLES SPACED EVERY 36 INCHES.
FOR THE PRESSURE DISTRIBUTION NETWORK USE SCHEDULE 40 PVC (ASTM D 2665) OR ABS (ASTM 2661) PIPE.
THE LATERALS SHALL BE LAID LEVEL. CONNECT THE FLUSHING ASSEMBLY AT THE OUTER END OF EACH LATERAL.
PLACE A MINIMUM OF 3 INCHES OF ADDITIONAL AGGREGATE OVER THE CROWN OF THE PIPES. PLACE A SUITABLE
BACKFILL BARRIER, SUCH AS FILTER FABRIC, OVER THE AGGREGATE.
SQUIRT TEST OF THE RESIDUAL HEAD ON THE MOUND LATERALS
REMOVE THE END CAPS OF THE FLUSHING ASSEMBLY IN ORDER TO FLUSH THE LATERALS CLEAN FROM ANY DRILLING
DEBRIS OR DIRT. PROVIDE ONE 1/8-DIAMETER HOLE IN A TEMPORARY CAP ON THE ENDS OF THE FLUSHING ASSEMBLY
FOR THE SQUIRT TEST. RUN THE SQUIRT TEST TO MEASURE THE 5-FT RESIDUAL HEAD (+/-10%)
AFTER THE TEST REMOVE THE TEMPORARY, PERFORATED CAPS AND REPLACE WITH THE PERMANENT CAPS.
COVERING OF THE MOUND SYSTEM
PLACE A FINER TEXTURED SOIL, SUCH AS CLAY OR SILT LOAM OVER THE TOP OF THE BED, TO A MINIMUM THICKNESS
OF 6 INCHES.
PLACE ADDITIONAL 6 INCHES OF GOOD QUALITY TOPSOIL OVER THE ENTIRE MOUND SURFACE. PLANT GRASSES
ADAPTABLE TO THE CLIMATE OVER THE MOUND SYSTEM.
PUMP SELECTION
A PUMP CAPABLE OF DELIVERING 37GPM AGAINST 27 FEET OF HEAD CAN BE USED. THE ELEVATIONS BETWEEN THE
PUMP INVERT AND THE MANIFOLD INVERT MUST BE FIELD VERIFIED. THE PUMPING SYSTEM MUST ALSO INCLUDE PUMP
CONTROLS AND AN ALARM SYSTEM, AUDIBLE AND VISUAL. THE PUMP DOSING WILL BE ON DEMAND WITH 90 GALLONS
PER DOSE. USE ORENCO, PF500511 HIGH HEAD EFFLUENT PUMP, 115/230V, 50GPM, 1
2 HP, OR EQUIVALENT, APPROVED
BY THE ENGINEER. WHEN THE PUMP SHUTS OFF THE EFFLUENT LINE WILL DRAIN BACK INTO THE PUMP CHAMBER. THE
PUMP SYSTEM MUST BE TESTED BEFORE THE FINAL INSPECTION OF THE SEPTIC SYSTEM AND A LETTER OF THE TEST
SUBMITTED TO THE ENGINEER.
INSPECTION PIPE
INSTALL A 4 INCH DIAMETER INSPECTION PIPE ON BOTH ENDS OF THE GRAVEL BED. THE PIPE WILL BE OPEN ON THE
BOTTOM. THE BOTTOM OF THE PIPE WILL BE AT THE SAME ELEVATION AS THE BOTTOM OF THE GRAVEL BED. THE
BOTTOM 8 INCHES OF THE INSPECTION PIPE WILL BE PERFORATED. THE PIPE WILL BE COVERED WITH A VENT CAP.
