Permitted Construction / Details:
Install the OWTS exactly as depicted in the LKP Engineering, Inc design stamped, signed, and dated March 30, 2021
by Luiza Petrovska, PE. The OWTS is designed to serve a new four bedroom single family residence.
The OWTS consist of a 1500-gallons, three-compartment concrete septic tank by Valley Precast with an effluent
filter, and an Orenco Pump assembly. Set the pump floats to pressure dose approximately 90 gallons of effluent on
demand to 684 square feet of soil treatment area with three rows of 19 Quick 4 Plus Standard chambers arrange in
bed configuration over a 3-foot sand filter of secondary sand media. Install inspection ports in all four corners of
the bed.
Contact Eagle County Environmental Health and the design engineer well in advance of requesting inspections prior
to backfilling any component of the OWTS. The design engineer is responsible for conducting all inspections
necessary to certify the installation and assure the functionality of the system. System certification, along with
photos and a record drawing is required to be submitted to and approved by Eagle County Environmental Health
prior to the use of the system or occupancy of the dwelling.
Note: Gradation of the sand media use must be completed within one month prior to the installation and confirmed
with the design engineer. If the sand does not conform to secondary or preferred sand media requirements, it
cannot be used for the sand treatment media.
THIS PERIMT EXPIRES BY TIME LIMITATION AND BECOMES NULL AND VOID IF THE WORK AUTHORIZED BY THE PERMIT IS NOT COMMENCED WITHIN 120 DAYS OF
ISSUANCE, OR BEFORE THE EXPIRATION OF AN ASSOCIATED BUILDING PERMIT

Dear John:
At the request of the installer, Rodney Morgan, RDM Excavating, on July 20, 2022, we visited your
property on Lot 56, Cordillera Filing No. 3, 720 Andorra Road, Eagle County, Colorado. The purpose of our
site visit was to inspect the installation of the septic system.
The system was installed in overall compliance with the septic system design, Drawing No. 21-
3485OWTS.dwg, dated March 30, 2021. At the time of the inspection, we observed the following:
The building sewer line was connected to the south side of the residence, west of the SE corner. The
cleanout will be level with the lower patio, as shown on the attached as-built sketch. The building sewer line
was about 29 feet of 4-inch diameter SDR35 PVC pipe. They installed 1500-gallons, three compartments,
precast, concrete, septic tank by Valley Precast with an effluent filter in the second compartment and an
Orenco PF500511 high-head pump assembly in the third compartment. The inlet of the tank had the sanitary
Tee installed. The tank access ports had corrugated, plastic risers with secured, green, plastic covers. The
covers were secured with nuts, bolts and screws. All risers were 38-inch high. The control panel was installed
on the south side of the residence, at the SE corner.
The effluent line from the pump chamber to end manifold of the soil treatment area, was about 28 feet
long, 1.5-inch diameter, sch40 pvc pipe. The end manifold was about 7 feet long, 1.5-inch sch40 pvc. The soil
treatment area consisted of infiltrator chambers arranged in a bed configuration with three rows of 19 Quick
Four Plus Standard Infiltrator Chambers in each. Three feet of the onsite silty, clayey, rocky soil was
overexcavated from the bottom of the trenches and replaced with three feet of secondary sand media (the graph
is attached to this report). Pressure dosing system consisting of 78-ft, 1.5-inch sch40 laterals perforated at the
top with 1/8-inch holes at 4-ft intervals, suspended from the inside side of each row of infiltrator chambers.
The ends of the laterals were equipped with flushing assemblies. A squirt test was performed during the
inspection and it was satisfactory. Claire and Kurt from the ECEH Department were present during the
inspection and the squirt test as well. All inspection ports were installed (in the end caps of the first and last
chamber of each row of infiltrators) in the opening opposite the pressure distribution lines.
The septic tank installation and startup of the pump system were done by the installer, Rodney Morgan
and his crew. The control panel for the pump was on the back of the house.
There is no other warranty either expressed or implied.

Dear John,
On March 25, 2021, we visited your property on Lot 56, Cordillera Filing No. 3, 720
Andorra Road, Eagle County, Colorado. The purpose of our site visit was to select an area on the
property for the proposed Soil Treatment Area. During the same site visit, the excavator/installer
Travor Dunsdon, with Altitude Septic, dug two profile holes.
The OWTS is for a proposed 4-bedroom residence, and it will consist of a 1500-gallons,
three-compartments concrete septic tank by Valley Precast with an effluent filter, and an Orenco
Pump assembly, and the Soil Treatment Area will consist of 57 Quick 4 Plus, Standard Infiltrator
Chambers in a bed configuration with a 3-foot sand filter and pressure dosing.
The site plan by Brent Alm, AIA, Alm Architects, Inc., was used as a basis for the septic
system design. Directions to the site from Edwards are as follows: from US Highway- 6 turn left
onto Squaw Creek Road, continue for 2 miles and turn left onto Cordillera Way, continue for 0.7
miles and turn left onto Andorra Road. The lot is at the end of the road, just before the cul-de-
sac, on the right (uphill) side of the road.
! Preliminary Investigation
G Address: 720 Andorra Road, Edwards, CO 81632.
G Legal Description: Lot 56, Cordillera Filing No. 3, Eagle County,
Colorado.
G Existing Structures: There were no existing structures on the lot.
G Location of existing and proposed wells on property: There are no wells
on the property. It is services by public water.
G ECEHD (Eagle County Environmental Health Department) records: There
were no old record. The property was vacant.
G Topography: The topography of the leachfield site is moderate with an
average slope of about 27 percent. Drainage is to the south.
G Soil Data: The profile holes were observed on March 25, 2021. The soil

A soil sample taken from Profile Hole # 1 at 48-inches was classified as
Type 2, LTAR = 0.6 gpd/sf with <35% Rock (>2mm) LOAM
Recommended: Due to the weathered bedrock with consistency of sandy
gravelly soil, encountered in both, Profile Hole Numbers 2 and 3, STA in a
bed configuration with 3 rows of 19 infiltrators in each, over 3 feet of
unlined sand filter and with pressure distribution will be used.

G Landscape position: The proposed STA will be located in the Back/Side
Slope Slope Position south from the proposed residence (down gradient)
G Topography: The topography of the proposed leachfield site is moderate.

grass, sage brush, and juniper trees. The site surrounding the leachfield
