Permitted Construction / Details:
Replace the existing damaged tank, permit 1787-98, exactly as depicted in the LKP Engineering, Inc design,
stamped, signed, and dated February 4, 2021 by Luiza Petrovska, PE. The new tank will be sized to accommodate
the maximum daily flows for a 5 bedroom single family residence.
The repair consists of a new sewer line connecting to the new Infiltrator Poly IM-1530 two compartment septic tank
with effluent filter in the outlet tee. Abandon the damaged tank in accordance with Eagle County Public Health
Agency OWTS Reg 43.9.C.
Note: This permits the replacement of the failing septic tank, to repair and restore functionality to the system as
intended and so requires that the replaced components are brought up to current code. This permit does not
address the soil treatment area, according to permit 1787-98 was sized for only a maximum daily flow from a three
bedroom single family residence. At time of soil treatment area repair, replacement, due to failure, or remodel,
changes to regulation such as a transfer of title requirement, the soil treatment area must be brought up to code and
sized to served the assessed number of bedrooms.
Contact Eagle County Environmental Health and the design engineer well in advance of requesting inspections prior
to backfilling any component of the OWTS. The design engineer is responsible for conducting all inspections
necessary to certify the installation and assure the functionality of the system. System certification, along with
photos and a record drawing is required to be submitted to and approved by Eagle County Environmental Health
prior to the use of the system.
THIS PERIMT EXPIRES BY TIME LIMITATION AND BECOMES NULL AND VOID IF THE WORK AUTHORIZED BY THE PERMIT IS NOT COMMENCED WITHIN 120 DAYS OF

Dear Barry:
At the request of Travor Dunsdon, Altitude Septic, LLC, the installer, on February 10 and
today, February 12, 2021, we visited your property on Lot 31, Block 6, Lake Creek Meadows
474 Eagle Crest Road, Eagle County, Colorado. The purpose of our site visit was to inspect the
installation of the new septic tank.
The system was installed in overall compliance with the repair design, dated February 4,
2021. At the time of the inspection we observed the following:
He installed a 1500-gallons, Infiltrator IM-1530, a two compartments plastic tank with an
effluent filter in the second compartment from Valley Precast. The inlet and outlet of the tank
had the T’s installed. In the outlet Tee, an effluent filter was installed with the handle extended
close to the top of the riser. The new pipes (the building sewer line and the effluent line) were
laid over a bed of crushed rock. The new building sewer line was constructed with SCH40 pvc to
repair the low point. The tank access ports had 42-inch corrugated, plastic risers with green,
plastic lids secured with screws. The height of the risers will be adjusted (cut) to be level with
the ground surface. According to Travor, he brought the old septic tanks to the Eagle County
Dump for disposal.
There is no other warranty either expressed or implied.

Dear Barry:
On February 3, 2021, we visited your property on Lot 31, Block 6, Lake Creek Meadows,
474 Eagle Crest Road, Eagle County, Colorado, Old Septic Permit 1787-98, Eagle County,
Colorado. The purpose of our site visit was to locate the uncovered components of the existing
septic tank.
From our conversation, emails, and from the documents received from the Eagle County
Environmental Health Department, we understand that the existing system was built in
November of 1998 with a 1250-gallon septic tank for four bedrooms and soil treatment area
(leachfield) for three bedrooms. We also reviewed the inspection report (attached) by Travor
Dunsdon, Altitude Septic, dated January 28, 2021. In his report, Travor found that the septic tank
is damaged beyond repair and has to be replaced as well as the building sewer line between the
tank and the house.
During our site visits, we observed (as uncovered by Travor) and located the access
manholes of the existing septic tank. The plastic tank covers and the access openings were
damaged and distorted.
We recommend that the old septic tanks be removed and disposed according to the ECEH
Department regulations, OWTS #43.9C.1 - Abandonment of Tank. The proposed new septic tank
will be Infiltrator IM-1530, a two compartments plastic tank with an effluent filter in the second
compartment. It would be preferable to install a concrete tank, but we understand that there is no
access for the delivery truck to bring the concrete tank to its proposed location.
Following are the instructions for the septic tank installation and the repair of the building
sewer lines:
x Bends in the building sewer shall be limited to 45 degrees.
x The septic tank will be installed level.
x The base under the new septic tank will be prepared according to
the manufacturer’s recommendation.
x The tank will have risers over each access manhole and all risers

will extend to or above final grade.
x Each riser lead will have a secure closing mechanism, such as a
lock, special headed bolts or screws, to prevent unauthorized
access.
x The septic tank and all the piping will be backfilled up to the
inverts prior to scheduling an inspection.
The installer, Travor Dunsdon with Altitude Septic will submit an application and a filing
fee for a repair permit, and will coordinate the abandonment of the old tank, the installation,
connection and backfilling of the new tank, and will make sure that all the required setbacks are
met. Once the new septic tank is installed with all the connections completed, you or your
installer will give a 48-hour notice to LKP Engineering and the ECEH Department to schedule
the inspection. DO NOT backfill any part of the system prior to the inspection.
This report has been prepared according to locally accepted Professional Engineering
standards for similar conditions at this time. There is no other warranty either expressed or
implied.
The findings and recommendations of this report are limited to the information available
at the time of writing of the above report. We are not responsible for technical interpretation by
others of the data presented in this report.
This report has been prepared for the exclusive use of Barry Wallach, for the proposed
septic tank replacement on Lot 31, Block 6, Lake Creek Meadows, 474 Eagle Crest Road, Eagle
County, Colorado.

As requested,
Altitude Septic performed a septic system inspection at the above address on January 28, 2021. The 1.736-acre lot
is located southwest of Edwards, with an existing five-bedroom residence, in an area that requires utilizing an
OWTS and is listed under parcel number #210518203002. The system was installed under permit #1787-98 which
required a 1,000-gallon tank with 844 square feet of absorption area for three bedrooms, or the option of a 1,250-
gallon tank with 1,125 square feet of absorption area to accommodate four bedrooms. The finalized permit
documents the installation of a 1,250-gallon septic tank with 930 square feet of absorption area, which means the
tank capacity is adequate for a four-bedroom residence, but the absorption area is only adequate for a three-
bedroom residence.
The septic tank is located off the northwest corner of the residence in between two piers which support the upper
deck. The lids were not accessible at our original visit, but the tank was located with a sewer camera through the
clean-out, which also showed a low spot in the line between the house and the septic tank. The locate revealed
that a concrete slab was over one lid and two pine trees had been planted over the second lid. The trees were
removed, and an excavator was used to remove the concrete slab and dig up the septic tank lids in order to
complete the inspection and give access for pumping out the tank. Both baffle tees are in place as required and the
liquid level in the tank was normal. Both lids are broken due to the distortion of the tank and the excess weight on
top of them. The top of the tank is also distorted which makes proper riser installation an impossible task, and we
are concerned that the baffle wall which separates the two compartments may be compromised because of the
distortion. We were informed that the septic tank was last pumped out in 2007.
The absorption area is northwest of and below the septic tank in a trench configuration which utilizes serial
distribution. The as-built drawing shows six trenches of equal length with inspection ports at the end of each
trench. We were able to find the inspection port for the first trench and probably the fourth trench. The cap was
broken at the first trench and the inspection pipe had dirt and debris in it which prevented us from seeing if
standing water was present. We did push a fiberglass rod into the pipe and near the bottom it was moist and
squishy, but no standing water was present in the fourth trench. The area was walked and observed with no
indication of surfacing or failure.
After the inspection we would make the following recommendations:
• Based on the condition of the tank we would recommend replacing it with a 1,500-gallon tank which would
be the proper size for the number of bedrooms and would accommodate risers so that access to the tank
would be feasible without excessive digging.
• Replace the cap on the first inspection port.
• Repair the low spot in the line between the house and septic tank as discussed.
