Permitted Construction / Details:
Install the new system exactly as depicted in the LKP Inc design dated January 20, 2022, stamped and signed on
March 20, 2023 by Luiza Petrovska, PE. This system is designed to accommodate the maximum daily flows for a 4
bedroom residence.
The OWTS consists of one sewer line from the residence. A 1,500 gallon, three-compartment, Valley Precast septic
tank with a total volume of 2000 gallons and an effluent filter in the outlet tee will be used. An Orenco PF5005 high
head pump assembly will be installed in the third compartment. The pump will be set to dose 90 gallons per dose.
The STA consists of a 2 foot unlined sand filter bed with four rows of 17 Infiltrator Quick4 Plus Standard chambers
for a total of 68. Gradation analysis should confirm that sand is secondary media. Install distribution lateral orifice
size, orifice shields, spacing, and flushing assemblies should be completed as designed. Inspection ports should be
present at each corner of the bed. Do not dig in wet or frozen ground. Location of the water line will be recorded in
the record drawing.
Contact Eagle County Environmental Health and the design engineer well in advance of requesting inspections prior
to backfilling any component of the OWTS. The design engineer is responsible for conducting all inspections
necessary to certify the installation and assure the functionality of the system. System certification, along with
photos and a record drawing is required to be submitted to and approved by Eagle County Environmental Health
prior to the use of the system or occupancy of the dwelling.
THIS PERIMT EXPIRES BY TIME LIMITATION AND BECOMES NULL AND VOID IF THE WORK AUTHORIZED BY THE PERMIT IS NOT COMMENCED WITHIN 120 DAYS
OF ISSUANCE, OR BEFORE THE EXPIRATION OF AN ASSOCIATED BUILDING PERMIT

Dear Steve:
At the request of the installer, Scot Cliver, Scott Development, on August 18, October 26, and
November 6, 2023 we visited your property on Lot 7, Block 1, Cordillera Filing No. 34, 1435 Gore Trail,
Eagle County, Colorado. The purpose of our site visits was to inspect the installation of the septic system.
The system was installed in overall compliance with the septic system design, Drawing No. 21-
3539OWTS.dwg, dated March 20, 2023. At the time of the inspection, we observed the following:
The building sewer line was connected to the east side of the residence, near the NE building corner.
The cleanout was 22 inches south of the NE building corner, and 3.7 feet east, as shown on the attached as-
built sketch. The building sewer line was about 29 feet of 4-inch diameter SDR35 PVC pipe. They installed
1500-gallons, three compartments, precast, concrete, septic tank by Valley Precast with an effluent filter in the
second compartment and an Orenco PF500511 high-head pump assembly in the third compartment. The inlet
of the tank had the sanitary Tee installed. The tank access ports had corrugated, plastic risers with secured,
green, plastic covers. The covers were secured with nuts, bolts, and screws. All risers were about 24-inch high.
The control panel was installed on the north side of the residence, at the NE corner.
The effluent line from the pump chamber to end manifold of the soil treatment area, was about 22 feet
long, 1.5-inch diameter, sch40 pvc pipe. The end manifold was about 8 feet long, 1.25-inch sch40 pvc. The soil
treatment area consisted of infiltrator chambers arranged in a bed configuration with four rows of 17 Quick
Four Plus Standard Infiltrator Chambers in each. Two feet of the onsite silty, clayey, soil with basalt rocks of
cobble and boulder size, was overexcavated from the bottom of the trenches and replaced with two feet of
secondary sand media (the graph is attached to this report). Pressure dosing system consisting of 68-ft, 1.25-
inch sch40 laterals perforated at the top with 1/8-inch holes at 4-ft intervals, suspended from the inside side of
each row of infiltrator chambers. The ends of the laterals were equipped with flushing assemblies. A squirt test
was performed during the inspection and it was satisfactory. Claire and Kurt from the ECEH Department were
present during the inspection and the squirt test as well. All inspection ports were installed (in the end caps of
the first and last chamber of each row of infiltrators) in the opening opposite the pressure distribution lines.
The septic tank installation and startup of the pump system were done by Valley Precast. Start up letter
attached.
There is no other warranty either expressed or implied.

Dear Steve,
On August 4 and 10, 2021, we visited your property on Lot 7, Block 1, Cordillera Filing
No. 34, 1435 Gore Trail, Edwards, Eagle County, Colorado. The purpose of our site visits was to
select an area on the property for the proposed Soil Treatment Area. During the second site visit,
the excavator Jose Gonzales with Blue Monster Services, dug three profile holes.
The OWTS is for a proposed 4-bedroom residence, and it will consist of a 1500-gallons,
three-compartments concrete septic tank by Valley Precast with an effluent filter, and an Orenco
Pump assembly, and the Soil Treatment Area will consist of 68 Quick 4 Plus, Standard Infiltrator
Chambers in a bed configuration with a 2-foot sand filter and pressure dosing.
The site plan by the architect Greg Macik, Tab Associates, Inc., was used as a basis for
the septic system design. Directions to the site from Edwards are as follows: from US Highway-
6 turn left onto Squaw Creek Road, continue for 3.1 miles to Fenno Drive, continue for 3.4
miles, left on Settlers Loop, continue for 0.3 mi, straight to Summit Trail for 0.9 miles left on
Gore Trail for 1.5 miles. The lot is on the left (north) side of the road, one lot before the cul-de-
sac.
! Preliminary Investigation
G Address: 1435 Gore Trail, Edwards, CO 81632.
G Legal Description: Lot 7, Block 1, Cordillera Filing No. 34, Eagle County,
Colorado.
G Existing Structures: There were no existing structures on the lot.
G Location of existing and proposed wells on property: There are no wells
on the property. It is services by public water.
G ECEHD (Eagle County Environmental Health Department) records: There
were no old record. The property was vacant.
G Topography: The topography of the leachfield site is moderate with an
average slope of about 11 percent. Drainage is to the north.

G Soil Data: The profile holes were observed on August 10, 2021. The soil
exposed in the profile holes was as follows:

A soil sample taken from Profile Holes 1 and 3 at 48-inches was classified
as Type 3, LTAR = 0.35 gpd/sf with 35% to 65% Rock (>2mm) CLAY
LOAM, R-1, Option 1 with 2 feet deep sand filter and pressure
distribution.

G Landscape position: The proposed STA will be located, in the
Summit/Shoulder Slope Position northwest from the proposed residence
(down gradient)
G Topography: The topography of the proposed leachfield site is moderate.
The rows of infiltrators in the bed will run along the contours.
G Vegetation: The vegetation within the proposed leachfield site consisted of
grass, aspen and aspen seedling trees. There were also basalt rocks on the
surface. The site surrounding the leachfield was similar.
G Natural and cultural features: The property is located within the Cordillera
Subdivision at The Summit, a residential subdivision in a gated
