Permitted Construction / Details:
Install the OWTS exactly as depicted in the HP Geotech design report dated October 15, 2015, stamped,
signed and dated November 3, 2015 by David A. Young, PE. The system is designed to serve a total of
seven bedrooms, 6 of which are in the main dwelling unit and another in anticipation of an accessory
dwelling.
NOTE: The septic tank and a 6", C900 PVC effluent line were installed by an unlicensed contractor and
without an OWTS permit and as such, must either be certified by the design engineer as being suitable
and properly installed or removed and replaced under the supervision of the design engineer. Prior to
installation, a survey must be performed to lay out system components in accordance with setback
requirements and an additional soil profile pit must be excavated on the downhill side of the lowest
trench to verify soil types.
The system consists of a 2,000 gallon Valley Precast septic tank with an effluent screen on the outlet tee;
and 3,000 square feet of soil treatment area (prior to taking the 30% adjustment for using chambers) via
the installation of 175 Infiltrator Quick 4 Plus Standard chambers placed in seven shallow trenches that
are serially connected with 25 chambers per row. Be sure to scarify the trench bottoms and side walls
prior to chamber placement.
Contact the design engineer at least 48 hours in advance of requesting inspections necessary to certify
the installation and to prepare the record drawing. Engineer certification of proper installation along with
the record drawings and photos are required to be submitted to, and approved by, Eagle County
Environmental Health prior to the use of the system or occupancy of the dwelling.

This letter is to inform you that the above-referenced, OWTS-8-16-15213 permit, has been
inspected and finalized. This OWTS permit was approved and sized to serve the 7 bedrooms, 6
of which are in the main dwelling unit and another in anticipation of an accessory dwelling.
Additional information about the maintenance your septic system needs can be accessed
through our website links, provided on the Environmental Health Department’s septic system
resource page.
Be aware that changes in the use of your property or alterations of your building may require
commensurate changes to, or relocation of, your septic system . Landscape features, trees with
tap roots, irrigation systems and parking areas above the soil treatment area can cause
premature system failure. It is equally important that you notice and immediately repair
dripping faucets and hissing toilets as this will certainly cause the system to fail.
