Install the new OWTS exactly as depicted in the LKP Engineering, Inc. design stamped, signed and dated October 14, 2021 by Luiza Petrovska, PE. The new system will be sized to accommodate the maximum daily flows for a 3 bedroom residence. Abandonment of the existing tank must be completed in conformance with ECPHA OWTS Regulation 43.9.C. This OWTS consists of a three-compartment, 1000 gallon Valley Precast septic tank with an effluent filter between the second and third compartment and an Orenco PF500511 high-head effluent pump in the third compartment. Set the floats to send 90 gallons of effluent to the soil treatment area (STA). The STA consists of 48 Infiltrator Quick 4 Plus Standard chambers arranged in a bed configuration with six rows of 8 chambers set atop two feet of secondary sand. Note: Gradation of the sand media use must be completed within one month prior to the installation and confirmed with the design engineer. If the sand does not conform to secondary or preferred sand media requirements, it cannot be used for the sand treatment media. Contact Eagle County Environmental Health and the design engineer well in advance of requesting inspections prior to backfilling any component of the OWTS. The design engineer is responsible for conducting all inspections necessary to certify the installation and assure the functionality of the system. System certification, along with photos and a record drawing is required to be submitted to and approved by Eagle County Environmental Health prior to the use of the system. (970) 390-0307 www.LKPEngineering.com September 18, 2022 Matthew Lou PO Box 3056 Vail, CO 81658 mattlou2@hotmail.com RE: Inspection of Septic System Installation OWTS – Replacement -3 Bedroom Residence Lot 1, Crowley Exemption 3940 Sweetwater Road Eagle County, Colorado Project No. 21-3549, Permit OWTS-025195-2022 Dear Matt: At the request of Travor Dunsdon, Alpha Excavation, LLC, your septic installer, on September 14, 2022, we met with you and Travor, and Claire Lewandowski, REHS, Eagle County Environmental Health Specialist III, on your property at Lot 1, Crowley Exemption, 3940 Sweetwater Road, Eagle County, Colorado. The purpose of our site visit was to inspect the installation of the new septic tank and the new Soil Treatment Area (STA) for the replacement of the old system. According to Travor, the old tank was completely removed (taken to the Eagle County Landfill) in conformance with ECPHA OWTS Regulation 43.9C. The new system was installed in overall compliance with the septic system design, Drawing No. 21- 3549OWTS.dwg, dated October 14, 2021. At the time of the inspections, we observed the following: They installed new, 1000-gallons, three compartment, top seam, precast, concrete, septic tank by Valley Precast with an Orenco PF500511 high-head pump assembly in the third compartment. The control panel for the pump was on the east wall of the existing residence, as shown on the attached, as-built sketch. New, 4-inch diameter ABS, black pipe (that replaced the old cast iron pipe) was used to connect the building sewer to the new septic tank. The access ports on the septic tank had variable heights at the time of the inspection, with 27” riser over the pump chamber, 30” riser over the middle compartment, and 32” riser over the inlet compartment. According to Travor, the height of the risers will be adjusted to fir the finish grade. The tank covers were secured with nuts, bolts, and screws. The effluent line from the pump chamber to the end manifold was about 33 feet, 1.5-inch diameter, sch40 pvc. The soil treatment area consisted of infiltrator chambers arranged in a bed with six rows of eight (8) Quick4 Plus, Standard, infiltrator chambers. Two feet of the onsite soil were overexcavated from the bottom of the STA and replaced with two feet of secondary sand media (the sand spec is attached to this report). Pressure dosing system consisting of 34-ft, 1.5-inch sch40 laterals perforated at the top with 5/32-inch holes at 4-ft intervals, suspended from top of the infiltrator chambers. The ends of the laterals were equipped with flushing assemblies. A squirt test was performed during the inspection.
